WebPT billing services for PT, OT and SLP practices

We work inside your WebPT account through named, two-step accounts you control, after a signed BAA. We check therapy modifiers, units and plan of care dates, send claims through your clearinghouse, post remittances and work denials and A/R.

In short: we work in your WebPT account with our own named logins, in the queues you assign, and report from your own data. We claim no partnership with the vendor, and the therapy billing rules here come from CMS manuals and the CMS therapy services page.

What is WebPT and who bills out of it?

WebPT is a software company in Phoenix, Arizona, that builds an electronic medical record and billing platform for rehab therapy. Its site names physical therapy, occupational therapy, speech-language pathology, behavioral health and chiropractic among the practices it serves.

Its billing pages show three paths: WebPT Billing for medium and enterprise practices, Therabill for small practices, and WebPT RCM, a full-service option where the vendor's own team bills for you.

The vendor's own FAQ says many clinics use third-party billers and that its tools work with internal or external teams. That is the setup this page covers: your clinic keeps WebPT, and we work the revenue cycle inside it. WebPT also sells its own RCM service.

How does an outside billing team get into your WebPT account?

We work inside accounts you create and control.

1
Sign the BAA first
Nucleus Align signs a Business Associate Agreement (BAA) with your practice before any record is opened. Our staff have HIPAA training and NDAs in place.
2
Your administrator adds named users
Each person on our team gets their own login with role-based access.
3
Turn on two-step login
Every account we use is protected with two-step login, including the clearinghouse and payer portals we reach for you.
4
Confirm product and routing
We agree which WebPT billing product you run, how claims reach the payer, and which payer portals we may use.
5
Agree scope and reporting
Which work queues we own and which stay with your front desk are written down at setup, along with how we report.

Which therapy modifiers must be on a WebPT claim?

Every Medicare outpatient therapy line needs the modifier for its discipline, per Claims Processing Manual chapter 5, section 20.1. WebPT says its billing has built-in rehab-specific modifiers.

ModifierWhen it applies
GPOutpatient physical therapy services
GOOutpatient occupational therapy services
GNOutpatient speech-language pathology services
CQPhysical therapy furnished in whole or in part by a physical therapist assistant, added next to GP
COOccupational therapy furnished in whole or in part by an occupational therapy assistant, added next to GO

CMS requires CQ and CO from January 1, 2020, and pays services carrying them at 85 percent of the fee schedule amount from January 1, 2022. We check the rendering clinician on each line against the modifier before a claim leaves.

What is the KX threshold for 2026, and what happens without the modifier?

For calendar year 2026 CMS sets the KX modifier threshold at $2,480 for physical therapy and speech-language pathology combined, and a separate $2,480 for occupational therapy. CMS states that claims for services over the threshold without the KX modifier are denied.

KX is the provider's confirmation that the services are medically necessary and documented. CMS warns that routine KX use on claims far below the threshold can draw scrutiny.

In WebPT we track each Medicare patient's yearly running total and add KX only past the threshold, where the note supports skilled care. CMS indexes the figure each year, so we use the amount on the CMS therapy services page for the year of service.

How many units can you bill under the 8-minute rule?

A single timed code reaches one unit at 8 minutes of direct, one-on-one treatment, and each further unit needs about 15 more minutes. The Medicare Claims Processing Manual, chapter 5, section 20.2 lists the bands, and the 8-minute rule governs how minutes turn into timed code units.

UnitsTotal minutes in the day, per CMS
18 through 22
223 through 37
338 through 52
453 through 67

When several timed codes are delivered on the same day, CMS says the total timed minutes decide the number of units, and the units billed cannot exceed what those total minutes allow. Only time spent directly treating the patient counts, not waiting or rest breaks.

Example with made-up numbers: 20 plus 18 minutes of two timed services is 38 minutes, so 3 units. We check the unit count on each WebPT charge against the documented minutes before the claim is built.

What plan of care certification does Medicare expect before you bill?

Medicare covers outpatient therapy under a plan of care that a physician or non-physician practitioner has certified. Medicare Benefit Policy Manual chapter 15, section 220.1.3, says certification needs a dated signature on the plan or on another document approving it. The rows below come from that section, and the last row from Claims Processing Manual chapter 5, section 20.1.

ItemWhat CMS says
Timely initial certificationSigned and dated within 30 days after the first day of treatment, which includes the evaluation
Verbal orderMust be followed by a signature within 14 days to count as timely
RecertificationWhen a significant change is needed, or at least every 90 days after treatment starts
Institutional claimsReport the date the plan was set or last reviewed in occurrence code 17, 29 or 30

CMS says to keep the certification in the clinical record. We match the first date of service, the certification date and the recertification date in the WebPT chart before we release claims and send missing signatures back to your clinic.

How do claims move from WebPT to the payer, and how are payments posted?

Professional therapy claims go out on the CMS-1500 form or its electronic version, the 837P. WebPT's billing pages say clearinghouse functions, including electronic workers' compensation and auto claims, are included in the subscription.

Hospitals, skilled nursing facilities and home health agencies bill outpatient therapy on the institutional claim, the UB-04 or its electronic form, the 837I. The vendor lists Medicare Part A institutional billing as a capability of WebPT Billing for medium and enterprise practices.

On the payment side, the vendor describes automated posting, editable 835 files and rejection alerts. An ERA is the electronic remittance advice, X12 835 format. We post from it, then work any line carrying a CARC or RARC that signals a denial or reduction. See denial management for that follow-up.

What do we do in WebPT each day?

We follow each visit from charge to cash in the queues you assign.

  • Charge review: discipline modifier, CQ or CO, units against minutes and diagnosis pointers.
  • Claim edits: automated eligibility checks and multiple claim scrubbing rules, then claims held by the platform or clearinghouse are cleared.
  • Submission: claims go out through your clearinghouse within 24 hours of charge entry.
  • Posting: payments and adjustments from the 835, with short pays flagged.
  • Denial work: denials are worked within 48 hours, with appeals built from the remittance codes.
  • A/R follow-up and patient statements: unpaid payer balances by age, then patient balances after insurance.

Which reports do we pull for a therapy practice?

We describe reports by purpose, because the names and filters depend on your WebPT product and build. The vendor lists built-in reports and claim-level status tracking, and scope and reporting are agreed at setup.

PurposeWhat we look for
Charge lagDays from visit to charge entry, by clinic and clinician
Claim statusSent, rejected or unpaid claims, by payer and age
A/R aging by payerBalances in 0 to 30, 31 to 60, 61 to 90 and over 90 days, split by payer
Denial trendsTop denial reasons by CARC and RARC, by payer, clinic and clinician
KX watch listMedicare patients nearing the annual threshold

These measures feed our revenue cycle management service.

How we handle billing in WebPT

We run billing inside your WebPT setup through named accounts, your clearinghouse and your payer portals, and we report from your own data. We claim no partnership with the vendor. The rules behind this work are on our physical therapy billing page; see also CentralReach, the EHR hub and security.

FAQ

Common questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside WebPT?

Yes, if your practice gives it access. WebPT's own billing FAQ says many clinics use third-party billers and that its tools work with internal or external teams. Your administrator creates a named user for each person after a BAA is signed. We never use shared logins.

02What access does a billing partner need in WebPT?

Named logins with role-based access to charges, claims, remittances and billing reports, protected by two-step login. The vendor lists role-based workflows on WebPT Billing, so billing roles can stay apart from clinical charting, and you can switch access off at any time.

03Which WebPT reports matter for denial and A/R tracking?

We look at charge lag, claim status by payer, A/R aging by payer, denial trends by CARC and RARC, and payment variance. Report names depend on your product and build, and the vendor lists built-in reports and claim-level status tracking, so scope and reporting are agreed at setup.

04How do claims move from WebPT to the clearinghouse and payer?

The charge becomes a claim in the billing module, passes the platform's rules, and goes out as an 837P for professional claims. WebPT says clearinghouse functions are included in the subscription. We send claims within 24 hours of charge entry.

05How does WebPT post ERA and 835 remittances?

The vendor describes automated payment posting with customizable rules, plus editable 835 files for corrections. We post from the 835, flag short pays, and work lines that carry denial codes. An edited 835 should trace to a named user, so your audit trail stays clean.

06What billing mistakes are common in therapy charts and claims?

These are general checks, not product defects. We look for a missing GP, GO or GN modifier, a missing CQ or CO on assistant services, units above the documented minutes, a plan of care certified late, and KX added without support. We also check demographics and eligibility.

07Is WebPT data safe with an offsite billing team under a BAA?

A signed Business Associate Agreement comes first, then named accounts, two-step login and role limits you control. Our staff have HIPAA training and NDAs, backups are encrypted and automatic, and a compliance officer oversees it. The security page has the detail.

08Does WebPT handle institutional UB-04 claims or only professional claims?

The vendor lists Medicare Part A institutional billing as a capability of WebPT Billing for medium and enterprise practices. Private therapy practices mostly bill on the CMS-1500 or 837P. Hospitals, skilled nursing facilities and home health agencies bill outpatient therapy on the institutional claim, so confirm your module.

09What does outsourcing billing cost for a WebPT practice?

Our fees do not depend on which EHR you use. Current pricing is on the pricing page. WebPT also sells its own RCM service, WebPT RCM, so you can compare the two options before you decide who runs your billing.

10Is there a billing company for physical therapy practices on WebPT?

Yes. Nucleus Align supports rehab therapy practices that run WebPT. With named logins you grant, we check modifiers, the KX threshold and timed units before claims go out within 24 hours of charge entry, then post remittances and work denials. Scope and reporting are agreed with you at setup.
