ChiroTouch Billing Services for Chiropractic Practices
We work inside your ChiroTouch Advanced account through named, two-step accounts you control, after a signed BAA. We check modifiers, diagnoses and note support, send claims through your clearinghouse, post remittances and work denials and A/R.
In short: we work in your ChiroTouch account with our own named logins, in the queues you assign, and report from your own data. We claim no partnership with the vendor, and the Medicare chiropractic rules here come from CMS manuals and an MLN article.
Already on this system? We work in it. No EHR, or tired of paying for one? Our billing clients get EHR or PM software free.
What Is ChiroTouch and Who Bills Out of It?
ChiroTouch is chiropractic EHR and practice management software with a cloud version. Its site cites 12,500+ practices and 36,000+ providers.
It comes in two editions. ChiroTouch Advanced is for insurance-billing practices. ChiroTouch Core is for cash-based practices that take card and recurring payments. Outside billing help applies to Advanced, where claims and the clearinghouse live.
The vendor says one system covers the EHR, billing, scheduling, intake and payments. Compliance Scan, the vendor says, reviews chart notes and billing codes and flags missing documentation.
The vendor says Advanced billing covers private insurance, Medicare, personal injury and workers' compensation. For the code and payer side, see our chiropractic billing page. See also EHR systems, WebPT and SimplePractice.
What Access Does a Billing Partner Need in ChiroTouch?
How Do Claims Move from ChiroTouch to the Clearinghouse and Payer?
Advanced practices send claims through a clearinghouse that the vendor bundles. ChiroTouch offers two, and each is available to Advanced customers only.
| Product | What the vendor says |
|---|---|
| CT ProClear | Integrated clearinghouse powered by TriZetto. The vendor's FAQ says it is required and covers electronic claims, eligibility and payment processing. |
| CT MaxClear | Claims management integrated with the Change Healthcare clearinghouse. |
| CT Verify | Real-time eligibility checks, with alerts when a patient is not covered for a scheduled service. |
Both clearinghouse pages say the codes in the provider's SOAP note (the visit note: subjective, objective, assessment, plan) link to the billing screen.
On the payment side, the vendor says ERA files download automatically, payments post to the patient ledger, and split payments between insurer and patient are tracked. It also says payments are matched to claims and discrepancies flagged.
Professional claims use the CMS-1500 data set, sent as the 837P. The vendor's Claims Worklist is the central claim status screen. We work it daily.
Which Medicare Rules Shape a Chiropractic Claim in ChiroTouch?
Medicare covers chiropractors only for manual manipulation of the spine to correct a subluxation. Medicare Benefit Policy Manual chapter 15, section 240, says no other chiropractor service is covered, and an x-ray is not required to show it.
| Item | Rule | Source |
|---|---|---|
| AT modifier | Report the AT modifier on 98940, 98941 and 98942 only for active or corrective care. MACs deny those codes without it, and it is never placed on maintenance care. | MLN SE1602; Benefit Policy Manual 240.1.3 |
| Diagnoses | Primary diagnosis is subluxation at a stated level, plus a second diagnosis for the condition. | MLN SE1602; Benefit Policy Manual 240.1.2 |
| CMS-1500 box 14 | Date of initial treatment or of the exacerbation. A missing date draws CARC 16 with RARC MA122. | Claims Processing Manual ch. 12, 220 |
| CMS-1500 box 19 | Date of the x-ray, only when an x-ray is used to show subluxation. | Claims Processing Manual ch. 12, 220 |
| GA and GZ | GA goes with a signed ABN, and GZ only in rare cases, when care is expected to be denied as maintenance. | Benefit Policy Manual 240.1.3 |
| Level of subluxation | The precise level, by exact bones or by area, must be on the claim. Treatment unrelated to the level cited is denied with CARC B22. | Benefit Policy Manual 240.1.4; ch. 12, 220 |
CMS Medicare Coverage Database article A58412, published by the MAC First Coast Service Options, adds that every treatment must be labelled acute, chronic or maintenance.
What Must the Note Show Before the Claim Goes Out?
The note must support the AT modifier. MLN SE1602 states that AT alone does not show a service was reasonable and necessary, and a MAC can deny after medical review.
- Initial visit (Benefit Policy Manual 240.1.2): history, present illness, an exam, a subluxation diagnosis with its level, a treatment plan with goals, and the date of first treatment.
- Subluxation by exam: two of four findings (pain or tenderness, asymmetry or misalignment, range of motion abnormality, tissue or tone changes), one being asymmetry or range of motion.
- Later visits: changes since last visit, an exam of the area treated, and that day's treatment.
CMS says a statement of "pain" is not enough. Compliance Scan is a first pass; we still read the note.
What Billing Mistakes Are Common in ChiroTouch?
These are the checks we run in any chiropractic account, not defects of the vendor.
| Area | What we check |
|---|---|
| Charge capture | Every visit has a charge, and the codes picked in the note match what the provider documented, since the note feeds the billing screen. |
| Modifiers | AT present on active care and absent on maintenance, and GA or GZ only where an ABN exists. |
| Diagnosis and level | Subluxation is primary, the level matches the note, and the second diagnosis supports the condition. |
| Demographics and eligibility | Name, birth date and member ID match the card and the eligibility response. |
| Other coverage | Personal injury and workers' compensation cases are set up before the first claim. See workers' comp and no-fault billing. |
What Do We Do in ChiroTouch Each Day?
We follow each visit from charge to cash in the queues you assign.
- Charge review: codes, modifiers and diagnoses against the note, with medical coding questions sent back to the provider.
- Claim edits: automated eligibility checks and multiple claim scrubbing rules, then claims sent within 24 hours of charge entry.
- Remittances: ERA 835 posting, with payment variances checked.
- Denials: worked within 48 hours of receipt, by CARC and RARC, through denial management.
- A/R follow-up and patient statements for balances after insurance.
Which ChiroTouch Reports Matter for Denial and A/R Tracking?
We describe reports by purpose. The vendor lists end-of-day summaries, payor trends, ledgers, the Claims Worklist and a payment reconciliation report.
| Purpose | What we look for |
|---|---|
| Charge lag | Days from visit to charge entry, by provider and location |
| Claim status | Sent, rejected or unpaid claims, by payer and age |
| A/R aging by payer | Balances in 0 to 30, 31 to 60, 61 to 90 and over 90 days |
| Denial trends | Top reasons by CARC and RARC, by payer and provider |
| Payment variance | Paid amounts against the contract, and unmatched or short remittances |
| Payment reconciliation | Card payouts and fees against posted payments |
These feed our revenue cycle management service.

Common Questions
Straight answers from the team that does the work.
Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.
01Can an outside billing company work inside ChiroTouch?
Yes, if your practice gives it access. Your administrator adds a named user for each person after a BAA is signed, with role-based access and two-step login. We never use shared logins. Insurance claims live in ChiroTouch Advanced, so that is the edition we work in.
02What access does a billing partner need in ChiroTouch?
A named user for each team member with role-based access, a signed BAA first and two-step login. We also need your clearinghouse product and the payer portals we may open, and we agree scope and reporting with you at setup.
03Which ChiroTouch reports matter for denial and A/R tracking?
The Claims Worklist for claim status, payor trends and ledgers for A/R by payer, and end-of-day summaries for charge and payment checks. We pair them with denial reasons by CARC and RARC, and with payment variance checks. Report names and filters vary by edition, so we confirm them with you at setup.
04How do claims move from ChiroTouch to the clearinghouse and payer?
In Advanced, claims go through the clearinghouse your account uses: CT ProClear, powered by TriZetto, or CT MaxClear, tied to Change Healthcare. The provider's note codes feed the billing screen, claims go out electronically, and the Claims Worklist tracks status.
05How does ChiroTouch post ERA and 835 remittances?
The vendor says ERA files download automatically and payments post to the patient ledger, with payments matched to claims and discrepancies flagged. We still review each remittance against the claim, because adjustments, short pays and denials each need a human decision.
06What billing mistakes are common in ChiroTouch (charge capture, modifiers, demographics)?
The usual misses are visits with no charge, AT on maintenance care or missing on active care, a subluxation level that does not match the note, a blank date of initial treatment, and demographic or eligibility mismatches. We check each before submission.
07Does Medicare require the AT modifier on chiropractic claims?
Yes, for active or corrective care. MLN SE1602 says every claim for 98940, 98941 or 98942 dated on or after October 1, 2004 should carry AT when that care is given, and MACs deny those codes without it. AT is never used for maintenance therapy.
08Which ChiroTouch product do billers need for claims, ProClear or MaxClear?
Which one your account has depends on the clearinghouse your practice enrolled with. ProClear runs on TriZetto and MaxClear on Change Healthcare, and both are Advanced only. We confirm which one is active at setup, so enrollment, remittances and claim status are read from the right place.
09Can a practice on ChiroTouch Core use an outside biller for insurance claims?
ChiroTouch describes Core as its cash-based edition, and the clearinghouse products and eligibility tool are listed as Advanced only. A practice that starts billing payers should ask the vendor about moving to Advanced first, and then we can set up claims, posting and denial work.
10Medical billing service for chiropractors using ChiroTouch
Yes. Nucleus Align works inside ChiroTouch on accounts you create, so you keep your system. We work your ChiroTouch claim queues, checking the AT modifier and note support before claims go out, then posting remits and working denials. Claims go out within 24 hours of charge entry and denials are worked within 48 hours.
11Can you bill insurance from ChiroTouch for our chiropractic practice?
You stay on ChiroTouch, and we take the insurance claims, posting and denial work you hand over, in the queues you assign. Claims go out within 24 hours of charge entry and denials are worked within 48 hours. Nucleus Align does not move you to another platform. Book a call to agree the scope.
12Can an outsourced billing team use our ChiroTouch logins?
No. Nucleus Align uses its own named ChiroTouch accounts, which you create with role-based access, behind two-step login and a signed BAA. No one shares a password. Each user is named and you decide the role. Your own staff keep their logins. You can remove access at any time.
