Azalea Health Billing and RCM Support for Rural Hospitals and Clinics
An outside billing team can work inside Azalea Health through named accounts you create under a signed BAA. Azalea also sells its own billing service, so first settle who works each claim.
In short: outside billing teams can work in Azalea Health, a cloud EHR and practice management system aimed at rural and critical access hospitals and rural clinics, from named, two-step accounts under a BAA. Azalea also offers its own billing team, so we agree queue ownership with you at setup.
Already on this system? We work in it. No EHR, or tired of paying for one? Our billing clients get EHR or PM software free.
What Is Azalea Health, and Which Facilities Run It?
Azalea Health is a cloud-based EHR and practice management vendor. Its About page says the company was founded in Valdosta, Georgia in 2008 and designs its platform for smaller hospitals and healthcare practices.
Its own pages name rural hospitals, critical access hospitals (CAHs), rural emergency hospitals and rural health clinics (RHCs) as the core users. Under 42 CFR 413.70, Medicare pays a critical access hospital 101 percent of reasonable costs for inpatient services, so its payment rules differ from a standard hospital.
One organization can send hospital and clinic claims out of the same system. See all EHR systems we work in.
Which Azalea Products Touch Billing?
Five parts of the Azalea line matter to a biller. The names and descriptions below come from Azalea's own solution pages.
| Azalea product | What the vendor says it does |
|---|---|
| Hospital EHR | Single-screen charting for rural and critical access hospitals, with automated claim generation from documentation |
| Ambulatory EHR | Clinic EHR with built-in practice management, smart claim generation and clearinghouse integration |
| Practice Management | Scheduling, split claim billing, claim management, patient statements and eligibility checks; can run beside another EHR |
| RCM Billing Services | Azalea's own outsourced billing team |
| Healthcare Analytics | Dashboards, canned report subscriptions and an ad hoc report builder |
Which Claim Types Does a Rural Hospital with Clinics Send?
Hospital and RHC claims are institutional claims, sent as an X12 837I transaction. Under 45 CFR 162.1102, the institutional claim standard in the current eCFR through August 14, 2027 is the 005010X223 version of the 837 with its errata, and the rule lists later dates after that.
CMS says the same for clinics. Section 50 of the Medicare Claims Processing Manual, Chapter 9, states that the electronic format required for billing RHC and FQHC services is the ASC X12 837 institutional claim transaction. The same chapter sends some technical components, such as the technical part of an EKG at an independent clinic, to Medicare on professional claims (CMS-1500 or 837P), so we check which path each line takes.
Two RHC details from that chapter matter on every claim. Revenue code 0521 is listed as the clinic visit by a member to the RHC or FQHC. And an RHC should report modifier CG on one line with the HCPCS code for the primary reason for the face-to-face visit.
On the hospital side, UB-04 accuracy depends on bill type, revenue codes and patient status. See hospital and facility billing.
Payments come back as an ERA. 45 CFR 162.1602 names ASC X12N 005010X221 as the 835 standard, and we post against that file.
Does Azalea Also Bill for Its Customers, and Who Owns the Claims Then?
Yes. Azalea's RCM Billing Services page describes outsourced billing that covers claim scrubbing and submission, denials and appeals, accounts receivable management, patient collections and a call center. The vendor says it uses its own reporting and Analytics platform for that work.
So before anyone else logs in, settle who works which queue. If your staff bill in Azalea, we take the queues you assign. If Azalea's team handles claims, we wait until you end that service and agree the handover with the vendor in writing. If the work is split, we write down who owns each queue.
Azalea's Hospital EHR page says it works with in-house staff or a current billing partner.
What Access Does a Billing Partner Need in Azalea Health?
We need named user accounts that you create, limited to billing roles, after a BAA is signed. We never share logins.
Our security page lists how we protect data: HIPAA training and NDAs for all staff, encrypted automatic backups and a dedicated compliance officer.
What Do We Do Inside Azalea Health?
- Charge review against the documentation, so a missing diagnosis, provider or revenue code is caught before the claim leaves.
- Claim edits and submission through your clearinghouse, with claims sent within 24 hours of charge entry.
- Posting of ERA 835 files and manual remittances, including adjustment codes.
- Denial work queues, with denials worked within 48 hours.
- A/R follow-up by payer and age, then patient statements.
Which Azalea Health Reports Matter for Denial and A/R Tracking?
Azalea's RCM page describes real-time and prescheduled reporting, canned report subscriptions and an ad hoc report builder in Azalea Analytics. We describe what we need by purpose and agree exact reports at setup.
| What we need to see | Why |
|---|---|
| Charge lag, from service date to charge entered | Late charges delay every payment behind them |
| Claim status by payer | Shows what is unsent, rejected, pending or paid |
| A/R aging by payer | Shows which payer is holding money |
| Denial trends by reason code | Points to a fix at the source, such as registration |
| Payment variance against the contract | Catches underpayments |
What Billing Mistakes Are Common in Azalea Health?
We do not attribute defects to Azalea. These are the checks we run on any rural hospital and clinic setup, tied to official rules where one exists.
| Check | Why it matters |
|---|---|
| RHC claims carry revenue code 0521 and modifier CG on the right line | Medicare Claims Processing Manual, Chapter 9, lists revenue code 0521 and says RHCs should report modifier CG on a visit line |
| Clinic charges go out as institutional 837I claims for Medicare | The same chapter names the 837 institutional claim for RHC billing, with listed exceptions for some technical components |
| Split claims by payer or service are all accounted for | Azalea's practice management page says claims can split automatically by payer or service rules, so each part needs its own follow-up |
| Swing-bed and inpatient stays are billed as the separate situations they are | Azalea's hospital FAQ says its workflows reflect swing beds, so we confirm the claim type per stay |
How We Handle Billing in Azalea Health
We start by finding out who owns each claim today, then take the queues you give us from named accounts under a signed BAA. Claims run through your clearinghouse, and we align each workflow with your system and payer rules.
Other community hospital systems have pages too: TruBridge, MEDITECH and, for clinics, athenahealth. To talk through your setup, book a call.

Common Questions
Straight answers from the team that does the work.
Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.
01Can an outside billing company work inside Azalea Health?
Yes, if you give it access. Your Azalea administrator creates a named user for each person on our team, limited to billing roles, after a BAA is signed. We never share logins. Scope and reporting are agreed with you at setup, and the accounts stay under your control.
02What access does a billing partner need in Azalea Health?
A billing partner needs named accounts with role-based access to charges, claims, payments and reports, plus two-step login. It also needs access to your clearinghouse and payer portals under the same rule. The BAA comes first, and the accounts stay under your administrator's control.
03Which Azalea Health reports matter for denial and A/R tracking?
Charge lag, claim status by payer, A/R aging by payer, denial trends and payment variance. Azalea's pages describe prebuilt billing reports, canned report subscriptions and an ad hoc report builder in Azalea Analytics. We agree the exact reports with you at setup.
04How do claims move from Azalea Health to the clearinghouse and payer?
Azalea's practice management page says claims can go through your existing clearinghouse or Azalea's built-in connection. The payer replies with acceptance or rejection files, then later an 835 remittance. We confirm at setup which route you use and who watches the rejection reports.
05How does Azalea Health post ERA and 835 remittances?
Azalea does not publish its internal posting steps, so we confirm at setup how your account posts. The federal standard for the remittance file is ASC X12N 005010X221 under 45 CFR 162.1602. We ask how your clearinghouse delivers the file, post payments and adjustments against it, and match totals to the bank deposit.
06What billing mistakes are common in Azalea Health (charge capture, modifiers, demographics)?
We do not name defects in the vendor's system. On any rural clinic setup we check modifier CG and revenue code 0521 on RHC lines, hospital bill type and revenue codes, split claims, and registration data. Mistakes at charge entry cause most denials downstream.
07Is Azalea Health data safe with an offsite billing team under a BAA?
We sign a business associate agreement with every client. All staff get HIPAA training and sign NDAs, every login uses two-step verification, and backups are encrypted and automatic. A dedicated compliance officer oversees it. See our security page for more detail.
08Does Azalea Health produce UB-04 institutional claims as well as CMS-1500 professional claims?
Azalea describes hospital and ambulatory products with automated claim generation from documentation. Medicare RHC claims are institutional, sent as 837I with UB-04 data elements. Professional claims use 837P and CMS-1500 data. We confirm at setup which claim type each charge produces, then check bill type, revenue codes and modifiers.
09Can a billing vendor connect Azalea Health to our existing clearinghouse and payer portals?
Azalea's practice management page says it integrates with existing clearinghouses and also offers a built-in connection. We do not build the connection. You or Azalea set it up, and we work claims through it. Payer portals need named accounts for us, which you request from each payer.
10Is there a billing company that works in Azalea Health?
Yes. Nucleus Align works inside Azalea Health on accounts you create, so you keep your system. We work the billing queues you assign in Azalea Health for a rural hospital or clinic, covering claim checks, ERA posting, denials and A/R. Because Azalea also offers its own billing team, we agree queue ownership at setup. Claims go out within 24 hours of charge entry and denials are worked within 48 hours.
11Can you bill from Azalea Health for a critical access hospital?
You stay on Azalea Health, and we take the hospital and clinic billing queues you hand over once we agree who owns each one. Claims go out within 24 hours of charge entry and denials are worked within 48 hours. Nucleus Align does not move you to another platform. Book a call to agree the scope.
12Do we have to share our Azalea Health passwords with a billing company?
No. You create separate named Azalea Health users for the Nucleus Align team, with only the access you choose, plus two-step login and a signed BAA first. Your Azalea administrator controls what each account can see. Your own staff keep their logins, and you can end ours whenever you decide.
