Medisoft Billing and RCM Support for Small Practices
An outside billing team can work inside CGM MEDISOFT through named accounts you create under a signed BAA. We start by confirming your version, edition and clearinghouse connection, because CGM supports only recent versions and ties several features to eMEDIX.
In short: outside billing teams can work in CGM MEDISOFT from named, two-step accounts under a BAA. We confirm the version, the Basic or Advanced edition and the eMEDIX connection first, then run charges, claims, ERA posting, denials and A/R.
What is CGM MEDISOFT, and who runs it?
CGM MEDISOFT is a practice management system from CompuGroup Medical (CGM) that handles scheduling, charge entry, claims, remittances and patient statements. CGM's page titles it "Practice Management and EHR". CGM's wider portfolio is on our CompuGroup Medical page.
CGM releases a new version every year, and v31 is the latest on its page. CGM's upgrade matrix says versions before v30 are no longer supported by the developer, so we ask your version first.
The v31 brochure says the Basic edition is being phased out. Advanced has full patient accounting and revenue cycle tools, MultiLink codes and an AR Tracker with AR status codes and tasks. Basic offered only limited versions of the first two.
CGM says MEDISOFT can interface with an EHR such as CGM APRIMA. See every EHR system we work in.
How do claims move from MEDISOFT to the clearinghouse and payer?
A procedure or MultiLink code on an appointment becomes a ready-to-post charge after check-out, and the claim then goes to a clearinghouse before the payer. CGM's brochure names eMEDIX as the only approved and fully supported clearinghouse for CGM MEDISOFT.
If your claims go out another way, we ask how it was approved and who supports it. CGM's matrix marks several features as available only with eMEDIX.
CGM's MEDISOFT page also opens with a banner asking if MEDISOFT PM users are impacted by Optum's EDI changes. If that applies to you, we ask which connection is live and whether files were held.
Which CMS-1500 items matter most when MEDISOFT builds a claim?
These are the items we check on a small-practice claim. The rules come from the CMS Claims Processing Manual, chapter 26, which gives Medicare's instructions. Commercial payers publish their own.
| CMS-1500 item | What chapter 26 says |
|---|---|
| Item 21 | Enter up to 12 diagnosis codes, shown on lines A to L, at the highest level of specificity, with no period in the code. |
| Item 24B | Enter the place of service code for each line. This is a required field. |
| Item 24D | Procedure code with up to four modifiers. An unlisted or not otherwise classified code needs a narrative in item 19 or an attachment. |
| Item 24E | Required. One diagnosis letter per service line, taken from item 21. |
| Shaded area of item 24 | Holds supplemental data, not extra service lines. For Medicaid drug rebates, the NDC goes in with the N4 qualifier, then the 11-digit number, and the quantity qualifier is UN, F2, GR or ML. |
| Item 32 | Name, address and ZIP of the service location. Only one fits in the block, so a second location needs a separate claim. |
| Item 33a | The NPI of the billing provider or group. Required. |
CGM's matrix lists an option to limit diagnosis pointers in Transaction Entry, which fits the 24E rule, and Transaction Entry Alerts that warn of date-sensitive entries such as outdated codes.
Definitions: place of service, modifier, NDC, NPI.
How does MEDISOFT catch charges that never became claims?
CGM's v31 adds Create Unprocessed Charges. It adds charges for every checked-out appointment at once, using each visit's Procedure and MultiLink codes, and flags anything it cannot process cleanly.
A visit with no charge never reaches a payer, so we run the same test as a routine check: compare checked-out appointments with charges entered, work the flagged items and watch charge lag. The brochure says MultiLink codes, which the function uses, are not in Basic.
CGM also lists a visual flag for patients with multiple open cases, which it says helps avoid mis-posted transactions. We confirm the case on a charge before posting.
How does MEDISOFT post ERA and 835 remittances?
Posting happens in Revenue Management, where CGM says v31 lets staff assign AR status codes and AR notes directly within an ERA. The brochure says updates save to the AR Tracker and the claim, and ERA codes can be mapped once to AR status codes.
A new preference checks patient responsibility lines when you post a secondary remit. The ERA carries the payer's adjustments as CARC and RARC codes, and we read every one rather than rely on the automatic check.
Unpaid and short-paid lines move to accounts receivable follow-up.
How do patient statements and payments work in MEDISOFT?
CGM's matrix lists statements through eMEDIX, including a QR code for online patient payment and improved delivery preferences. It marks fully integrated eMEDIX statements and CGM PAY, CGM's payment product, as priced separately.
The matrix also lists a write-off tool that sorts balances by responsible party. We separate insurance from patient balances before statements go out. See our patient billing page.
What access does a billing partner need in MEDISOFT?
We need a named user for each team member, created by you and limited to billing roles, after a BAA is signed.
Data protection is on our security page: HIPAA training and NDAs for all staff, encrypted automatic backups and a dedicated compliance officer.
What do we do inside MEDISOFT?
- Charge review against the visit, catching a missing diagnosis or modifier.
- Claim edits and submission through your clearinghouse, with claims sent within 24 hours of charge entry.
- ERA posting in Revenue Management, with each CARC read against the contract.
- Denial work queues, with denials worked within 48 hours.
- AR follow-up using the AR Tracker, by payer and age.
- Patient statements on balances left after insurance.
Which MEDISOFT reports matter for denial and A/R tracking?
CGM's matrix lists an AR tracker for working AR balances, a financial KPI dashboard of charges, adjustments and payments year over year, and a patient encounters dashboard. We agree report names with you at setup.
| What we need to see | Why |
|---|---|
| Charge lag, from visit date to charge entered | Late charges delay every payment behind them |
| Claim status by payer | Shows what is unsent, rejected, pending or paid |
| A/R aging by payer | Shows where money is stuck and which payer to call first |
| Denial trends by reason and payer | Shows whether one front-end fix would remove a group of denials |
| Payment variance against contract | Flags underpayments early |
What billing mistakes are common in MEDISOFT?
We attribute no defects to the vendor. These checks come from what CGM documents, plus general checks.
| Check | Why it matters |
|---|---|
| Version and edition: pre-v30 versions are unsupported by the developer, and Basic lacks MultiLink and the AR Tracker | The screens and fixes available depend on it |
| Attachment info: v31 adds it at claim level, with case-level settings as fallback | Payers on one case can need different attachments |
| Clearinghouse connection after any EDI change, including the Optum notice on CGM's page | A routing change can hold a whole batch |
Which practices and specialties use MEDISOFT?
CGM's product page lists no specialties. See family medicine billing, internal medicine billing and pain management billing.
Similar systems: Crystal PM, Office Ally, CollaborateMD.

Common questions
Straight answers from the team that does the work.
Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.
01Can an outside billing company work inside Medisoft?
Yes, if you give it access. Your MEDISOFT administrator creates a named user for each person on our team, limited to billing roles, after a BAA is signed. We never share logins. Scope and reporting are agreed with you at setup.
02What access does a billing partner need in Medisoft?
A named user for each team member, limited to charge, claim, Revenue Management and report screens, with two-step login. We also need named access to your eMEDIX account and payer portals. You control every account, and the BAA comes first.
03Which Medisoft reports matter for denial and A/R tracking?
We need charge lag, claim status by payer, A/R aging by payer, denial trends and payment variance against contract. CGM's matrix lists an AR tracker and KPI dashboards for charges, adjustments and payments. We agree the exact report names with you at setup.
04How do claims move from Medisoft to the clearinghouse and payer?
The charge becomes a claim, which goes to a clearinghouse and then the payer. CGM's brochure names eMEDIX as the only approved and fully supported clearinghouse for MEDISOFT. If you send another way, we ask how it was approved before we start.
05How does Medisoft post ERA and 835 remittances?
CGM says v31 lets staff assign AR status codes and notes inside the ERA, saved to the AR Tracker and the claim, and can check patient responsibility lines on secondary remits. We still read every CARC and RARC ourselves before posting.
06What billing mistakes are common in Medisoft (charge capture, modifiers, demographics)?
We do not blame the vendor. We check version and edition, unprocessed charges, the case on each charge, up to four modifiers per line, one diagnosis letter per line and claim attachment info. CGM documents several of these as v31 features, so we confirm which you have.
07Which Medisoft version do you need to know before starting?
We ask for the version and the edition. CGM's matrix says versions before v30 are no longer supported by the developer, and its brochure says Basic is being phased out. Several v31 features rely on tools that Basic does not include.
08How are patient statements handled for Medisoft practices?
CGM lists statements through eMEDIX, including a QR code for online payment, and CGM PAY as a separately priced option. We send statements only after insurance is resolved, so patients are not billed for balances still in dispute with a payer.
09Is there a medical billing company for Medisoft users?
Yes. Nucleus Align takes on billing for practices that run MEDISOFT. We work in your MEDISOFT with named logins you grant, review charges, send claims within 24 hours of charge entry, post remittances and work denials within 48 hours. Scope and reporting are agreed with you at setup.
10Can you bill from Medisoft without moving us to a new system?
Yes. Nucleus Align works in the MEDISOFT you already use, so your charge entry, patient accounts and claim path stay where they are. You keep ownership of the system and its data, and you decide which roles our team receives. Book a call and we map the handoff.
11Can an outsourced billing team use our Medisoft logins?
No shared logins. Your MEDISOFT administrator creates a named user for each Nucleus Align team member, limited to billing roles, with two-step login on our side and a BAA signed first. You can review or remove access whenever you choose. Nothing changes in how your team documents care.
