ChartLogic billing services for orthopedic, ENT and pain practices

We work inside your ChartLogic account through named, two-step logins that you control, after a signed BAA. We review each charge against the exam, check modifiers and diagnosis pointers, send claims, post remittances and work denials and A/R.

In short: we work in your ChartLogic account with our own named logins, on the billing tasks you assign, and report from your own data. ChartLogic also sells billing services, so we confirm who owns each task first. We claim no partnership with the vendor.

What is ChartLogic and who uses it?

ChartLogic is an ambulatory EHR and practice management suite. Its site describes it as the Ambulatory Division of Medsphere Systems Corporation, based in Sandy, Utah, and says the company has been building software since 1994.

The vendor says the software was first created at a podiatry clinic. Its specialty pages lead with orthopedic surgery, otolaryngology (ENT), pain management and podiatry.

That puts ChartLogic in the specialty ambulatory group on our EHR hub, next to ModMed for dermatology, ophthalmology and orthopedics, and WebPT for rehab therapy. These practices bill professional claims on the CMS-1500 and often carry global surgery periods.

Which ChartLogic products touch billing?

Four product lines matter to a biller. Names come from the vendor's own solution pages.

ProductWhat the vendor listsWhat it means for billing
ChartLogic EHRNote Template Builder, Voice Dictation, ePrescribe, Labs, Patient Tracker, Referral Reply, Data ShareCodes and modifiers are picked inside the exam, so charge accuracy starts with the clinician's template.
ChartLogic PMAppointment Scheduler, Claims Center, Collection Center, Patient Registration and Demographics, Reporting and AdministrationClaims, collections and billing reports live here. Registration errors travel into every claim.
Revenue Cycle ManagementA service from the vendor covering eligibility, coding, claim status, collections, credentialing and old A/RIf your practice buys it, the vendor's staff already work your claims. We agree task ownership first.
InterfacesDemographic, charge and lab interfacesThese decide whether charges and patient data move by hand or by feed.

What access does a billing partner need in ChartLogic?

Your practice creates a named, role-based account for each person on our team. Nothing is shared, and no record is opened until the paperwork is signed.

1
BAA before access
Nucleus Align signs a Business Associate Agreement (BAA) first. Our staff carry HIPAA training and NDAs.
2
Named users from your administrator
One login per person, role-based, limited to the billing, claim and payment screens we need.
3
Two-step login on
Two-step login protects every account we use, including clearinghouse and payer portals.
4
Map your modules
We note which ChartLogic modules you run, whether a charge interface feeds billing, and whether the vendor's billing service is active.
5
Write down scope and reporting
Task ownership between us, your staff and the vendor, plus how we report, is agreed at setup.

Our security page covers data protection.

How do charges and claims move out of ChartLogic?

Charges are entered in the exam, pass to practice management, and leave as claims through the Claims Center. The vendor's interfaces page says staff enter CPT codes, ICD codes and any modifiers inside the ChartLogic exam, and that a saved and finished exam sends the billing information to the practice management system automatically.

The same page lists a demographic interface that carries appointment, insurance and referring provider data. Its practice management page describes claim scrubbing that catches errors before a claim leaves.

The vendor's pages do not name the clearinghouse behind the Claims Center, so we confirm which one your account uses at setup. At setup we confirm which one your account uses and whether professional claims leave as an electronic 837P. Claims go out within 24 hours of charge entry.

What billing mistakes are common in ChartLogic?

We attribute no defects to ChartLogic. These checks follow from how the vendor says charges are entered and from CMS claim rules.

CheckWhy it mattersSource
Codes picked in the exam match the noteThe vendor says codes and modifiers are entered in the exam and sent on when it is finished. A wrong pick in a template reaches the claim without a second look, so we compare charge to note.ChartLogic interfaces page
Up to four modifiers per lineCMS says the CMS-1500 can capture up to four modifiers with the HCPCS code in item 24D. We check order and payer edits for each line.CMS Claims Processing Manual, chapter 26, item 24D
One diagnosis pointer per lineItem 24E takes one reference letter or number per line, tying the service to a diagnosis in item 21. Wrong pointers invite medical necessity denials.CMS Claims Processing Manual, chapter 26, item 24E
Referring provider in item 17CMS says a referring or ordering physician goes in item 17, with a separate claim for each one. The name must match the referral on file.CMS Claims Processing Manual, chapter 26, item 17
Rendering provider NPI on each lineItem 24J carries the rendering provider's NPI, so surgeon and assistant lines need the right provider selected.CMS Claims Processing Manual, chapter 26, item 24J
Global period before billing a visitCMS says surgical codes with a 000, 010 or 090 entry carry a postoperative period, and preoperative and related visits by the surgeon fall inside the package. We check dates before an office visit goes out.CMS Claims Processing Manual, chapter 12, section 40.1

How does ChartLogic post ERA and 835 remittances?

The vendor's pages do not describe how it posts an ERA, the electronic 835 remittance file, so we confirm at setup how your account posts it. At setup we confirm whether the 835 arrives through your clearinghouse or from the payer, and whether posting is automatic or manual.

We post payments and adjustments line by line, check each against the contract rate or fee schedule, and flag short pays for the denial queue. Rejections that never reached the payer are kept apart from denials so each gets the right fix.

What do we do in ChartLogic each day?

Each procedure is followed from charge to cash, on the screens your administrator opens for us.

  • Charge review against the exam note: code, modifiers, units, dates, place of service, diagnosis. See medical coding.
  • Claim edits: automated eligibility checks and multiple claim scrubbing rules added to the vendor's own scrubbing.
  • Submission through your clearinghouse route within 24 hours of charge entry. See medical billing.
  • Remittance posting from the 835, with short pays flagged.
  • Denials worked within 48 hours. See denial management.
  • A/R follow-up and patient statements, payer balances first.

Which ChartLogic reports matter for denial and A/R tracking?

The vendor lists Reporting and Administration inside practice management, with billing and workflow analysis reports, but no report names. We describe reports by purpose, and scope is agreed at setup.

PurposeWhat we look for
Charge lagDays from procedure or visit to charge entry, by surgeon and location
Claim statusSent, rejected or unpaid claims, by payer and age
A/R aging by payerBalances by age bucket, split by payer and patient
Denial trendsTop denial reasons by CARC and RARC, by payer and procedure
Payment variancePaid amounts against contract rates or the fee schedule

How we handle billing in ChartLogic

Inside your ChartLogic account we work through named logins and your own claim route, and report from your data. We claim no partnership with the vendor. If the vendor's billing service is active, queues are split in writing first so no claim is worked twice.

FAQ

Common questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside ChartLogic?

Yes, once your practice grants access. ChartLogic also sells billing services, so we first agree which tasks the vendor keeps and which we own. Your administrator adds named users for us after a BAA is signed, and logins are never shared.

02What access does a billing partner need in ChartLogic?

A named, role-based account for each person, limited to billing, claim and payment screens, with two-step login on. Your administrator creates them after the BAA is signed. We also ask which modules and interfaces you run, so we know where charges originate.

03Which ChartLogic reports matter for denial and A/R tracking?

The vendor lists reporting inside practice management but no report names, so we work by purpose: charge lag, claim status, A/R aging by payer, denial trends and payment variance. Scope and reporting are agreed at setup, and the figures come from your own data.

04How do claims move from ChartLogic to the clearinghouse and payer?

Codes are entered in the exam, the finished exam passes billing data to practice management, and claims leave through the Claims Center. The vendor does not name its clearinghouse, so we confirm yours at setup. We send claims within 24 hours of charge entry.

05How does ChartLogic post ERA and 835 remittances?

The vendor's pages do not describe its ERA posting, so we confirm at setup how your account posts it. At setup we confirm whether the 835 comes through your clearinghouse or the payer and whether posting is automatic. We then post line by line, compare to the contract rate and flag short pays for the denial queue.

06What billing mistakes are common in ChartLogic (charge capture, modifiers, demographics)?

We attribute no defects to the vendor. The checks we run are charge against the exam note, modifier order and pointers on the CMS-1500, referring provider data in item 17 and demographics that carry into every claim. Surgical practices also need the global period checked before a visit is billed.

07Does ChartLogic offer its own billing service?

Yes. The vendor lists a Revenue Cycle Management service covering eligibility, coding, claim status, collections, credentialing and old A/R. If your practice uses it, we agree in writing which queues stay with the vendor and which we handle, so no claim is worked twice.

08Which specialties does ChartLogic target?

The vendor's specialty pages lead with orthopedic surgery, otolaryngology, pain management and podiatry, and its list covers dozens of specialties, including a group for surgical specialties. Our billing guides for those four specialties are linked at the end of this page.

09What is the ChartLogic charge interface?

It is a feed the vendor describes on its interfaces page. Staff enter CPT codes, ICD codes and modifiers in the exam, and a finished exam sends the billing information to the practice management system. We confirm whether your practice uses it and check what it sends.

10Why do global periods matter for ChartLogic orthopedic billing?

CMS says surgical codes with a 000, 010 or 090 entry carry a postoperative period, and related surgeon visits fall inside the package. A visit billed inside it can be denied. We check procedure dates against the period before office visits go out.

11Medical billing service for ChartLogic users

Yes. Nucleus Align works inside ChartLogic on accounts you create, so you keep your system. We work the billing tasks you assign in ChartLogic, checking the charge interface, global period and modifier edits before claims go out, then posting remits and working denials. Claims go out within 24 hours of charge entry and denials are worked within 48 hours.

12Can Nucleus Align work in ChartLogic without sharing a login?

Yes. Each person on our team gets a named ChartLogic account of their own under a business associate agreement, with two-step login. Your account owner sets and removes each user. That keeps every action traceable to one person, and you decide when access ends.
