Office Practicum billing services for pediatric practices

Office Practicum (OP) is a pediatric EHR and practice management platform, and an outside billing team can work inside it when your administrator grants named, role-based access. We review charges, check vaccine and Medicaid details, send claims through your clearinghouse, post remittances and work denials.

In short: we use named, two-step accounts in Office Practicum under a signed BAA and report from your own data. We claim no partnership with the vendor. Vaccine program rules on this page come from the CDC Vaccines for Children Operations Guide and Medicaid rules in 42 CFR 441.

What is Office Practicum and who uses it?

Office Practicum, shortened to OP, describes itself as a pediatric EHR. Its home page says it is built only for pediatrics and trusted for more than 30 years.

The vendor groups the product into clinical care, practice operations, financial performance, and patient engagement and growth. The financial area covers billing and claims, revenue cycle management, patient payments, credentialing and enrollments, and analytics.

The vendor also sells its own pediatric revenue cycle service, which it says requires the OP platform. Practices that want an outside team instead can bring us in.

How does an outside billing team get access in Office Practicum?

1
Sign the BAA first
We sign a Business Associate Agreement (BAA) with you before any patient record is opened. Our staff have HIPAA training and NDAs.
2
Your administrator adds named users
Each person on our team gets their own OP login with role-based access to the billing screens and reports we need.
3
Turn on two-step login
Every account we use has two-step login, including the clearinghouse and payer portals we reach for you.
4
Confirm the clearinghouse and payer enrollments
We confirm which clearinghouse your OP account sends through, which payers need enrollment, and who holds those credentials.
5
Agree scope and reporting
Which queues we own, such as charges, rejections, denials and patient balances, and how we report are written down at setup.

No shared logins. See our security page.

How do claims move from Office Practicum to the payer?

The vendor describes one connected path. Eligibility is confirmed before the visit, superbill charges are generated as providers document care, a claim scrubber applies customizable rules, and claims go out through integrated clearinghouses.

The vendor says acknowledgments and front-end rejections often come back within hours. A front-end rejection stops a claim before the payer accepts it; a denial comes after. See rejection versus denial.

The vendor says ERAs map payments directly to claims. An ERA is the electronic remittance advice, the X12 835 file a payer sends with payment and adjustment detail.

OP's public pages do not name which clearinghouse a given practice uses or list payer-specific edits, so we confirm both with you at setup. At setup we confirm the clearinghouse in use and which payers return ERAs electronically.

What makes pediatric billing in Office Practicum different?

The vendor's billing FAQ names the pediatric details: a well visit and a sick visit on the same day, vaccines billed with administration codes, Medicaid rules that differ by state, and siblings seen on the same day.

On the vaccine side, the vendor's VacLogic+ module forecasts what is due from immunization history and AAP recommendations. It also separates private and Vaccines for Children inventory. It says administration codes are attached automatically based on payer guidelines.

We review those automatic results, because a rule that fits one payer can be wrong for a state Medicaid plan.

What are the Vaccines for Children and Medicaid billing rules?

The Vaccines for Children (VFC) program supplies publicly purchased vaccine at no charge to enrolled providers for eligible children. The CDC VFC Operations Guide says providers cannot charge an eligible child's parent a fee for the vaccine itself, but can charge a fee to administer each vaccine.

The administration fee is capped at a regional amount that varies by state, on a scale determined by CMS. For Medicaid-eligible children, the provider must accept the reimbursement set by the state Medicaid agency or its health plans.

Medicaid is usually the payer of last resort, but the CDC guide says not for the administration fee of Medicaid-eligible VFC children. Vaccination is part of the Medicaid EPSDT benefit, which under 42 CFR 441.50 covers beneficiaries under age 21, and the state may later seek reimbursement from the primary insurer.

If a primary insurer pays less than Medicaid would for administration, the provider can bill Medicaid for the balance, up to the Medicaid amount. A parent of a child with Medicaid as secondary coverage must never be billed for a vaccine or an administration fee.

For a VFC-eligible child without Medicaid, the CDC guide allows a single bill to the family within 90 days of administration. Unpaid administration fees may not go to collections.

Procedure codes and modifiers for administration are set in each state's own Medicaid provider manual, so we check the manual for each state you bill.

What billing mistakes are common in Office Practicum?

These checks come from the rules above or the vendor's own description. We attribute no defect to the vendor.

CheckWhy we run it
Same-day sick and well visitThe vendor says both may be billable when documentation and payer requirements support them, so we confirm the note supports each
VFC dose billed as administration onlyVFC providers bill an administration fee, not a vaccine fee (CDC guide)
Private versus VFC stockWe match the claim to the stock actually used
Administration fee and payer orderMedicaid pays the fee for Medicaid-eligible VFC children even when another insurer is primary (CDC guide). We check coordination of benefits
Family and guarantorEach sibling has a separate claim. We check each child's insurance and the guarantor, the person responsible for the bill
Patient balance on a VFC feeOne bill within 90 days for non-Medicaid VFC children, no collections (CDC guide). We stop statements that break this

What do we do inside Office Practicum?

  • Charge review: we compare visit, vaccines, in-house labs, screenings and procedures to the charges generated.
  • Coding: we check codes, modifiers and diagnoses against the note.
  • Claim edits: demographics, insurance order and guarantor are fixed before submission.
  • Submission: claims go out within 24 hours of charge entry, through your clearinghouse.
  • Remittances: we post the ERA (835) and match payments to charges.
  • Denial work queues: denials are worked within 48 hours.
  • A/R follow-up and family patient statements for what remains open.

Which Office Practicum reports matter for denial and A/R tracking?

The vendor documents Practice Analytics dashboards covering A/R days, payer mix and reimbursement metrics, and Pediatric Benchmarking. We describe reports by purpose and agree the set at setup.

Report purposeWhat it answers
Charge lagDays between the visit and charge entry
Claim statusWhich claims are rejected or waiting on the payer
A/R aging by payerWhich payers hold older balances
Denial trendsWhich denial reasons repeat by payer
Payment varianceWhere payments differ from the expected amount
Vaccine paid, billed, collectedWhether vaccine revenue matches what was given

Where the vendor's dashboard answers a question, we use it; otherwise we build the list from claim data.

FAQ

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside Office Practicum?

Yes, if your practice gives it access. Your administrator creates a named user for each person on our team after a BAA is signed, with role-based access and two-step login. We never use shared logins. Claims are built in your OP account, so we confirm at setup who owns which queue.

02What access does a billing partner need in Office Practicum?

Access to charges, claims, remittances, patient balances and billing reports, plus the clearinghouse and payer portals your claims use. It is role-based, tied to a named user and protected by two-step login. Scope and reporting are agreed at setup, and no logins are shared.

03Which Office Practicum reports matter for denial and A/R tracking?

The vendor describes Practice Analytics dashboards with A/R days and reimbursement metrics. We track charge lag, claim status, A/R aging by payer, denial trends and payment variance, using your own claim and remittance data. The exact set is agreed with you at setup.

04How do claims move from Office Practicum to the clearinghouse and payer?

The vendor says claims are scrubbed with customizable rules and sent through integrated clearinghouses. It says acknowledgments and front-end rejections often return within hours. We confirm the clearinghouse in use and who holds payer enrollments at setup, then write it down.

05How does Office Practicum post ERA and 835 remittances?

The vendor says ERAs map payments directly to claims and that denials are flagged for follow-up. An ERA is the X12 835 file a payer sends with payment and adjustments. We post it, match each payment to its charge and send denials to a work queue.

06What billing mistakes are common in Office Practicum (charge capture, modifiers, demographics)?

We check for missing charges on same-day sick and well visits, vaccine fees billed to VFC families, wrong stock type, insurance order for siblings and wrong guarantor details. These are checks on any pediatric claim, not defects in the software.

07Can a practice bill a family for a VFC vaccine?

No. The CDC VFC Operations Guide says providers cannot charge an eligible child's parent a fee for the vaccine itself, only a capped fee to administer it. If Medicaid is secondary, the guide says the parent must never be billed for either.

08Does Medicaid pay the vaccine administration fee if another insurer is primary?

For Medicaid-eligible VFC children, the CDC guide says Medicaid must pay the administration fee even though it is normally the payer of last resort, because vaccination is part of EPSDT. The state may later seek repayment from the primary insurer.

09What does EPSDT mean for pediatric billing?

EPSDT is the Medicaid benefit for early and periodic screening, diagnosis and treatment. Under 42 CFR 441.50 it applies to eligible Medicaid beneficiaries under age 21. Requirements differ by state, so we check each state's Medicaid manual before claims are sent.

10Does Nucleus Align have a partnership with Office Practicum?

No. We claim no partnership, reseller status or endorsement with the vendor. We work inside client systems under the access your practice grants. Vendor and product names appear as plain text names only, and scope is agreed at setup first.

We work in your system with named logins you grant, covering vaccine and VFC claim checks, Medicaid rules, family statements, remittance posting and denial work. Scope is agreed at setup.
