ModMed Medical Billing Support for Specialty Practices

Yes, an outside team can bill inside ModMed. After a signed Business Associate Agreement, the practice administrator adds each of our people as a named user, and the work stays in your own Practice Management.

In short: we bill in your ModMed under named logins with two-step login. Claims go out within 24 hours of charge entry and denials are worked within 48 hours.

Which ModMed products and specialties does this cover?

ModMed is the trade name of Modernizing Medicine, Inc., a vendor of software for specialty practices. The product we meet most is EMA, the vendor's specialty EHR, which its site describes as iPad-based and web-based, with coding suggestions drawn from clinical documentation.

Around EMA sit ModMed Practice Management, ModMed RCM Services, ModMed ASC for surgery centers and ModMed Scribe for documentation. The vendor's gastroenterology line is called gGastro. Billing screens live in Practice Management.

The vendor's home page lists allergy, dermatology, gastroenterology, OBGYN, ophthalmology, orthopedics, otolaryngology, pain management, plastic surgery, podiatry and urology. It is built for specialty groups, not hospitals or emergency departments.

ModMed sells its own billing service. Can an outside team still work in the system?

Yes. A practice that owns a ModMed license can choose who works its billing screens, and the practice administrator decides who gets an account. The vendor offers ModMed RCM Services too, and we have no tie to it.

The vendor says that service covers dermatology, OBGYN, ophthalmology, orthopedics, otolaryngology, pain management, plastic surgery and podiatry, and that gastroenterology practices and surgery centers have a separate offer.

What does access setup look like?

Your administrator holds the keys at every step.

1
BAA first
We sign a Business Associate Agreement with your practice before anyone opens a chart. Our staff complete HIPAA training and sign NDAs.
2
One user per person
One named user per team member, never a shared login, with a role that reaches billing screens and reports only.
3
Two-step login
The vendor's certification page lists a multi-factor authentication use case for EMA, and we ask for it on our accounts.
4
Agree scope and reporting
Worklists, reports and reporting are agreed at setup.
5
Use your existing connections
We submit through the clearinghouse and payer portals you already hold.

How does a claim leave ModMed and reach the payer?

A visit note in EMA hands its billing data to Practice Management, and Practice Management carries the claim to the payer. The vendor lists a clearinghouse interface inside Practice Management, customizable claim scrubbing before submission, automated claim file submission and timely filing reminders.

A professional claim travels as an 837P file. 45 CFR 162.1102 names the X12 837 as the HIPAA standard for health care claims, and the paper equivalent is the CMS-1500.

A claim scrubber runs edits before the payer sees the claim, and the vendor says you can customize its rules. We read charges ourselves as well: rendering provider, place of service, diagnosis links and modifiers.

For Medicare, 42 CFR 424.44 requires a claim to be filed no later than one calendar year after the date of service, with exceptions.

Does ModMed produce UB-04 institutional claims or only professional claims?

ModMed's pages describe professional billing and do not mention the UB-04, so we confirm at setup what your build produces. A hospital or facility claim on the UB-04 needs bill type, revenue codes and patient status, so ask your vendor contact before you assume it is supported.

Surgery centers are the practical question. The vendor sells ModMed ASC for charting, scheduling and quality reporting, but its page does not describe the claim, so we confirm it at setup. The CMS Claims Processing Manual, chapter 14, section 50, says an ASC bills its Medicare contractor on the X12 837 professional format or, in rare cases, on Form CMS-1500.

Our ambulatory surgery center billing page covers ASC rules.

How are ERA and 835 remittances posted?

An ERA is the electronic X12 835 file a payer sends to explain a payment, and 45 CFR 162.1602 adopts it as the HIPAA standard for remittance advice. The vendor lists posting of remittance advice among its Practice Management billing features.

ModMed's pages document no posting rules, so we confirm at setup how your account posts. Each batch we post is matched to the bank deposit, and every CARC and RARC on a denied line goes to a work queue.

Is ModMed certified health IT?

The vendor publishes separate ONC certification pages for EMA, gGastro and ModMed ASC. Each says the module was certified by an ONC-ACB and that certification is not an HHS endorsement.

The EMA page links an EHI export data dictionary, real world testing plans and results, and documentation for a certified FHIR API. We give no certification numbers here and take them from your certified product listing at setup.

We hold no certification, partnership or reseller link with the vendor.

What do we do inside a ModMed account?

  • Check charges for rendering provider, place of service, diagnosis links and modifiers before a claim is built
  • Clear scrubber hits and rejections and resubmit them
  • Send claims within 24 hours of charge entry
  • Post ERA 835 files and tie each batch to the deposit
  • Work denials within 48 hours from the remittance codes
  • Follow unpaid claims by payer and age, file appeals, send patient statements

Scope is on our medical billing and revenue cycle management pages.

Which ModMed reports matter for denial and A/R tracking?

We pick reports by the question they answer. The vendor says Practice Management includes a billing workflow with financial dashboards, tasks and reports, plus reporting for rejections and denials. Its RCM page names days in A/R and net collection rate as indicators you can watch on drill-down dashboards.

What we track and the vendor feature it maps to
PurposeWhat we look atWhere it lives, if enabled
Charge lagDays from visit to charge entry, by providerBilling tasks and reports

We confirm with your administrator which reports your edition includes.

What billing checks do we run in ModMed?

We assign no defects to the vendor. Each check ties to something its pages describe.

Checks we run and why
CheckReason
Scrubber rulesThe vendor says scrubbing is customizable, so we confirm who changed the rules and when
Suggested codingEMA suggests codes from documentation, so a coder reads the suggestion before the claim goes
EligibilityThe vendor runs automated eligibility searches before appointments, so we look at stale or missing results
Timely filingReminders exist, but each payer sets its own limit, so we track it per payer
Patient quotesThe vendor has a Quoting Tool and Text-to-Pay, so we match patient payments to the right account
Surgery center claimsASC claims follow the professional format per CMS, so we verify the claim form before submission

For modifier rules behind scrubber edits, see our NCCI edits guide.

FAQ

Common questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside ModMed?

Yes. Your administrator creates named accounts for our team after we sign a Business Associate Agreement. We use two-step login, never share logins, and the practice can remove any account. Work stays in your own Practice Management, under your clearinghouse and payer accounts.

02What access does a billing partner need in ModMed?

A named user per person, with a role limited to charge review, claims, remittance posting, worklists and reports. Your administrator sets the role. We ask for no more than the billing screens, and the BAA is signed before any chart is opened.

03Which ModMed reports matter for denial and A/R tracking?

We want charge lag, claim status, A/R aging by payer, denial trends and payment variance. The vendor lists reporting on rejections and denials, and days in A/R and net collection rate on its dashboards. Which reports you have depends on your setup, so we confirm it first.

04How do claims move from ModMed to the clearinghouse and payer?

The vendor lists a clearinghouse interface in Practice Management, customizable scrubbing and automated claim file submission. A professional claim goes as an 837P to the payer. We submit through the clearinghouse your practice already uses, and claims go out within 24 hours of charge entry.

05How does ModMed post ERA and 835 remittances?

The vendor lists posting of remittance advice among its billing features. We match each 835 batch to the deposit and send denial codes to a work queue, worked within 48 hours.

06What billing mistakes are common in ModMed (charge capture, modifiers, demographics)?

We name no vendor defects. We check the usual ones: missing modifiers, wrong diagnosis links, stale insurance details, unreviewed code suggestions and unworked denials. The vendor says scrubbing is customizable, so we also confirm who set the rules and when.

07Is ModMed data safe with an offsite billing team under a BAA?

We sign a Business Associate Agreement with every client. Staff complete HIPAA training and sign NDAs, every system uses two-step login, backups are encrypted and automatic, and a compliance officer oversees it. Each person has a named account that you can audit and remove.

08Does ModMed handle institutional UB-04 claims or only professional claims?

ModMed's pages describe professional billing and do not mention the UB-04, so we confirm at setup what your build produces. For surgery centers, CMS chapter 14 says Medicare ASC claims use the 837 professional format or, rarely, the CMS-1500. Ask your vendor contact before assuming facility claim support.

09How do we track out-of-network claims and appeals in ModMed?

ModMed's pages do not describe a specific out-of-network feature, so we track them by payer in the A/R and denial work queues. For federal dispute steps, see our out-of-network billing and IDR service. Scope is agreed at setup.

10What does outsourcing billing cost for a ModMed practice?

Our fees are on the pricing page and do not depend on your EHR. The vendor also sells its own RCM Services on terms it sets, and we hold no tie to it. Compare quotes on scope, reporting and who owns each worklist.

11Is there a medical billing service for ModMed practices?

Yes. Nucleus Align provides billing for specialty groups that run ModMed. Working in your ModMed with access you grant, we handle charge review, claim submission within 24 hours of charge entry, remittance posting and denials within 48 hours. Scope and reporting are agreed at setup.

12Can you bill from ModMed without us changing systems?

Yes. Nucleus Align works inside the ModMed you already use. Your documentation, scheduling and charge entry stay in place while we take on billing tasks you choose to hand over, through named logins your administrator creates for our team. You can book a call on our contact page to walk through your setup.
