NextGen Healthcare billing services for ambulatory practices and groups
We work inside your NextGen system through named, two-step accounts that you control, after a signed BAA. We review charges, send claims through your clearinghouse, post remittances and work denials and A/R in the queues you assign.
In short: we work in your NextGen Enterprise or NextGen Office system with our own named accounts, report from your data and claim no vendor partnership. Form and transaction rules cited here come from CMS and federal regulation.
What is NextGen Healthcare and who uses it?
NextGen Healthcare is a vendor of EHR and practice management software for ambulatory care. Its about page states the mission as "To simplify the business of ambulatory care".
NextGen sells two EHR products by practice size. NextGen Enterprise is listed for practices with 10 or more providers, and NextGen Office for practices with fewer than 10 providers.
Its specialty menu names behavioral health, cardiology, FQHC, ophthalmology, orthopedics and primary care among others, and says the vendor serves more than 40 specialty markets. NextGen's pages describe ambulatory settings.
| Item | What NextGen's site says |
|---|---|
| Vendor | NextGen Healthcare |
| EHR products | NextGen Enterprise (10+ providers) and NextGen Office (under 10 providers) |
| Practice management | NextGen Enterprise PM, described as integrating scheduling, eligibility checks, billing, claims processing, reporting and analytics |
| Charge review | NextGen Charge Review Rules Engine, placed between the EHR and the PM system |
| Clearinghouse and EDI | NextGen EDI and Clearinghouse, with Waystar named as the revenue cycle partner |
Is NextGen certified under the ONC program?
NextGen says so on its own pages. Its Enterprise EHR page states that NextGen Enterprise EHR version 8 has achieved certification under the ONC Health IT Certification Program for HTI-1 Phase 2, and its NextGen Office page calls that product ONC-Certified.
Certification details for each product edition are published in the federal ONC Certified Health IT Product List (CHPL).
Does NextGen produce CMS-1500 and UB-04 claims?
NextGen's public pages do not publish a claim-form specification, so what your system sends depends on your build, and we check a sample claim at setup. The form rules come from federal standards and apply in any system.
Under 45 CFR 162.1102, the standard for a professional claim is the X12 837P, ASC X12N/005010X222, and the standard for an institutional claim is the X12 837I, ASC X12N/005010X223. Professional services go on the CMS-1500 or the 837P. Facility services are billed on the UB-04, which CMS Claims Processing Manual Chapter 25 calls a uniform institutional provider bill.
Because NextGen's pages target ambulatory practices, most NextGen claims we expect to see are professional. If you bill facility fees from a NextGen system, we check that the institutional claim carries the bill type and revenue codes the payer expects. See CMS-1500 versus UB-04.
What access does a billing partner need in NextGen?
A billing partner needs named user accounts that you create, limited to billing roles, and a signed BAA before anyone sees patient data. We do not use shared logins.
Our security page covers HIPAA training, NDAs and encrypted backups.
What do we do inside a NextGen system?
We run the billing work you assign, in the queues you give us. We claim no partnership with NextGen Healthcare.
- Review charges for missing or mismatched codes, modifiers and diagnoses before they are released to billing
- Work claim edits and rejections so claims are sent within 24 hours of charge entry
- Submit claims through the clearinghouse your account uses
- Post payments from the ERA, the X12 835 electronic remittance advice, and review variances against the contract
- Work denial queues, with denials worked within 48 hours
- Follow up on aging A/R by payer and send patient statements
For charge review, NextGen describes its Charge Review Rules Engine as sitting between the EHR and the PM system. Charges that pass go on to the PM system, and an error is corrected automatically or sent to billing staff. We work the second group.
How do claims move from NextGen to the clearinghouse and payer, and how are ERAs posted?
NextGen's EDI page names Waystar as its revenue cycle partner and describes the clearinghouse as cloud-based and compatible with NextGen systems. Whether your claims use that route or a clearinghouse you contracted separately is set in your own configuration, and we confirm it before we start.
The same page lists claim attachments, claim monitoring and coverage detection. For payments, it describes remit and deposit management that reconciles payments with ERAs and produces download files for auto-posting, with alerts for missing items.
The 835 itself is a federal standard. Under 45 CFR 162.1602, the health care claim payment and advice transaction is ASC X12N/005010X221. We read each 835 at the line level for CARC codes and compare payment to the contract, not only the total.
Which NextGen reports matter for denial and A/R tracking?
We pull reports by purpose, whatever your system calls them. NextGen's pages mention financial and operational analytics and a flexible reporting tool, but publish no report titles we can quote.
| Purpose | What we look for |
|---|---|
| Charge lag | Days from visit to charge entry, by provider and location |
| Claim status | Claims sent, rejected, pending and paid, by payer |
| A/R aging by payer | Balances by age bucket, so older balances are worked first |
| Denial trends | Top denial reasons and payers, to find a cause upstream |
| Payment variance | Paid amount against the contracted rate, line by line |
If a report disagrees with the bank deposit, we reconcile the difference first.
What billing mistakes are common in NextGen?
We attribute no defects to NextGen. When something posts or sends wrongly, we trace it to the setting in your build or the payer rule behind it. These are checks we run in any ambulatory system.
| Check | Why it matters |
|---|---|
| Modifiers on the charge | The CMS-1500 can carry up to four modifiers per line in item 24D, according to the CMS Claims Processing Manual, Chapter 26. We check that the right ones survive the move from the encounter to the claim. |
| Rules engine exceptions | Charges the rules engine cannot fix go to staff. An unworked exception queue is a charge that never reaches the claim. |
| Demographics and eligibility | NextGen lists real-time eligibility checks. We compare registration data against the eligibility response. |
| Clearinghouse rejections | A rejection is not a denial: the payer has not accepted the claim yet, so we work rejections first. |
| Remittance posting | We compare the posted adjustment to the CARC and group code on the 835, so a contractual write-off is not applied to a denial. |
For denial work in detail, see our denial management page.
How we handle billing in NextGen
We start with a signed BAA and named, two-step accounts, then map your build: where charges enter, which clearinghouse sends claims and where ERAs land. Claims go out within 24 hours of charge entry, and denials are worked within 48 hours. For the wider picture, read our revenue cycle management and medical billing pages, or compare how we work in athenahealth, eClinicalWorks and Greenway Health. When you are ready, book a call.

Common questions
Straight answers from the team that does the work.
Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.
01Can an outside billing company work inside NextGen Healthcare?
Yes, if you give it access. Your administrator creates a named account for each person, limited to billing roles, after a BAA is signed. We use those accounts and never a shared login. You keep control and can disable any account whenever you choose.
02What access does a billing partner need in NextGen Healthcare?
Named user accounts with billing roles, access to the reports you choose, and portal access to your clearinghouse if you use one. Each account uses two-step login, and we do not share credentials between people or use a generic login.
03Which NextGen Healthcare reports matter for denial and A/R tracking?
The ones that show charge lag, claim status, A/R aging by payer, denial trends and payment variance. NextGen describes financial and operational analytics and a flexible reporting tool, but publishes no report titles, so we map each purpose to whatever your build offers.
04How do claims move from NextGen Healthcare to the clearinghouse and payer?
NextGen's EDI page names Waystar as its revenue cycle partner, and many practices send through it, but your configuration decides. Charges are reviewed, claims are created, edited and sent as 837 transactions, and the clearinghouse returns acknowledgments. We confirm your route before we start.
05How does NextGen Healthcare post ERA and 835 remittances?
NextGen describes remit and deposit management that reconciles payments with ERAs and creates download files for auto-posting, with alerts for missing items. We review each 835 at the line level, checking CARC and RARC codes and contract variance, so a denial is never posted as an ordinary adjustment.
06What billing mistakes are common in NextGen Healthcare (charge capture, modifiers, demographics)?
We attribute no defects to NextGen; we trace a problem to the setting or payer rule behind it. Our checks are not system-specific: modifiers lost between encounter and claim, rules engine exceptions left unworked, registration data that does not match eligibility, and adjustments posted against the wrong code.
07Is NextGen Healthcare data safe with an offsite billing team under a BAA?
A signed BAA covers it contractually, and we add controls: named accounts with role-based access, two-step login, HIPAA training and NDAs for all staff, encrypted automatic backups and a dedicated compliance officer. Our team is offshore. See our security page.
08Does NextGen Healthcare handle institutional UB-04 claims or only professional claims?
NextGen's public pages describe ambulatory practices and do not publish a claim-form specification, so we check a sample claim at setup. Under 45 CFR 162.1102 institutional claims use the 837I and professional claims the 837P. If you bill facility fees from NextGen, we confirm in your build that it produces the institutional claim.
09How do we track out-of-network claims and appeals in NextGen Healthcare?
We tag out-of-network claims by payer and plan, then track each through its status, appeal deadline and payment against the expected rate. NextGen's billing-company page describes tools to track appeals and resubmissions. Disputes under the No Surprises Act follow our out-of-network and IDR process.
10How long does it take to move billing to a new vendor while staying on NextGen Healthcare?
It depends on your volume, open A/R and payer enrollments, so we give no fixed number. You keep NextGen. The steps are the BAA, account creation, a build review, a clearinghouse check and a handover of open claims and denials.
11What does outsourcing billing cost for a NextGen Healthcare practice or facility?
Our billing fee is a share of collections, and the range and what it covers are on our pricing page. We do not charge for your NextGen software, which stays a cost between you and the vendor. Volume and payer mix affect the percentage.
12Is there a billing company that works in NextGen Enterprise or NextGen Office?
Yes. Nucleus Align works inside both NextGen Enterprise and NextGen Office using named, role-limited accounts your administrator creates. We handle claim review, submission, remittance posting and denial work, with a BAA signed first. Scope is agreed at setup. Scope and reporting are agreed with you at setup.
