CPT 90839: psychotherapy for crisis, first 60 minutes
90839 reports psychotherapy for a patient in crisis, counted as total face-to-face time with the patient and family, even if the time is not continuous. It covers the first 60 minutes, and the add-on 90840 covers each further 30 minutes.
In short: 90839 counts total crisis time with the patient or family, and the clinician must give full attention. It is not reported with 90791 or 90792 on the same date.
What does CPT 90839 report?
90839 is psychotherapy for crisis, covering the first 60 minutes. The CMS fee schedule file labels it "Psytx crisis initial 60 min", and the add-on 90840 covers each additional 30 minutes, according to the CMS telehealth list label "Psytx crisis ea addl 30 min".

MLN1986542 notes that clinical psychologists, clinical social workers, marriage and family therapists and mental health counselors can bill this code, in addition to physicians and the other practitioners in the booklet.
How is time counted for 90839?
Time is the total face-to-face time with the patient, the family, or both, spent on the crisis, even if it is not continuous. The First Coast Service Options billing article A57520 adds that the clinician must give the patient full attention and cannot serve another patient during that time.
The patient must be present for all or some of the service. The note should therefore show who was seen, when each period started and stopped, and why the visit was a crisis rather than a scheduled session.
A therapist sees a patient for 40 minutes at midday, then meets with the patient's parent for 35 minutes that afternoon to plan safe next steps. The two periods belong to the same crisis, so the time is added together before the codes are chosen.
What cannot be billed with 90839?
The contractor article says not to report 90839 with 90791 or 90792. NCCI Policy Manual Chapter XI, section C, goes further and bars the diagnostic evaluation codes with crisis psychotherapy for the same date of service.
Where does 90839 apply, and what are G0017 and G0018?
G0017 and G0018 are the HCPCS Level II counterparts of 90839 and 90840. The October 2026 HCPCS file describes G0017 as psychotherapy for crisis furnished in an applicable site of service, meaning any place of service where the non-facility rate for crisis psychotherapy applies, other than the office, for the first 60 minutes. G0018 is each additional 30 minutes.
MLN1986542 lists G0017 and G0018 as "psychotherapy for crisis in an applicable site of service". Check which pair your payer expects for an off-site crisis visit before you build the claim.
90839 is on the CY 2026 CMS telehealth list as a maintained code, and POS 02 or POS 10 applies for telehealth, per the CMS telehealth FAQ. The mental health billing page covers the plan-level telehealth rules.
How does 90839 appear on the claim?
On the CMS-1500, Item 24D carries 90839 and Item 24G carries 1 unit, since the MUE is 1 per date of service. Time beyond the first 60 minutes goes on its own line with the add-on code. Item 24E points to the crisis diagnosis, and the note needs the actual times.
How does 90839 appear on the claim form?
A crisis session is one line with 90839 in Item 24D and 1 unit in Item 24G. If the crisis runs past the first hour, the add-on goes on its own line. Item 24E points to the crisis diagnosis, and Item 24A holds the single date of service.
RVU26D gives 90839 status A, work RVU 3.58 and global XXX, and the MUE table limits it to 1 per date with indicator 2, a policy edit. Because the edit is policy based, a second crisis session on the same date is not fixed by adding a modifier.
Example: A patient in acute distress is seen from 9:15 to 10:20 on 10/09. The line is 90839, pointer A (F32.A), 1 unit, $260.00. The note lists the start and stop times, the safety assessment and the plan. If the chart lacks times, the payer may question the claim under CO-16.
Why is 90839 denied?
| Code | What X12 says | What usually causes it |
|---|---|---|
| CO-97 | The benefit is included in the payment for another service already adjudicated | 90839 billed with 90791 or a scheduled psychotherapy code on one date |
| CO-50 | Not deemed a medical necessity by the payer | The note does not describe a crisis state |
| CO-5 | The procedure code or bill type is inconsistent with the place of service | Place of service does not match the code the plan expects, such as an off-site visit billed as 90839 |
| CO-197 | Precertification, authorization or notification absent | A prior authorization or notification the plan requires for the visit |
| CO-24 | Charges are covered under a capitation or managed care plan | Behavioral health is carved out to a separate vendor |

01When can I bill crisis psychotherapy 90839?
Bill 90839 when you spend face-to-face time with a patient, a family member or both on a crisis, and the clinician gives full attention for the first 60 minutes. The patient must be present for all or some of it, and the note should show the times.
02Is 90839 billed by the hour or by total time?
By total time. The First Coast Service Options billing article A57520 says the codes report the total duration of face-to-face time with the patient and family spent on the crisis, even if it is not continuous. The first 60 minutes are 90839, and each additional 30 minutes is 90840.
03Can 90839 be billed with 90791 on the same day?
No. The contractor article says not to report 90839 with 90791 or 90792, and NCCI Policy Manual Chapter XI says the diagnostic evaluation codes are not separately reportable with crisis psychotherapy on the same date of service. Bill the crisis visit alone.
04How many units of 90839 does Medicare allow per day?
The practitioner MUE table effective October 1, 2026 lists 90839 at 1 unit per date of service, with adjudication indicator 2, a policy based date of service edit. Report time beyond the first 60 minutes with the add-on code 90840 on its own line, not with a second unit of 90839.
05Who can bill 90839 under Medicare?
MLN1986542 says clinical psychologists, clinical social workers, marriage and family therapists and mental health counselors can bill 90839 and 90840, alongside the physicians and other practitioners it lists. The booklet marks 90839 and 90840 with a dagger for these four license types. State licensure still applies to each clinician.
06Is 90839 a Medicare telehealth code?
Yes. It is on the CY 2026 CMS telehealth services list with the action maintain, and MLN1986542 marks it as a current telehealth code. Use POS 02 when the patient is not at home and POS 10 when the patient is at home.
