athenahealth medical billing and RCM support for ambulatory groups

We work inside your athenahealth account through named, two-step accounts you control, after a signed BAA. We review charges, send claims, post remittances and work denials and A/R.

In short: we work in your athenahealth account with our own named user accounts and report from your data. We claim no partnership with athenahealth.

What is athenahealth and who uses it?

athenahealth is a healthcare technology vendor whose own site describes its focus as ambulatory care. Its main product, athenaOne, is described as a fully integrated electronic health record (EHR), medical billing and practice management, and patient engagement solution.

For billing, the chart, charge, claim and payment can sit in one system. Our work is the revenue side: charge review, coding checks, claim edits, remittance posting, denial work and A/R follow-up.

The vendor also lists specialty workflows, including behavioral health and psychiatry. Its athenaIDX line is described as revenue cycle management for hospital and ambulatory care, with emergency medicine, anesthesia, radiology and pathology named.

athenahealth's pages say athenaEDI integrates with athenaIDX, athenaIDX for Group Management and athenaPractice. We confirm on day one which of these you run.

How do claims move from athenahealth to the clearinghouse and payer?

athenahealth's athenaOne pages say its rules engine and billing experts identify and resolve claim errors before submission, and that automated scrubbing and claim alarms help prioritize follow-up.

After that, the claim goes out as an electronic file. athenahealth operates athenaEDI, which its site calls an all-payer clearinghouse with workflows for claims, remits, attachments, and auto-reconciling and posting. Some organizations use it. Others send through their own clearinghouse. We use the route you already have.

According to CMS, the CMS-1500 is the standard form for non-institutional providers and suppliers, and the National Uniform Claim Committee (NUCC) designs and maintains it. The CMS-1450, also known as the UB-04, is the paper form institutional providers use.

athenahealth's public pages do not say how its athenaOne product treats institutional claims, so we confirm at setup what your build produces. If you bill facility charges, we confirm in your build which claim type is produced before we touch a claim.

What access does a billing partner need in athenahealth?

We need named user accounts that you create and control, limited to billing access. No shared logins, and nothing starts before the Business Associate Agreement (BAA) is signed.

1
Sign the BAA
The Business Associate Agreement is signed before anyone on our team sees patient data.
2
You create named accounts
Your administrator creates one user account per team member, each with role-based access limited to billing work.
3
Two-step login
Every account uses two-step login, and no account is shared.
4
Agree the work lists
We agree in writing which claim edit lists, denial work and follow-up queues we own, and which stay with your staff.
5
Start small
We begin with a limited batch of claims, then widen the scope.

All staff have HIPAA training and sign NDAs, and backups are encrypted and automatic. See our security page.

What do we do inside athenahealth?

We work the revenue side of your athenahealth account. You set the scope.

  • Review charges for missing or mismatched codes, modifiers and diagnoses before release
  • Work claim edits and rejections so claims are sent within 24 hours of charge entry
  • Submit claims through the clearinghouse you already use, athenaEDI or your own
  • Post payments from the ERA (electronic remittance advice, the X12 835 file) and match them to expected amounts
  • Work denials within 48 hours, including appeals
  • Follow up on unpaid claims by payer and age
  • Flag front-end problems, such as eligibility and demographics, back to your staff
Front-end checks matter here
athenahealth's practice management page describes tools that read insurance cards and verify coverage. We work with whatever is turned on in your account and flag claims whose data does not match the payer's record.

Which athenahealth reports matter for denial and A/R tracking?

We describe reports by purpose, because names differ by product and configuration. athenahealth's practice management page mentions executive summary dashboards and data reporting, and its athenaIDX page mentions dashboards that surface denial trends. We describe reports by purpose and map each to the name in your account.

PurposeWhat it tells youSuggested review cadence
Charge lagDays from visit to charge entryWeekly
Claim statusWhich claims are unbilled, sent, accepted, rejected or paidDaily
A/R aging by payerOpen balances by payer in age bandsWeekly
Denial trendsDenials by payer, reason code and providerWeekly and monthly
Payment variancePaid amount against expected amount by payerMonthly

If a report does not give us a view, we build it from an export of your data, source agreed first.

What billing mistakes are common in athenahealth?

We do not attribute defects to athenahealth. These are general checks we run on any ambulatory account.

CheckWhy we run it
Charge on the right visit, date and rendering providerA wrong provider or date gives a claim that does not match the payer's records
Modifiers present on the line, not only in the noteOn the CMS-1500 the procedure code and modifiers travel on each service line, so a missing modifier is a claim defect, not a chart defect
Demographics and insurance match the cardA changed policy, subscriber ID or birth date causes front-end rejections
Right claim type for the settingCMS describes the CMS-1500 for professional claims and the UB-04 for institutional claims
ERA matched to the claim before the balance movesAdjustment and denial codes need to land in the right bucket

Where a check shows a pattern, we report it so the fix is made at the source.

Which practices and facilities does this suit?

athenahealth's site lists specialty practices, enterprises and health systems. For a multi-site group, see our page for large groups and health systems. For urgent care, see urgent care networks and urgent care billing. Other ambulatory systems we cover include eClinicalWorks and NextGen. Hospital-scale systems are covered on the Epic page.

How we handle billing in athenahealth

We work inside your account, in your queues, under accounts you control. Claims are sent within 24 hours of charge entry, and denials are worked within 48 hours.

We are an outside billing company with no partnership with athenahealth. We follow your build, your clearinghouse and your payer rules. Our medical billing and revenue cycle management services cover the full cycle, and denial management takes over once a claim is denied.

FAQ

Common questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside athenahealth?

Yes, if your organization creates accounts for them. Your administrator sets up named, role-based accounts for the outside team after a Business Associate Agreement is signed. We never use a shared login, and your staff can see what our accounts do.

02What access does a billing partner need in athenahealth?

Only what the billing role requires: named accounts, two-step login, and access to the claim edit lists, denial work, payment posting and reports we agreed to own. Your administrator sets the permissions and can change or remove them at any time.

03Which athenahealth reports matter for denial and A/R tracking?

The useful views are claim status, A/R aging by payer, denial trends by reason code, and payment variance. athenahealth's pages mention dashboards and data reporting but publish no report titles, so we agree the exact sources in your account on setup.

04How do claims move from athenahealth to the clearinghouse and payer?

The claim is checked, then sent as an electronic file through a clearinghouse to the payer. athenahealth offers athenaEDI, an all-payer clearinghouse, and some organizations use their own. The payer then returns an ERA 835 remittance once it pays or denies.

05How does athenahealth post ERA and 835 remittances?

athenahealth's site says athenaEDI supports auto-reconciling and posting of remits. How much is turned on depends on your account. We check that each ERA matches its claim, review the adjustment and denial codes, and work every unposted or mismatched payment.

06What billing mistakes are common in athenahealth, such as charge capture, modifiers and demographics?

We do not name vendor defects. The common problems in any ambulatory system are charges on the wrong date or provider, missing modifiers, diagnoses that do not support the service, and insurance data that does not match the card. We check each before release.

07Is athenahealth data safe with an offsite billing team under a BAA?

A Business Associate Agreement is the legal base for sharing patient data with us. Staff have HIPAA training and sign NDAs, every system uses two-step login, backups are encrypted, and a dedicated compliance officer oversees it. You also control who has access.

08Does athenahealth handle institutional UB-04 claims or only professional claims?

athenahealth's athenaIDX page says it supports hospital and ambulatory care, but its public pages do not describe the claim type athenaOne produces, so we confirm it at setup. CMS says professional claims go on the CMS-1500 and institutional claims on the UB-04. We confirm which your build produces.

09How do we track out-of-network claims and appeals in athenahealth?

We agree a tracking method on setup, using your work lists and notes, or a shared log if your build lacks a field. Each out-of-network claim records its payer, dates, appeal stage and next action. See our out-of-network billing and IDR service.

10How long does it take to move billing to a new vendor while staying on athenahealth?

The system stays the same, so the move is about access, work lists and handover of open claims. The pace depends on how many accounts your administrator must create and how clean the open A/R is. We start with a limited batch of claims and widen it.

11Can a billing vendor connect athenahealth to our existing clearinghouse and payer portals?

We use the clearinghouse and payer portals you already have, with named logins. Any change to the claim route, such as a new clearinghouse connection, is made by your team or athenahealth, and we test it before we rely on it.

12What does outsourcing billing cost for an athenahealth practice or facility?

Cost depends on volume, specialty and scope, and our pricing is on the pricing page. athenahealth's site says athenaOne is priced as a percentage of collections, so check what your current plan already covers, such as billing services, before you compare any outside quote.
