Netsmart billing services for community behavioral health

We work inside your Netsmart account through named, two-step accounts you control, after a signed BAA. We check programs, service lines and payer order, send claims through your clearinghouse, post remittances and work denials and A/R.

In short: we work in your Netsmart account with our own named logins, in the queues you assign, and report from your own data. We claim no partnership with the vendor, and the claim rules here come from Netsmart's own pages and CMS manuals.

What is Netsmart and who uses it?

Netsmart is a health IT company whose CareRecord EHR line serves human services and post-acute care, with behavioral health as its largest focus on the vendor's site. Its own pages describe organisations that run many programs at once: community mental health, addiction treatment, autism and ABA services, child and family services, and intellectual and developmental disabilities (IDD).

The vendor also lists CCBHC clinics (Certified Community Behavioral Health Clinics), county agencies and post-acute settings such as home health and hospice. Its About page cites more than 50 years in the field.

That makes it different from the small-practice tools on the EHR hub. A billing lead here is usually reconciling several funding streams, not one commercial payer mix. For solo therapy practices, see SimplePractice and TherapyNotes.

Which Netsmart products touch billing?

Netsmart sells several EHRs and a separate revenue cycle product line. Names and descriptions below are the vendor's own.

ProductWhat the vendor says
myAvatarBehavioral health EHR for community-based, residential and inpatient care, with billing and claims management
myEvolvWeb-based EHR for behavioral health and human services, including addiction treatment, autism, CCBHC and IDD
RevConnectClearinghouse, batch eligibility, coverage discovery, patient liability statements and a payment portal
Netsmart RCM servicesEnrollment, eligibility, coding, billing, follow-up, cash posting and denial management

The vendor sells outsourced RCM too, so we confirm who owns each queue. Which product you run decides the screens we use, so we confirm product and version at setup.

What access does a billing partner need in Netsmart?

We need named, role-based accounts that your administrator creates. Nothing is shared and nothing is opened before the paperwork is signed.

1
Sign the BAA first
Nucleus Align signs a Business Associate Agreement (BAA) before any record is opened. Our staff have HIPAA training and NDAs.
2
Your administrator adds named users
Each person on our team gets their own login with role-based access, limited to billing and the programs in scope.
3
Turn on two-step login
Every account we use is protected with two-step login, including the clearinghouse and payer portals we reach for you.
4
Confirm product, programs and routing
We list your Netsmart product, which programs bill insurance, which are grant funded, and which clearinghouse sends claims.
5
Agree scope and reporting
Which work queues we own and which stay with your team are written down at setup, along with how we report.

Our security page covers data protection.

How do claims move from Netsmart to the clearinghouse and payer?

A charge becomes a claim in the EHR, then a clearinghouse checks it and routes it to the payer. The vendor's RevConnect is a clearinghouse, but your organisation may use another.

Professional services go on the CMS-1500 as an electronic 837P. Programs billed as institutional claims, such as community mental health center services under Medicare, use the UB-04 as an 837I. We confirm per payer and program which one is expected.

We send claims within 24 hours of charge entry and fix rejections before they age.

What billing mistakes are common in Netsmart?

We attribute no defects to Netsmart. These checks come from what the vendor says its billing does and from CMS manuals.

CheckWhy it mattersSource
Every repeated service gets its own dated lineCMS says a community mental health center or hospital must report a line item date of service for every iteration of every revenue code on outpatient bills. Five services means five dated lines.CMS Claims Processing Manual, chapter 25, form locator 45
Diagnosis pointer on each professional lineOn the CMS-1500, item 24E takes the reference number or letter that ties each service to a diagnosis from item 21, one reference per line.CMS Claims Processing Manual, chapter 26, item 24E
Claims built from notes and placement dataThe vendor says myEvolv generates claims from clinical notes and placement data, so note errors become claim errors. We trace them to the source.Netsmart myEvolv page
Rates set by coordination of benefitsThe vendor says myEvolv determines payer rates based on coordination of benefits workflows. A wrong payer order changes what each payer is billed.Netsmart myEvolv page
Grant and contract programsThe vendor says myAvatar handles grant funding and wrap-around service reimbursement. We confirm per program whether a service bills a payer or a funder.Netsmart myAvatar page

The CMS rules above apply to Medicare. Medicaid agencies and other payers set their own rules, and we read each payer's manual first.

How do CCBHC, Medicaid and grant funding change the billing?

They change which payer or funder owns each service, which is the first thing we map. The vendor says myAvatar supports value-based reimbursement, grant funding and CCBHC requirements, and myEvolv supports Medicare, Medicaid and complex payer billing.

Medicaid rules vary by state, so the claim rules for your state agency or managed care plan come from that source, not from the EHR. We state no CCBHC payment method, because it depends on your state.

In any funding mix we check the right payer on the right service, authorization on file where the plan needs one, and eligibility before the visit. Our mental health billing and behavioral health billing pages cover the clinical side.

How does Netsmart post ERA and 835 remittances?

An ERA is the electronic X12 835 file a payer sends to explain a payment. The vendor says myAvatar offers integrated remittance and lists cash posting as a service, but publishes no posting steps.

We post payments and adjustments from the 835 to the right claim and line, check the CARC and RARC codes on every adjustment, and flag short pays against the contract or fee schedule. Whether the posting is automatic or manual in your build is agreed at setup.

What do we do in Netsmart each day?

We follow each service from charge to cash in the queues you assign.

  • Charge review: program, service code, units, dates and diagnosis against the note.
  • Claim edits: automated eligibility checks and multiple claim scrubbing rules.
  • Submission: claims go out through your clearinghouse within 24 hours of charge entry.
  • Posting: payments and adjustments from the 835, with short pays flagged.
  • Denial work: denials are worked within 48 hours. See denial management.
  • A/R follow-up and patient statements, payer balances first.

Which Netsmart reports matter for denial and A/R tracking?

The vendor says myAvatar has dashboards for billing status, accounts receivable and payer performance. Names differ by build, so we describe reports by purpose, and scope and reporting are agreed at setup.

PurposeWhat we look for
Charge lagDays from service to charge entry, by program and clinician
Claim statusSent, rejected or unpaid claims, by payer and age
A/R aging by payerBalances by age bucket, split by payer and funder
Denial trendsTop denial reasons by CARC and RARC, by payer and program
Payment variancePaid amounts against contract rates or the fee schedule

These measures feed our revenue cycle management service.

FAQ

Common questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside Netsmart?

Yes, if your organisation gives it access. Netsmart sells its own RCM services, but your administrator can create named users for an outside team after a BAA is signed. We never use shared logins, and you decide which queues we work.

02What access does a billing partner need in Netsmart?

A named, role-based account for each team member, two-step login and a signed BAA. Your administrator limits each role to the billing screens and programs in scope. We also need access to your clearinghouse and the payer portals you use.

03Which Netsmart reports matter for denial and A/R tracking?

Look for claim status, A/R aging by payer, denial reasons by CARC and RARC, and payment against expected rates. The vendor says myAvatar has billing, A/R and payer performance dashboards. Names differ by product and build, so we map each report to its purpose at setup.

04How do claims move from Netsmart to the clearinghouse and payer?

A charge becomes a claim in the EHR, then goes to a clearinghouse that checks it and sends it to the payer. Netsmart's RevConnect is a clearinghouse, but your organisation may use another. We send claims within 24 hours of charge entry and clear rejections before they age.

05How does Netsmart post ERA and 835 remittances?

The vendor says myAvatar has integrated remittance and its RCM page lists cash posting, but it does not publish posting steps, so we confirm at setup how your account posts. We post from the 835 to the right claim and line, check every CARC and RARC, and flag short pays. Automatic or manual posting is settled at setup.

06What billing mistakes are common in Netsmart (charge capture, modifiers, demographics)?

We attribute no defects to the vendor. Medicare requires each repeated service on an outpatient institutional bill to carry its own dated line, per CMS chapter 25. Because myEvolv builds claims from notes and placement data, we check codes, units, dates and payer order at the source.

07Does Netsmart have its own billing and clearinghouse tools?

Yes. The vendor lists RevConnect, covering a clearinghouse, batch eligibility, coverage discovery, patient liability statements and a payment portal. It also sells RCM services. We work in whichever setup you already run, including other clearinghouses and billing tools your team prefers.

08How does billing work for a CCBHC on Netsmart?

The vendor says myAvatar and myEvolv support CCBHC reporting and value-based care. The payment method depends on your state program, so we confirm it from your state Medicaid agency or plan, not from the EHR. We map each service to its payer or funder first.

09Do you work with Netsmart as a partner?

No. Nucleus Align has no partnership, certification or reseller status with Netsmart. We work inside client-owned accounts under your security rules. Product names are the vendor's trademarks, used only to describe systems our clients run. Scope is agreed with each client.

10Is there a billing company that works in Netsmart?

Yes. Nucleus Align supports behavioral health and human services organizations that run Netsmart. We work inside your Netsmart with named, role-limited accounts you grant, covering claims, remittance posting and denial work across your programs and funders. A BAA is signed first. Scope and reporting are agreed with you at setup.

11Medical billing service for community behavioral health agencies on Netsmart

Nucleus Align handles billing for behavioral health agencies in Netsmart, including claims by program and payer, denials worked within 48 hours, and claims sent within 24 hours of charge entry. Which programs and funders we cover is agreed with you at setup.

12Can an outsourced billing team use our Netsmart logins?

No shared logins. Your Netsmart administrator creates a named user for each Nucleus Align team member, limited to the billing functions you choose. Our staff sign in with two-step login and work under HIPAA training and NDAs, and you can remove access at any time.
