Editorial policy: how we write and review our pages

Billing rules change, and a wrong deadline costs real money. This page explains where our pages get their facts, who writes and reviews them, and how to tell us about a mistake.

In short: every rule, code, deadline and fee on our pages comes from an official source named on that page. Each page shows its author, a reviewer line, a last-reviewed date and its sources, and errors are corrected and re-dated.

Where do the facts on our pages come from?

Codes, rules, deadlines, fees and filing limits come from official sources only. That means CMS and CMS MLN, the Federal Register, the eCFR, NUBC, NUCC, X12, CDC and NCHS for ICD-10-CM, state Medicaid agencies, state Departments of Insurance, and a payer's own provider manual.

We do not use vendor blogs, law-firm summaries or forum posts as the source of a rule. If we cannot open and verify a fact in an official source, we leave it off the page.

Each page lists the sources it used in a Sources block at the bottom, with the publisher named and a link to the original. Key facts also name the source inside the sentence.

CPT descriptors belong to the American Medical Association, so we paraphrase them in plain words. HCPCS Level II, CARC, RARC and ICD-10-CM descriptors are public, and we quote them short.

How is a page written and reviewed?

1
Research
The writer opens the official sources for the topic and records each fact with its link and the line that supports it.
2
Write
The page is written in plain English for a billing lead or hospital administrator. Company claims use only numbers we have approved, worded the same way everywhere.
3
Check
Automated checks look for banned claims, mismatched company numbers and deleted codes. Any worked example with deadlines is calculated with a business-day tool, not by hand.
4
Review and date
A reviewer reads the page against its sources, and the page is stamped with a last-reviewed date.

Who writes our pages?

Pages are written by Salman Saeed, CEO and Co-Founder of Nucleus Align. His bio is on the team page.

Each page carries a reviewer line reading Reviewed by a Certified Medical Coder. That role is open until a certified coder joins as reviewer, and we will name that person here and on the pages when they do. We do not list reviewers who do not exist.

What does last reviewed mean?

The last-reviewed date is the day someone last checked the page against its official sources. It is not the day the page was first published and it does not change when we fix a typo.

When a source changes, such as a new CMS fee schedule or a Federal Register rule, the affected pages are re-checked and the date is updated. If a page has not been re-checked in a long time, treat its numbers as a starting point and confirm them at the source linked on the page.

How do you handle corrections?

If you find an error, email hello@nucleusalign.com with the page address and the source that shows the correct fact. We check it against the official source, fix the page and update the last-reviewed date.

We correct mistakes in place rather than quietly deleting the page, so the facts you read are the current ones.

What are our pages not?

Our pages explain billing rules in general. They are not legal, medical or financial advice, and they do not replace your payer contract or your counsel. Read more in our terms of use. Our own claims about how we work are in about us, and you can browse the guides, rules and glossary to see the process in practice.

01Who writes the pages on nucleusalign.com?

Salman Saeed, CEO and Co-Founder of Nucleus Align, is shown as the author of our pages, and his bio is on the team page. Every fact is checked against official sources before it is published, and each page lists those sources at the bottom.

02Who reviews your pages?

Each page shows a reviewer line reading Reviewed by a Certified Medical Coder. That role is open until a certified coder is hired for it. We will name the person here and on the pages once they join, and we do not invent reviewers in the meantime.

03What sources do you use?

Official sources only: CMS and CMS MLN, the Federal Register, the eCFR, NUBC, NUCC, X12, CDC and NCHS for ICD-10-CM, state Medicaid agencies, state Departments of Insurance and payers' own provider manuals. We do not take rules from vendor blogs or forums.

04What does the last reviewed date mean?

It is the date a page was last checked against its official sources. It moves when we re-verify the facts, such as after a rule or fee schedule changes, and not when we correct a typo. Confirm time-sensitive numbers at the source linked on the page.

05How do I report an error on a page?

Email hello@nucleusalign.com with the page address and the official source that shows the correct fact. We check it against that source, fix the page and update its last-reviewed date. We correct pages in place, so readers always see the current facts.
