Crystal PM Billing Services for Optometry and Eye Care Practices

We work inside your Crystal PM account through named, two-step accounts you control, after a signed BAA. We check medical and routine vision claims, send them through your claim path, post remittances and work denials and A/R.

In short: we work in your Crystal PM account with our own named logins, in the queues you assign, and report from your own data. We claim no partnership with the vendor, and the Medicare eye care rules here come from two CMS manuals.

Already on this system? We work in it. No EHR, or tired of paying for one? Our billing clients get EHR or PM software free.

What Is Crystal PM and Who Bills Out of It?

Crystal PM, from Crystal Practice Management, is practice management and EHR software for eye care, mainly optometry. The vendor's site calls it a platform for independent optometry practices and says 8,000+ optometry providers use it.

Its pages say the system is most widely used in optometry, vision therapy and myopia management, from single locations to groups and optical shops. It is an EHR and practice management system in one: scheduling, records, billing, payments, optical inventory and a patient portal.

The vendor offers a cloud version and a leased in-office server with the same features.

The vendor also runs its own billing service for Crystal PM users. If you use it, settle who owns each claim first. For the specialty side, see our ophthalmology billing page. See also EHR systems, CollaborateMD and ModMed.

Can an Outside Billing Company Work Inside Crystal PM?

Yes, if your practice grants access. The vendor's own billing team works the same way: its FAQ says the practice gives secure access to its Crystal PM environment so billers can post payments, manage claims and run reports.

1
Sign the BAA First
Nucleus Align signs a Business Associate Agreement (BAA) before any record is opened. Our staff have HIPAA training and NDAs.
2
Name Your Hosting
Tell us whether you run the cloud version or an in-office server. A server practice may need a VPN for remote log-in.
3
Your Administrator Adds Named Users
Each team member gets a separate login with role-based access, never a shared one.
4
Turn on Two-Step Login
Every account we use has two-step login, including clearinghouse and payer portals.
5
Map the Claim Path
We note how claims leave Crystal PM, which portals we may open, and who owns each work queue.
6
Agree Scope and Reporting
Scope and reporting are agreed at setup. Our security page covers data protection.

How Do Claims Move from Crystal PM to the Payer?

A routing slip carries the diagnosis and CPT codes to checkout, and the charge becomes an invoice and a claim. The vendor says electronic routing slips track the diagnosis, CPT codes, product orders and recalls.

On the way out, Crystal PM lists Integrated Claims Processing in every plan. Its partner page names several insurance services companies, among them Apex EDI, Trizetto, SpecCheck and Spexbot. The page does not say which one a practice uses, so we confirm it at setup.

StepWhat the vendor saysWhat we check
Charting to codesEHR has built-in ICD-10 codes and auto-populates billing codesCodes match the exam note and the diagnosis supports the service
Claim submissionElectronic claims for medical and vision insuranceWhich plan gets the claim: vision plan or medical plan
RemittanceThe Plus plan and above lists automated importing of EOBsPosting against the payer's ERA and EOB, with short pays flagged
Patient balancePayments post to the patient ledger; text and email pay links; QR codes for mailed statementsBalance after insurance matches the remittance

Professional claims follow the CMS-1500 data set. Optical sales sit in the same system, so we keep exam charges and optical charges apart in review.

Is It a Medical Claim or a Routine Vision Claim?

That choice drives everything after the exam. Medicare does not cover eye examinations done to prescribe, fit or change eyeglasses, or any refraction, according to the Medicare Benefit Policy Manual, chapter 16, section 90.

The same section says the exclusion does not reach physician services performed with an eye disease such as glaucoma or cataracts, and that the routine checkup exclusion does not apply to glaucoma screening exams.

SituationBilling pathSource
Exam only to prescribe, fit or change glassesNot covered by Medicare; handled under the patient's vision plan or as a patient balanceBenefit Policy Manual ch. 16, section 90
Refraction, any reasonMedicare excludes all refractive procedures, whoever performs themBenefit Policy Manual ch. 16, section 90
Visit tied to an eye disease such as glaucoma or cataractMedical claim; the exclusion does not applyBenefit Policy Manual ch. 16, section 90
Visit inside a surgical global periodNeeds the right modifier or the global surgery edit can deny itClaims Processing Manual ch. 12, section 40.3

The vendor says its billers handle both vision and medical claims. Commercial plans can differ from Medicare, so we check each payer's own rules before a claim goes out.

Which Eye Visit Codes and Modifiers Cause Trouble?

Under the Medicare Claims Processing Manual, chapter 12, section 40.3, MACs edit visits billed during a surgery's global period. The eye visit codes 92012 and 92014 sit in that edit list with standard E/M visit codes.

The same section excludes 92002 and 92004, listed there with the new-patient E/M codes, from those edits. Services billed in the global period without modifier 24, 25, 57, 78 or 79 are the ones edited.

We do not copy AMA code descriptors. When a practice bills an eye visit code against a general E/M code, we check the note, the diagnosis and the payer's own policy. For code detail, see CPT 92014 and how a modifier works.

What Billing Mistakes Are Common in Crystal PM?

These are checks we run in any eye care account, not vendor defects.

AreaWhat we check
Charge captureEvery exam, test and procedure on the routing slip became a charge, and optical items did not leak into medical claims.
Visit typeRoutine exam versus medical visit chosen from the note, with the diagnosis that supports it.
ModifiersRight and left eye, and modifier 24, 25 or 57 where the global surgery rules call for them.
Demographics and eligibilityName, birth date and member ID match the card and the eligibility response, for the medical plan and the vision plan separately.
Other coverageCoordination of benefits (COB) set up in the right order so the correct plan is billed first.

What Do We Do in Crystal PM Each Day?

We follow each visit from charge to cash in the queues you assign.

  • Charge review: codes, modifiers and diagnoses against the exam note, with medical coding questions sent back to the provider.
  • Claim edits: automated eligibility checks and multiple claim scrubbing rules, then claims sent within 24 hours of charge entry.
  • Remittances: ERA 835 posting, with payment variances checked.
  • Denials: worked within 48 hours of receipt, by CARC and RARC, through denial management.
  • A/R follow-up and patient statements for balances after insurance, through accounts receivable work.

Which Crystal PM Reports Matter for Denial and A/R Tracking?

We describe reports by purpose. The vendor lists daily transaction summaries with payment type and provider filters, exportable financial data and bulk payment links sent from A/R reports.

PurposeWhat we look for
Charge lagDays from exam to charge entry, by provider and location
Claim statusSent, rejected or unpaid claims, by payer and age
A/R aging by payerBalances in 0 to 30, 31 to 60, 61 to 90 and over 90 days
Denial trendsTop reasons by CARC and RARC, by payer and provider
Payment variancePaid amounts against the contract, and unmatched or short remittances

These feed our revenue cycle management service.

FAQ

Common Questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside Crystal PM?

Yes, if your practice gives it access. Your administrator adds a named user for each person after a BAA is signed, with role-based access and two-step login. We never use shared logins. Scope and reporting are agreed at setup, and the vendor's own billers get access the same way.

02What access does a billing partner need in Crystal PM?

Named accounts with role-based access to charges, claims, payments and reports, plus the payer portals you want us to use. Every login is named, uses two-step verification and is never shared, and a BAA is signed first. Scope and reporting are agreed at setup.

03Which Crystal PM reports matter for denial and A/R tracking?

The ones that show charge lag, claim status, A/R aging by payer, denial trends and payment variance. The vendor lists daily transaction summaries, payment filters and A/R reports with bulk payment links. We name reports only as your account shows them, then describe them by purpose.

04How do claims move from Crystal PM to the clearinghouse and payer?

Charges built from the routing slip become electronic claims through the integrated claims processing the vendor lists in its plans. Its partner page names several insurance services companies. It does not say which one your practice uses, so we confirm it at setup.

05How does Crystal PM post ERA and 835 remittances?

The vendor lists automated importing of EOBs from its Plus plan up, and payments post to the patient ledger. We post against the payer's ERA 835, check short pays and adjustments against the contract, and send denials to the work queue.

06What billing mistakes are common in Crystal PM (charge capture, modifiers, demographics)?

These are checks, not vendor defects. We look for exam and optical charges mixed together, the wrong visit type for a medical or routine claim, missing modifiers, and member IDs that do not match the card. We also check that the correct plan is billed first.

07Does Medicare cover a routine eye exam billed from Crystal PM?

No. The Medicare Benefit Policy Manual, chapter 16, section 90, excludes eye exams done to prescribe, fit or change glasses, and excludes all refractions. Visits tied to an eye disease such as glaucoma or cataracts are not caught by that exclusion.

08Do eye visit codes get edited during a surgical global period?

Yes. Medicare Claims Processing Manual, chapter 12, section 40.3, puts 92012 and 92014 in the visit-code edit list for global surgery. It excludes 92002 and 92004 from those edits. Visits billed without modifier 24, 25, 57, 78 or 79 in the global period are the ones flagged.

09Is there a billing company that works in Crystal PM for optometry?

Yes. Nucleus Align works inside Crystal PM on accounts you create, so you keep your system. We work the eye care queues in your Crystal PM account. For each visit we decide whether the claim goes to the medical plan or the vision plan, check eye visit codes against surgical global periods, post remits and follow up denials. Claims go out within 24 hours of charge entry and denials are worked within 48 hours.

10Can an outsourced billing team use our Crystal PM logins?

No. Nucleus Align uses its own named Crystal PM accounts, which you create with role-based access, behind two-step login and a signed BAA. No one shares a password. You set each user's role. Your own staff keep their logins. You can remove access at any time.
