Praxis EMR medical billing and RCM support
Praxis EMR has no billing module of its own, so billing for a Praxis practice runs in a separate system that receives charges from the EHR. An outside billing company can work that system on named logins the practice issues, after a BAA is signed.
In short: Praxis keeps charting and billing apart, so the work is the charge handoff and the billing system behind it. We work the queues you hand over, from named users with two-step login. Nucleus Align has no partnership with Praxis.
What is Praxis EMR and who uses it?
Praxis EMR is a template-free EHR from Infor-Med Inc, built around what it calls Concept Processing. Its own site describes the AI inside Praxis as learning from each provider, so a clinician charts in their own words instead of filling templates.
The vendor lists family practice, pediatrics, internal medicine, gastroenterology, podiatry, surgery, urology, women's health and urgent care and emergency medicine among its specialties, and says it supports concierge and membership models. It runs on cloud hosting, VMware servers or traditional server setups.
| Name on Praxis's site | What Praxis says it is | Why a biller cares |
|---|---|---|
| Concept Processor | The AI technology that learns how each provider documents and treats | The note is narrative, so ask how a visit becomes a coded charge |
| Datum+ | Discrete data embedded inside the provider's own text for reporting | Quality reporting data, not a claim report |
| Billing interfaces | HL7 connections to billing and practice management systems | The route every charge takes out of Praxis |
Does Praxis EMR have its own billing module?
No. Praxis states that it will never offer billing services and that it integrates with the billing system a practice chooses. Its Billing Freedom page gives three pathways: in-house billing software, a professional billing service, or the billing system the practice already uses.
That makes the handoff the center of billing in a Praxis practice. Charts live in Praxis, while charges, claims and remittances live in a second system, so most billing errors we look for show up where the two systems meet.
Can an outside billing company work inside Praxis EMR?
Yes. Praxis's Billing Partners page lists outside billing services next to billing software, and tells practices to contact Praxis if their preferred biller is not on the list. Nucleus Align has no partnership with Praxis and is not on that list.
Because billing runs in a separate system, our daily work usually sits in the practice's billing or practice management system. Whether we also get chart access in Praxis is the practice's call, and we ask only for what a billing task needs, under a signed business associate agreement.
What access does a billing partner need in Praxis EMR?
Named users with limited roles, created by the practice, in each system we touch. Nobody on our side shares a login.
How do claims move from Praxis EMR to the clearinghouse and payer?
A charge leaves Praxis through an interface to the billing system, and the billing system sends the claim to the payer through a clearinghouse. Praxis says its interfaces use HL7 and that all billing products use some form of clearinghouse.
Praxis's pages do not say which fields cross the interface, so we ask for them at setup: rendering provider, diagnosis and procedure codes, modifiers, place of service and units. We then compare a sample of visits against what arrived in billing.
From there the electronic professional claim follows the X12 837 standard, version 005010X222, which 45 CFR 162.1102 adopts under HIPAA. Paper professional claims use the CMS-1500.
How does Praxis EMR post ERA and 835 remittances?
Praxis's pages do not describe remittance posting, which fits a system with no billing module. Payment posting happens in the billing system that receives the ERA, and we post there.
The electronic remittance advice is the X12 835 version 005010X221 under 45 CFR 162.1602. We post each payment against the claim, keep contractual adjustments apart from patient responsibility, and send short pays and unexplained adjustments to the denial queue.
What do we do for a Praxis EMR practice?
- Review each charge that arrives from Praxis for diagnosis, procedure, modifier and provider before the claim goes out
- Edit and submit claims through the practice's clearinghouse, within 24 hours of charge entry
- Post ERA 835 payments and adjustments in the billing system
- Work denial queues within 48 hours and appeal with the chart note attached
- Follow up open accounts receivable by payer and age
- Send patient statements and answer balance questions
Which Praxis EMR reports matter for denial and A/R tracking?
Denial and A/R numbers come from the billing system, because that is where claims and payments sit. Praxis's pages name clinical and quality reporting tools, including Practice Guidelines and Queries and automated MIPS and ACO reporting, but no billing report.
- Charge lag: days between the visit and charge entry
- Claim status: unbilled, held, sent, rejected or paid
- A/R aging by payer, tracked as days in A/R
- Denial trends: payers, reason codes and providers that repeat
- Payment variance: paid amount against the contract or expected amount
Praxis's query tool lets a provider query any clinical parameter, so we ask your administrator whether a list of completed visits can be pulled for the reconciliation above.
Which billing systems does Praxis EMR connect to?
Praxis's Interfaces page lists dozens of billing and practice management systems, and its Billing Partners page lists billing services. The names below appear on those pages and also have a page here.
| System named on Praxis's lists | Where we describe working in it |
|---|---|
| Athena Health | athenahealth |
| CollaborateMD | CollaborateMD |
| Tebra billing software | Tebra |
| Medisoft | Medisoft |
| Medent and Azalea Health Innovations | Both are on the interface list; see the EHR systems page |
The system a practice uses decides where claim edits, posting and A/R reports live, so we confirm it first.
What billing mistakes are common in Praxis EMR?
Praxis's pages do not list billing defects, so we describe the checks we run on any EHR that hands charges to a separate billing system. None of them is a claim about Praxis.
| Check | Why it matters |
|---|---|
| Visits with no charge in billing | The only copy of the visit is in the chart, and the filing clock runs anyway |
| Provider and NPI match | Two systems hold provider records; a mismatch rejects the claim |
| Demographics and insurance match | A patient updated in one system and not the other produces eligibility denials |
| Direct pay or membership visits | These should not create insurance claims |
| Narrative note against the code | The code on the claim has to be supported by what the note says |

Common questions
Straight answers from the team that does the work.
Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.
01Can an outside billing company work inside Praxis EMR?
Yes. Praxis's Billing Partners page lists outside billing services and says to contact the vendor if your biller is not listed. Our staff work on named logins the practice issues after a BAA is signed, mostly in the billing system. Nucleus Align has no partnership with Praxis.
02What access does a billing partner need in Praxis EMR?
Named users with role-based access, created by the practice, in the billing system and in Praxis only if chart access is needed. We sign the BAA first, use two-step login on every system and share no logins. The practice decides how much chart access to give and can remove it at any time.
03Which Praxis EMR reports matter for denial and A/R tracking?
Praxis's pages name clinical, quality, MIPS and ACO reporting, not billing reports. Denial and A/R numbers come from the billing system: charge lag, claim status, A/R aging by payer, denial trends and payment variance. We map each to the report name your system uses.
04How do claims move from Praxis EMR to the clearinghouse and payer?
The charge goes from Praxis to the billing system through an interface that Praxis says uses HL7. The billing system sends the claim through a clearinghouse to the payer. Praxis does not say which fields cross, so we ask for them at setup and compare sample visits.
05How does Praxis EMR post ERA and 835 remittances?
Praxis's pages do not describe remittance posting, so remittance work follows whichever billing system you use and we confirm that route at setup. Posting happens in the billing system that receives the ERA. The 835 standard is 005010X221 under 45 CFR 162.1602. We post each payment, separate contractual adjustments from patient balances and send short pays to the denial queue.
06What billing mistakes are common in Praxis EMR (charge capture, modifiers, demographics)?
Praxis's pages list none, so we check for the usual two-system gaps: visits with no charge, provider and NPI mismatches, demographics or insurance that differ between systems, and notes that do not support the code. These are checks we run on any EHR with a separate billing system.
07Does Praxis EMR include practice management or billing?
No. Praxis says it focuses on the EMR and will never offer billing services. It lists billing and practice management systems it connects to and describes three pathways: in-house billing software, a professional billing service, or the system you already use.
08Does Praxis EMR support direct pay and concierge practices?
Praxis says its integrated billing options cover both insurance and direct pay models and that it supports membership and concierge medicine. For billing, we confirm which visits should create insurance claims and which should not, so direct pay visits never reach a payer.
09Is there a billing company for Praxis EMR practices?
Yes. Nucleus Align supports practices on Praxis EMR, which has no billing module, so we agree a charge handoff with you and then handle claims, remittance posting and denial work. Claims go out within 24 hours of charge entry and denials are worked within 48 hours.
10Medical billing service for practices that use Praxis EMR
Nucleus Align takes on the billing Praxis EMR does not include: receiving charges, submitting claims through a clearinghouse, posting remits and working denials. You keep your documentation in Praxis EMR, and we agree scope and reporting with you at setup. You can book a call on our contact page to walk through your setup.
11Can an outsourced team use our Praxis EMR logins?
If our team needs to see anything in Praxis EMR, you create a named user for each person with only the access required. No shared logins, a BAA comes first, and our staff sign in with two-step login. You can end access at any time.
