TherapyNotes Billing Support for Behavioral Health Practices

Yes, an outside team can bill inside TherapyNotes. After a signed Business Associate Agreement, a Practice Administrator adds each of our people as a named user, and the work stays in your own account.

In short: we bill in your TherapyNotes under named logins with two-step login. Claims go out within 24 hours of charge entry and denials are worked within 48 hours.

What is TherapyNotes and who uses it?

TherapyNotes is a behavioral health EHR. The page title on the vendor's own site reads "Behavioral Health EHR/EMR", and its help center covers scheduling, notes, billing, insurance claims, ERA, ePrescribe, telehealth and a client portal called TherapyPortal.

The help center names psychologists, therapists, case managers, nurse practitioners and psychiatrists as clinicians. A signed note is what makes a claim ready to send. We have no tie to the vendor.

Can an outside billing company work inside TherapyNotes?

Yes, if the practice that owns the account adds us as users. The vendor's user guide says every person who needs access should have a unique user name and password, and that sharing credentials breaks its Terms of Service, so we work under named logins only. The work stays in your own accounts.

What access does a billing partner need in TherapyNotes?

A named user with a billing role.

1
Sign the BAA
A Business Associate Agreement is signed first. Our staff have HIPAA training and NDAs.
2
Create named users
A Practice Administrator opens Staff, then New User, and enters a user name, name and email address for each person. The user replaces the temporary password at first login.
3
Pick the role
The vendor says a Practice Biller has full billing access to all clients without access to clinical notes. Biller for Assigned Patients Only cannot submit claims, so it is too narrow.
4
Turn on two-step login
We use two-step login on every system, and no login is shared between people.
5
Agree scope and reporting
Scope and reporting are agreed at setup. The administrator can deactivate any of our users later.

How do claims move from TherapyNotes to the clearinghouse and payer?

TherapyNotes sends electronic claims through its own clearinghouse. The vendor's help center names that clearinghouse as CLAIM.MD and says a claim is ready to send once a clinician completes and signs a billable note for a client set up to bill insurance.

Billers work from a Submit Electronic Claims page. It shows pending claims for the last 90 days by default, with filters for payer, clinician and patient. Each box on the page is one claim, because dates of service are batched by patient.

Red warnings block submission and yellow ones do not, so we read the yellow ones every time.

For individual practitioners the HIPAA claim standard is the X12 837 professional transaction, a file called the 837P. The vendor also generates a paper CMS-1500 for payers that need one.

What billing quirks should you know in TherapyNotes?

These come from the vendor's help center. Help articles change as the product changes, so we confirm each one in your live account.

Documented behaviors that affect claims and payments
AreaWhat the vendor documentsWhat we check
BatchingDates of service are batched on one claim per patient, and if any date in the batch is rejected, the entire claim is rejected.Split a batch around a bad date.
DiagnosesAn electronic claim can carry up to four diagnosis codes or only the primary. The CMS-1500 carries only the primary. Different diagnosis sets split services into separate claims.Match the setting to the payer, and check paper claims apart.
ModifiersA claim can include up to four modifiers. Defaults can be set in a clinician billing override, a service code, or multiple practice locations settings, and they fill in automatically.Trace where a modifier came from.
Billing addressA PO box as the billing provider address is rejected. Payments can go to a separate alternate billing address.Street address in settings, PO box in the alternate field.
Unsupported payersA payer with no Payer ID in the clearinghouse list cannot receive claims electronically through TherapyNotes.Route them outside the system and log them.
Secondary claimsA secondary claim goes out after the primary ERA is posted. A rejection can say the clearinghouse does not support the secondary policy, and the fix is to remove it.Post primary first, confirm secondary enrollment.
Corrected claimsResubmission types are New Claim, Amended Claim and Void Claim. Amended puts the number 7 on the claim, and the payer claim control number is required for Amended and Void.Get the control number from the payer first.

How do we read a claim rejection in TherapyNotes?

The vendor explains that a claim can be rejected at the clearinghouse, in the payer's EDI intake, or during adjudication, and that where it happened hints at the cause. A clearinghouse rejection means the scrubbing step failed on codes, taxonomy, enrollments or demographics.

Rejection messages carry a payer ID, status, code and message, and the message is the useful part. The help center lists common ones with fixes, such as an incomplete subscriber address or a member ID equal to the group number.

A rejection never reached the payer's adjudication, so we fix and resend it. A denial comes back on the remittance and goes to a separate queue. A rejected claim shows the status Pending Resubmit.

How does TherapyNotes post ERA and 835 remittances?

An ERA is the electronic form of an explanation of benefits, and TherapyNotes receives it from its clearinghouse into the billing account. Enrollment for ERA is a separate step from claim enrollment, and the payer page shows a green check, a yellow caution sign or a grey ban icon for each.

A biller can also upload an ERA file received outside the system. Assisted posting reads the ERA and fills in payment, copay, coinsurance and deductible for review.

Three behaviors matter. A red No Match Found link appears when the payer's client name or service code differs from the chart, for example a middle initial. Only the last occurrence of a repeated service on an ERA is editable. And claim-level adjustments leave a gap between payment and Total Allocated that a biller must assign before posting.

We read CARC and RARC codes on the ERA before we accept any suggestion, because a suggestion that balances the line can still hide a denial.

What do we do inside a TherapyNotes account?

  • Review notes, service codes, modifiers and diagnoses before batching
  • Clear warnings and send claims within 24 hours of charge entry
  • Work the Electronic Claim History rejection queue and resubmit with the right type
  • Confirm payer enrollment for claims and ERA, and upload outside ERA files
  • Post ERA payments and work denials within 48 hours
  • Follow up unpaid claims by payer and age
  • Run eligibility verification and send patient statements

Which TherapyNotes reports matter for denial and A/R tracking?

We choose a report by the question it answers. The vendor's help center lists an Insurance Aging Report, a Revenue Report, an Insurance Adjustment Report, a Write-Off Report and a Prior Authorization Report.

What we track and why
PurposeWhat we look at
Charge lagSigned note to sent claim
Claim statusOpen rejections, silent claims
A/R aging by payerBalances by payer and age, from the aging report
Denial trendsTop denial reasons from remittance codes by payer
Payment variancePaid against contract rate, from adjustment and write-off reports

Billing rules for therapy and psychiatry are on mental health billing and behavioral health billing. Other behavioral health systems are on the SimplePractice and Netsmart pages.

FAQ

Common questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside TherapyNotes?

Yes. A Practice Administrator creates a named user for each of our people after we sign a Business Associate Agreement. We use two-step login, never share logins, and you can deactivate any account. The work stays in your own accounts.

02What access does a billing partner need in TherapyNotes?

A named user with the Practice Biller role. The vendor describes it as full billing access to all clients without access to clinical notes. A narrower role, Biller for Assigned Patients Only, cannot submit claims. The administrator can change or remove the role at any time.

03Which TherapyNotes reports matter for denial and A/R tracking?

The vendor's help center lists an Insurance Aging Report, Revenue Report, Insurance Adjustment Report and Write-Off Report. We pair them with remittance codes for denial trends by payer, and with charge-to-claim timing for lag. Scope and reporting are agreed at setup.

04How do claims move from TherapyNotes to the clearinghouse and payer?

A signed billable note makes a claim ready. The biller selects it on the Submit Electronic Claims page and sends it to the vendor's clearinghouse, which the help center names as CLAIM.MD. The clearinghouse scrubs it and forwards it to the payer, and status updates return to the claim history.

05How does TherapyNotes post ERA and 835 remittances?

Enrolled payers route ERA to the account through the vendor's clearinghouse, and a biller can also upload ERA files. Assisted posting fills in payment details for review. The biller matches any No Match Found lines, assigns claim-level adjustments and checks that Payment Amount equals Total Allocated before posting.

06What billing mistakes are common in TherapyNotes (charge capture, modifiers, demographics)?

The vendor's rejection list is mostly demographic: incomplete subscriber addresses, a member ID matching the group number, ZIP and state mismatches and PO box billing addresses. We also check modifiers, which can fill in from three default settings, and diagnosis settings before claims go out.

07Why was my TherapyNotes claim rejected and what do I do?

Open the Electronic Claim History and read the rejection message, which shows a payer ID, code and message. Where it happened tells you the cause: the clearinghouse checks codes and demographics, the payer's EDI checks identifiers. Fix the record, then resubmit as a new, amended or void claim.

08Do I have to enroll with each payer to get ERA in TherapyNotes?

Yes, for most payers. The vendor says ERA enrollment is separate from claim enrollment, and the payer page shows a green check, yellow caution sign or grey ban icon for status. A payer with no ERA support sends paper or file remittances that you upload or enter by hand.

09Is there a billing company for therapy practices on TherapyNotes?

Yes. Nucleus Align supports behavioral health practices that run TherapyNotes. We use named user accounts you create, submit claims through the CLAIM.MD path, post ERAs, fix rejections and work denials within 48 hours, under a signed BAA. Scope and reporting are agreed with you at setup.
