Billing inside Tebra

Tebra medical billing for independent practices

Yes, an outside team can bill from within your Tebra account. You issue each of us a named login, and nothing begins until a BAA is signed.

In short: you keep Tebra and we bill from inside it on logins you issue. Nucleus Align has no partnership with Tebra.

What is Tebra and who uses it?

Tebra is a cloud-based electronic health record (EHR) and practice platform from Tebra Technologies, Inc. Its own site says the EHR is built for independent practices, with charting, scheduling, billing, patient payments and telehealth in one system.

Tebra describes its EHR as built for practices with 1 to 10 providers, including primary care, behavioral health, psychiatry, therapy, pediatrics and nurse practitioner-led clinics. The same page contrasts it with hospital-grade systems.

We have not checked the federal certified product list, so we state no certification detail.

How do claims move from Tebra to the clearinghouse and payer?

Tebra's billing page says its rules engine auto-scrubs claims, applies payer-specific rules and sends batches by electronic data interchange (EDI).

Tebra's pages do not name the clearinghouse, so we confirm which one your account uses. We confirm which one your account uses before we touch a claim.

For eligibility, Tebra says it sends an ANSI X12 270 inquiry to more than 2,700 payers and receives a 271 response, covering active coverage, copay, deductible and coinsurance.

CMS names the CMS-1500 as the form for non-institutional billers, with design and upkeep handled by the NUCC. Hospitals and other facilities use the UB-04 (CMS-1450) instead. Tebra's pages do not mention institutional claims, so a facility would need to check first what its build can produce.

What access does a billing partner need in Tebra?

Our team works from personal logins that your administrator issues and can switch off. The Business Associate Agreement (BAA) comes first, and no login is ever shared.

1
BAA first
Both sides sign before our staff can open a single chart or claim.
2
One user per person
Your administrator adds each of our staff as a separate Tebra user, with a role that reaches billing screens only.
3
Second factor on every login
Each user signs in with two-step verification.
4
Divide the queues
You and we decide who owns the claim edits, the denial queue and the follow-up lists.
5
Set scope and reporting
Both are settled with you at setup.

Staff complete HIPAA training and sign NDAs, and backups are encrypted and run automatically. Our side of data protection is set out on the security page.

What do we do inside Tebra?

Our job is the money side of the account. The scope is yours to define.

  • Check each charge for the right codes, modifiers, diagnoses and rendering provider, then release it
  • Clear rejections and edit holds, aiming for submission inside 24 hours of the charge being entered
  • Send through whichever clearinghouse your account already uses
  • Match each ERA (electronic remittance advice, the X12 835 file) to what the payer was expected to pay, and post it
  • Take denials into a work queue within 48 hours and file appeals where they are warranted
  • Chase claims that are still unpaid, oldest and highest value first
  • Issue patient statements, and send eligibility or demographic errors back to your front desk
Automation is a setting, not a given
Tebra says its automation can auto-post ERAs by business rules. We review what is switched on in your account and check auto-posted payments against the remittance.

Which Tebra reports matter for denial and A/R tracking?

Report titles differ by plan and setup, so here is the purpose of each pull. Tebra's billing page mentions dashboards for denial patterns and A/R trends, and one view for tracking accounts receivable.

  • Charge lag: how many days pass between the visit and the charge being keyed
  • Claim status: what is unbilled, in transit, rejected, or paid
  • A/R aging by payer: where open money sits and how old it is
  • Denial trends: which payers, reason codes and providers repeat
  • Payment variance: paid amount set against the expected amount

When no standard report gives the view, we build it from a data export, and agree the source with you first.

What billing mistakes are common in Tebra?

No defect is being blamed on Tebra here. This is the checklist we run on any ambulatory account.

  • Provider and date: a charge tied to the wrong rendering provider or service date will not line up with the payer's file
  • Modifiers: professional claims that need a modifier and lack one are denied often
  • Identity and coverage: demographics and member ID must match the card and the 271 coverage response, or the claim is rejected
  • Auto-posting: Tebra says posting follows business rules, so one bad rule can misstate many payments until someone notices
  • Denial routing: Tebra says denials can be queued for appeal or automatic resubmission, so we confirm which rule fires
  • Unapplied cash: money not matched to a claim keeps A/R looking higher than it is

A problem that keeps repeating goes back to your staff for a fix at the source.

Which practices does this suit?

Tebra describes its users as independent practices. Our pages for behavioral health, mental health, pediatrics and family medicine cover the specialties Tebra names. Other ambulatory systems we cover include AdvancedMD, DrChrono and athenahealth. The full list is on the EHR systems page.

How we handle billing in Tebra

You keep Tebra. We take the billing queues you hand over and work them from our own named users. Claims go out within 24 hours of charge entry, and denials are worked within 48 hours.

Nucleus Align is an independent billing company and has no partnership with Tebra. Your configuration, clearinghouse and payer rules stay as they are. Pick medical billing for claims alone, revenue cycle management for the whole cycle, or add denial management for appeals.

FAQ

Common questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside Tebra?

Yes, once your practice issues them logins. Tebra's billing page itself describes a setup where the practice stays in its EHR while a partner handles claims. Your administrator adds each of us as a named user with a limited role after the BAA is signed, and nothing is shared.

02What access does a billing partner need in Tebra?

One personal user per team member, added by your administrator with a role that reaches billing functions only. Each signs in with two-step verification, and the BAA is signed beforehand. Which queues we own and which stay with your staff is settled before work begins.

03Which Tebra reports matter for denial and A/R tracking?

Five views do most of the work: claim status, A/R aging by payer, denial trends by reason code, charge lag and payment variance. Tebra's billing page mentions denial and A/R dashboards. We use whatever your plan includes and fill gaps from a data export.

04How do claims move from Tebra to the clearinghouse and payer?

Per Tebra, claims are auto-scrubbed, run against payer-specific rules and sent in batches by EDI. Tebra's pages do not name the clearinghouse, so we find out which one your account uses and then follow each claim to acceptance, rejection or payment.

05How does Tebra post ERA and 835 remittances?

Tebra says its automation posts ERAs and unapplied payments by business rules. We read the rules set in your account, then compare what was posted against the 835 file, so a faulty adjustment or patient-responsibility split cannot repeat across many payments.

06What billing mistakes are common in Tebra (charge capture, modifiers, demographics)?

The software is rarely the cause. Typical culprits in any system are a wrong rendering provider or service date, absent modifiers, and demographics that disagree with the payer's record. We screen for each before release and group rejections by reason to fix the source.

07Is Tebra data safe with an offsite billing team under a BAA?

The BAA comes before any access to patient data. Our staff hold HIPAA training and NDAs, sign in with two-step verification on personal users, and backups are encrypted and automatic. Tebra's security page separately describes its own business associate agreement covering protected health information on its platform.

08Does Tebra handle institutional UB-04 claims or only professional claims?

Tebra's pages describe billing for independent practices and never mention institutional claims. The CMS-1500 is the professional form and the UB-04 the institutional one. A facility billing under its own name should confirm which claim type its Tebra build can produce before we start.

09How do we track out-of-network claims and appeals in Tebra?

We separate claims by payer and network status, then follow them by age and reason code. A denied claim enters a work queue and its appeal is tracked to a decision. Out-of-network claims that were underpaid also get a separate dispute track where federal rules apply.

10How long does it take to move billing to a new vendor while staying on Tebra?

Tebra's billing page says a practice can keep its EHR while billing changes hands. The pace depends on open claims, the logins needed and the scope. Scope and reporting are agreed with you at setup, and we do not quote a standard timeline.

11Can a billing vendor connect Tebra to our existing clearinghouse and payer portals?

That depends on your account setup and contracts. We use the clearinghouse and payer portals you already have, and check how your Tebra account routes claims to them. We do not swap your clearinghouse unless you ask, and any change is agreed with you in writing first.

12What does outsourcing billing cost for a Tebra practice or facility?

Our fees are on the pricing page, which is the only place we state them. The scope you choose, such as claims only or the full cycle with denials and A/R, is agreed with you at setup, and the EHR itself is not what sets the price.
