Osmind billing services for psychiatry practices
We work inside your Osmind account through named, two-step logins that you control, after a signed BAA. We check the charge against the note, send claims through the route your plan uses, post remittances and work denials and A/R.
In short: we work in your Osmind account with our own named logins, on the billing tasks you assign, and report from your own data. Osmind also sells billing services, so we confirm who owns each task first. We claim no partnership with the vendor.
What is Osmind and who uses it?
Osmind is a psychiatry EHR and practice services company. Its home page describes AI-powered EHR, billing, prior authorization and patient engagement tools built for Spravato, TMS, ketamine and other treatments, and cites more than 1,000 practices.
The vendor's focus is interventional psychiatry, which its own page defines as psychiatry where the clinician's judgment is joined with direct delivery or oversight of a procedure. TMS means transcranial magnetic stimulation, a device-based treatment for depression. Spravato is the brand name of intranasal esketamine, which the vendor lists with a REMS (an FDA drug safety program).
That makes Osmind different from the general therapy tools on the EHR hub. For therapist-led practices, see SimplePractice. For community programs with several funders, see Netsmart.
Which Osmind plans decide who does the billing?
Osmind sells three plans, and the plan decides who submits claims. Descriptions below are from the vendor's pricing page.
| Plan | What the vendor says | What it means for billing |
|---|---|---|
| Osmind One | Software only, for practices billing cash or insurance on their own | Your team owns claims and prior authorizations. We can work as that team. |
| Osmind Care Network | Software, services and Osmind's payer contracts, for practices launching insurance, Spravato, TMS or VNS | Osmind handles billing and prior authorizations under its contracts. We confirm what is left to do. |
| Osmind 360 | Software and services with your own payer contracts, for established practices with 4+ providers | Osmind offers full revenue cycle work. We agree which queues stay with us or with you. |
If Osmind staff already work your denials or A/R, two teams in one account duplicate work. We settle task ownership in writing first.
What access does a billing partner need in Osmind?
We need a named, role-based account for each person on our team, created by your practice. Nothing is shared and nothing is opened before the paperwork is signed.
Our security page covers data protection.
How do claims move from Osmind to the clearinghouse and payer?
In Osmind a visit becomes a pre-populated claim, a clearinghouse checks it, and the payer receives it. A clearinghouse is the intermediary that edits and routes electronic claims. The vendor's pricing page lists ClaimMD and Office Ally integrations for claim submission, and its billing page cites a connection to more than 38,000 payers.
The vendor says Osmind autofills most claim fields, lets patients upload their own insurance, and builds claims from fee schedule templates. It describes real-time error checking, a claim scrubber, and one-click resend.
Professional psychiatry services go out on the CMS-1500 as an electronic 837P. The vendor's pricing page says Medicare and Medicaid are outside Care Network's contracted payers, but practices can bill them through the platform with the Office Ally or ClaimMD integration. We confirm per payer which route applies.
We send claims within 24 hours of charge entry.
What billing mistakes are common in Osmind?
We attribute no defects to Osmind. These checks come from what the vendor says its tools do and from the CMS claim form instructions.
| Check | Why it matters | Source |
|---|---|---|
| Spravato visit complete before the claim | The vendor says charting the encounter populates and submits the REMS monitoring form. An unfinished chart can leave the form and the claim out of step. | Osmind EHR page |
| TMS session notes match the authorization | The vendor says its TMS templates capture the session details payers require for authorization and reimbursement. We compare session count and dates on the claim to the approval. | Osmind EHR page |
| Modifiers fit the CMS-1500 line | CMS says item 24D holds the HCPCS code and up to four modifiers. We check the modifier order and payer edits for each line. | CMS Claims Processing Manual, chapter 26, item 24D |
| One diagnosis pointer per line | Item 24E takes one reference letter or number per line, tying the service to a diagnosis in item 21. Wrong pointers invite medical necessity denials. | CMS Claims Processing Manual, chapter 26, item 24E |
| Unlisted codes carry a narrative | When a payer wants a newer treatment on an unlisted or not otherwise classified code, CMS says the narrative goes in item 19 or an attachment. | CMS Claims Processing Manual, chapter 26, item 24D |
Drug and administration codes for newer treatments change by payer and by year, so we read the payer's current policy for the date of service. The CMS rules above apply to Medicare; other payers set their own.
How do prior authorizations work for TMS and esketamine?
That depends on the plan. The vendor's pricing page says Osmind One users handle prior auths themselves, Care Network includes support, and Osmind 360 offers it as an add-on covering Spravato and TMS.
A prior authorization is the payer's approval before treatment. Where we hold that task, we log approval numbers, date ranges and visit counts and check them against the schedule. Where Osmind holds it, we check approvals against the claim. See our prior authorization service.
How does Osmind post ERA and 835 remittances?
An ERA is the electronic X12 835 file a payer sends to explain a payment. The vendor's billing page says payer payment postings and patient payments are auto-reconciled on the claim itself, and that rejection reasons are shown.
Auto-reconciliation does not review the payment. We check the CARC and RARC codes on every adjustment, flag short pays against your fee schedule, and check patient balances so a card payment is not applied twice. How much posting is automatic in your build is agreed at setup.
What do we do in Osmind each day?
We follow each visit from charge to cash.
- Charge review: service code, modifiers, units, dates and diagnosis against the visit note.
- Claim edits: automated eligibility checks and multiple claim scrubbing rules, on top of the vendor's own error checks.
- Submission: claims go out through your clearinghouse route within 24 hours of charge entry.
- Posting: payments and adjustments from the 835, with short pays flagged.
- Denial work: denials are worked within 48 hours. See denial management.
- A/R follow-up, superbills and patient statements, payer balances first.
Which Osmind reports matter for denial and A/R tracking?
The vendor's billing page promises claims tracking, rejection reasons, reporting, and a per-patient invoice view. It does not publish report names, so we describe each by purpose. Scope and reporting are agreed at setup.
| Purpose | What we look for |
|---|---|
| Charge lag | Days from visit to charge entry, by clinician and treatment |
| Claim status | Sent, rejected or unpaid claims, by payer and age |
| A/R aging by payer | Balances by age bucket, split by payer and patient |
| Denial trends | Top denial reasons by CARC and RARC, by payer and treatment |
| Payment variance | Paid amounts against contract rates or the fee schedule |

Common questions
Straight answers from the team that does the work.
Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.
01Can an outside billing company work inside Osmind?
Yes, if your practice gives it access. Osmind also sells billing services, so we first agree which tasks stay with Osmind and which we own. Your administrator creates named users for us after a BAA is signed. We never use shared logins.
02What access does a billing partner need in Osmind?
A named, role-based account for each team member, two-step login and a signed BAA. Your administrator limits each role to billing, claims and payment screens. We also need the claim route you use, such as the ClaimMD or Office Ally integration, and any payer portals.
03Which Osmind reports matter for denial and A/R tracking?
Look for claim status, A/R aging by payer, denial reasons by CARC and RARC, and payments against expected rates. The vendor describes claims tracking and reporting but does not publish report names, so we map each report to its purpose at setup.
04How do claims move from Osmind to the clearinghouse and payer?
A visit becomes a pre-populated claim, a clearinghouse checks it and the payer receives it. Osmind's pricing page lists ClaimMD and Office Ally integrations for submission. We send claims within 24 hours of charge entry and clear rejections before they age.
05How does Osmind post ERA and 835 remittances?
The vendor says payer postings and patient payments auto-reconcile on the claim. We still check every CARC and RARC, flag short pays against your fee schedule and review patient balances. How much posting is automatic in your build is settled at setup.
06What billing mistakes are common in Osmind (charge capture, modifiers, demographics)?
We attribute no defects to the vendor. Our checks follow the CMS-1500: up to four modifiers in item 24D, one diagnosis pointer per line in item 24E, and actual units in item 24G. We also match TMS and Spravato visits to the authorization.
07Who handles prior authorization for TMS and Spravato in Osmind?
It depends on your plan. The vendor says Osmind One users handle their own, Care Network includes it, and Osmind 360 offers it as an add-on for Spravato and TMS. Where it stays with you, we can log approvals and check visit counts.
08Do you work with Osmind as a partner?
No. Nucleus Align has no partnership, certification or reseller status with Osmind. We work inside client-owned accounts under your security rules. Product names are the vendor's trademarks, used only to describe systems our clients run. Scope is agreed with each client.
09Is there a billing company for psychiatry practices on Osmind?
Yes. Nucleus Align supports psychiatry and interventional psychiatry practices that run Osmind. We work in your system with access you grant, covering plan and claim route checks, TMS and Spravato claims, remittance posting and denial work. Scope and reporting are agreed with you at setup.
