CPT 96127: brief emotional or behavioral assessment, billed per instrument

96127 reports a scored brief emotional or behavioral assessment, and each standardized instrument is one unit. That makes unit count the main billing detail, with an MUE of 4 per date in the Medicare file.

In short: 96127 is billed once per scored standardized instrument, so three forms means 3 units. The unit count and a scored form in the chart are what payers check.

What does CPT 96127 report?

96127 reports a brief emotional or behavioral assessment, such as a depression or attention questionnaire, that is scored and documented. It is billed once for each standardized instrument, so a visit with two instruments carries two units.

When is 96127 billable, and when is it not?

Bill it when a standardized instrument was given, scored and the result written in the record. Each separate instrument is its own unit.

Children are the common case. South Dakota Medicaid, for example, says brief emotional or behavioral assessments should be completed with every well-child visit and must be billed with 96127. Its manual also lists 96127 and 96161 for perinatal depression screening, and says maternal depression screening of an infant's mother is billed with 96161 under the child's Medicaid ID. That is one state's rule; check your own state's manual.

96127 is different from a developmental screen. The CMS file lists 96110 as status N, so Medicare does not pay it, while 96127 is status A. See the pediatric billing guide for the well-child sequence, and mental health billing and behavioral health billing for adult use.

How do units and modifiers work on the claim?

On the CMS-1500, Item 24D carries 96127 and Item 24G carries the number of instruments. Chapter 26 says that when multiple services are provided, Item 24G holds the actual number, so three scored forms means 3 units on one line.

With a well-child visit such as CPT 99392, the screen is a separate service. South Dakota Medicaid tells providers to append modifier 25 to a problem-oriented E/M billed with a preventive visit. That modifier belongs on the E/M, not on 96127.

The practitioner MUE table effective October 1, 2026 lists 96127 at 4 units per date, adjudication indicator 3, a date of service edit based on clinical data. Indicator 3 differs from the policy edit on the newborn codes, so confirm how your payer applies it.

Which fields carry 96127 and how many units?

96127 is counted by instrument, so Item 24G is the field that decides the payment. Chapter 26 says that when multiple services are provided you enter the actual number, so three scored questionnaires means 3 units on one line, not three lines of 1. Item 24E points to the reason for the screen, such as a developmental concern or a depression screening encounter.

RVU26D gives the code status A with a work RVU of 0.00 and global XXX. The MUE table caps it at 4 units per date. A fifth instrument on one date would need a separate clinical reason.

Example: At a 4-year well visit on 10/13 the nurse scores a developmental screen and a parent anxiety form. Line 1 is 99392, line 2 is 96127 with 2 units at $14.00 each, pointer A. The chart holds both scored forms. If the line shows 5 units, the MUE table would stop the extra unit and the claim could draw CO-151.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01How many units of 96127 can I bill?

One unit per scored standardized instrument. The Medicare MUE table effective October 1, 2026 lists 96127 at 4 per date with adjudication indicator 3, a clinical data edit. Your payer may set a lower cap, so check its policy before billing more than two instruments.

02Does 96127 need modifier 25?

No. Modifier 25 goes on a separate E/M billed the same day as a preventive visit, not on 96127. The screen is a separate service line with its own units, and the modifier placement is checked against the payer's policy.

03Can I bill 96127 with a well-child visit?

Often yes, but payers differ. South Dakota Medicaid requires it with every well-child visit, while other plans treat it as included. Test each plan and keep the scored form in the chart, with the instrument name and the score, so a payer review can match each unit to a form.

04What is the difference between 96127 and 96110?

96127 is a brief emotional or behavioral instrument. 96110 is a developmental screen. The CMS RVU26D file lists 96127 as status A and 96110 as status N, so Medicare does not pay 96110. Check each commercial plan separately, since they differ.

05Does 96127 apply to adults?

Yes in some programs. South Dakota Medicaid lists 96127 in its preventive services table for adult depression screening and for perinatal depression interventions, billed in line with that manual. Check your payer, because coverage, age and unit rules differ by plan.
