eClinicalWorks Medical Billing and RCM Support

Yes, an outside billing team can work inside eClinicalWorks. Your practice creates named, role-based user accounts for each of our team members, we sign a Business Associate Agreement first, and we work your charges, claims, remittances and denials in your own system.

In short: we work in your eClinicalWorks environment under our own named accounts, in the queues and reports you approve. Claims go out within 24 hours of charge entry and denials are worked within 48 hours.

What is eClinicalWorks and who uses it?

eClinicalWorks is a cloud-based EHR and practice management system from eClinicalWorks, a privately held vendor. Its own site says it serves independent ambulatory practices and more than 850 health centers.

The vendor's products and services page lists the EHR, an Inpatient EHR, a Pharmacy Management System, AI Workbench, Value-Based Care, Patient Engagement, Revenue Cycle Management, Interoperability and Health Centers.

For billing, the vendor describes four options: RCM technology that your own staff run, Central Billing Office (CBO) technology for groups that bill for several practices, an RCM service where eClinicalWorks does the billing, and RCM consulting.

Is eClinicalWorks certified health IT?

eClinicalWorks says its EHR software is certified under the ONC Health IT Certification Program. Its Certified EHR Technology page also says it is certified to the electronic health information export criterion at 45 CFR 170.315(b)(10) and offers certified FHIR APIs.

We claim no certification, partnership or reseller link with the vendor ourselves.

How do we get access to your eClinicalWorks system?

Access starts with your administrator, not with us. The steps below keep every action in your audit trail under a named person.

1
Sign the BAA
We sign a Business Associate Agreement with your practice before anyone sees a patient record. Our staff are covered by HIPAA training and NDAs.
2
Create named accounts
Your administrator creates one user account for each of our team members, with a role that limits access to billing screens, work queues and reports. No shared logins.
3
Turn on two-step login
Every account uses two-step login. We also use it on every system we touch ourselves.
4
Agree the queues and reports
We agree which charge, claim edit, denial and follow-up work lists we own, and which reports we run on what schedule.
5
Connect to your clearinghouse and portals
We work through the clearinghouse and payer portals you already use, under accounts you control.

What do we do inside eClinicalWorks?

We run the billing steps between charge entry and cash, using the tools already in your system. We do not ask you to change software.

  • Review charges for missing provider, place of service, diagnosis and modifier detail before claims leave the system
  • Review claim edits and rejections, correct them and resubmit through your clearinghouse
  • Send claims within 24 hours of charge entry
  • Post ERA 835 remittances and reconcile them against deposits
  • Work denials within 48 hours, using the CARC and RARC codes on the remittance
  • Follow up on unpaid claims by payer and age, and file appeals
  • Prepare patient statements

Our medical billing and revenue cycle management services describe the full scope.

How do claims move from eClinicalWorks to the payer?

Claims leave the system as electronic transactions in the X12 format. Under 45 CFR 162.1102, HIPAA adopts the X12 837 professional and 837 institutional transactions as the standards for health care claims. A practice claim goes out as an 837P and a facility claim as an 837I.

The vendor's RCM page says its technology includes bots that scrub and submit claims, eligibility and deductible information, and denial and appeal management. A claim scrubber is a set of edits that catches errors before the payer does.

How does eClinicalWorks post ERA and 835 remittances?

An ERA is the electronic 835 file a payer sends to explain a payment. The vendor says its RCM technology receives and posts electronic remittances, and that an EOB to ERA agent creates an ERA from paper when the payer offers none, reducing manual EOB posting.

We compare each posted payment to the deposit and route any claim with a denial code to a work queue.

Which eClinicalWorks reports matter for denial and A/R tracking?

We pull reports by purpose and use the vendor's own tools where your setup includes them. The vendor names an Alerts Dashboard, Work Queue Dashboards, the eCW RCM Dashboard, Provider Analytics and practice KPIs.

What billing mistakes are common in eClinicalWorks?

We cannot source a defect list for eClinicalWorks, and we do not attribute one to the vendor. These are the checks we run on any system, tied to the payer rules and vendor documents.

CheckWhy it mattersBasis
Visit type matches the provider typeFor an FQHC, 42 CFR 405.2463 defines a visit as a face-to-face encounter with listed provider types, plus medical nutrition therapy and diabetes self-management training. A mismatched visit can be denied.eCFR 42 CFR 405.2463
Professional claim or institutional claimHealth centers with dental, pharmacy or emergency modules can bill on both formats. The vendor says its tools cover split claims and UB04 billing.eClinicalWorks health center page; 45 CFR 162.1102
Demographics and eligibility before the visitThe vendor's RCM tools include patient eligibility and deductible information. We confirm coverage is checked before the claim is built.eClinicalWorks RCM page

Do you work with FQHCs and health centers on eClinicalWorks?

We work in health center setups when the client has built them. The vendor's health center page names UDS reporting, Title X family planning reporting, Ryan White reporting and sliding fee support.

Billing in these centers differs from a private practice. Under 42 CFR 405.2463, an FQHC visit is defined by the encounter and the provider type, so those two details on the claim matter. See our behavioral health billing and family medicine billing pages for the specialty detail.

Groups that bill for several sites may use the vendor's OpenConnect central billing setup, which the vendor says gives back-office staff single sign-on. See billing for large groups and health systems.

Does eClinicalWorks handle UB-04 claims or only professional claims?

The vendor says it does both. Its Inpatient EHR page lists institutional billing, charge capture, claims processing and EDI among its revenue cycle capabilities, and its health center page lists UB04 billing and split claims.

For facility claims we check bill type, revenue codes and patient status before submission, as set out in hospital and facility billing.

FAQ

Common questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside eClinicalWorks?

Yes. Your administrator creates named user accounts for our team with role-based access, after we sign a Business Associate Agreement. We use two-step login and never share logins. Every action stays in your audit trail, and you can remove an account whenever you choose.

02What access does a billing partner need in eClinicalWorks?

Only what billing needs: charge and claim screens, work queues, payment posting and reports. Each person gets a named account and a role your administrator controls, and you can remove access at any time. Read access to visit notes is added only where coding review needs it.

03Which eClinicalWorks reports matter for denial and A/R tracking?

We track charge lag, claim status, A/R aging by payer, denial trends and payment variance. The vendor names an Alerts Dashboard, work queue dashboards, an RCM Dashboard and Provider Analytics, which we use where your setup includes them, so each report answers one question.

04How do claims move from eClinicalWorks to the clearinghouse and payer?

Claims leave as X12 837 transactions, 837P for professional and 837I for institutional. The vendor says its RCM tools scrub and submit claims, and payers return rejections and 835 remittances. We work through your clearinghouse, so rejections and remittances come back to you in the same place.

05How does eClinicalWorks post ERA and 835 remittances?

The vendor says its RCM technology receives and posts electronic remittances, and an EOB to ERA agent creates an ERA from paper when a payer sends none. We still check each posting against the deposit and route denial codes to a work queue instead of leaving them posted at zero.

06What billing mistakes are common in eClinicalWorks?

We do not attribute defects to the vendor. The mistakes we check for on any system are wrong visit or provider type, missing modifiers, stale eligibility, claims sent on the wrong form, and denials left unworked after auto-posting. Our checks sit in the billing checks table above.

07Is eClinicalWorks data safe with an offsite billing team under a BAA?

Data protection comes from the BAA and our controls: HIPAA training and NDAs for all staff, two-step login on every system, named accounts, encrypted automatic backups and a dedicated compliance officer. Your administrator controls the accounts and can remove access immediately.

08Does eClinicalWorks handle institutional UB-04 claims or only professional claims?

The vendor says it handles both. Its Inpatient EHR page lists institutional billing and EDI, and its health center page lists UB04 billing and split claims. We check bill type, revenue codes and patient status on every facility claim before it goes out.

09How do we track out-of-network claims and appeals in eClinicalWorks?

We tag out-of-network claims in your work queues, record the denial or underpayment, and keep the dates for appeals and negotiation in one tracker. Where a claim qualifies for federal dispute resolution we move it to our out-of-network and IDR service.

10What does outsourcing billing cost for an eClinicalWorks practice?

Our fee does not depend on which EHR you use. It is a percentage of collections, and the current figures are on our pricing page. Compare it with the full cost of your current in-house billing team, including salaries, software and turnover, before you decide.

11Can you bill from eClinicalWorks?

Yes. Nucleus Align works inside your eClinicalWorks environment under our own named accounts, in the queues and reports you approve, covering claims, posting, denials and A/R. Claims go out within 24 hours of charge entry and denials are worked within 48 hours.

12Can an outsourced billing team use our eClinicalWorks logins?

No. Nucleus Align uses its own named eClinicalWorks accounts, which you create with role-based access, behind two-step login and a signed BAA. No one shares a password. Your eClinicalWorks administrator manages access. Your own staff keep their logins. You can remove access at any time.
