TruBridge billing services for community and rural hospitals

We work inside your TruBridge system with named, two-step accounts that your hospital controls, after a signed BAA. We review charges, send claims through your clearinghouse, post remittances and work denials and old balances.

In short: we use our own named accounts in your TruBridge environment, only in the work lists you assign. We claim no partnership with the vendor, and every rule on this page comes from CMS, eCFR or X12.

What should a hospital know before outsourcing billing in TruBridge?

TruBridge is the system name this page is written around, and your own build decides what your billing staff see on screen. We name TruBridge features only as the vendor documents them, and map the rest to your build at setup. What we can describe accurately is the claim, remittance and filing rules that apply to any hospital system, and the checks we run against them.

This page is for the community or rural hospital that wants an outside team to carry part or all of its revenue cycle. Many are critical access hospitals, which follow their own payment rules.

We claim no partnership with, or endorsement by, TruBridge. We work inside client systems under the client's own accounts.

Does TruBridge produce UB-04 and CMS-1500 claims?

TruBridge's public pages do not publish a claim-form specification, so we confirm which forms your build produces from a sample claim at setup. Ask your administrator for a sample claim. The form rules themselves come from CMS and X12.

A hospital bills facility services on the UB-04. CMS describes it in Chapter 25 of the Medicare Claims Processing Manual as a uniform institutional provider bill suitable for billing multiple third party payers, and it says the National Uniform Billing Committee (NUBC) maintains the approved coding lists. The electronic version is the 837I. Professional services go on the CMS-1500 or the 837P.

Form locator 4 holds the type of bill. Chapter 25 says the second digit identifies the type of facility, the third classifies the type of care and the fourth is a frequency code showing where the bill sits in the episode of care.

For hospitals that bill both facility and professional lines: Chapter 25 says a claim holding both components, with a Critical Access Hospital claim as one example, reports the rendering provider with qualifier 82.

What access does a billing partner need in TruBridge?

A billing partner needs named user accounts limited to billing roles. You issue and control them, and we start only after the paperwork is signed.

1
BAA first
We sign a Business Associate Agreement (BAA) with your hospital before anyone opens a record. Our staff have HIPAA training and NDAs in place.
2
Named accounts, no sharing
Your system administrator creates one account per team member on our side, with role-based access to the billing screens and work lists we need. Nobody shares a login.
3
Two-step login
Every account uses two-step login. We apply the same rule to the clearinghouse and payer portals we use for you.
4
Split the work
You decide which charge, edit, denial and follow-up queues we take and which stay with your office. We put the split in writing.
5
Read before we write
We begin by reading open charges, rejected claims and aged balances. We move to daily work once your team confirms the split.

You can disable any account at any time. Our security page lists how we protect data.

What do we do inside your TruBridge system?

Our hospital and facility billing service covers inpatient, outpatient and emergency claims.

  • Charge review. We compare each charge with the service record and check that it carries a valid revenue code before it drops to a claim.
  • UB-04 checks. We check type of bill, revenue codes, discharge status, condition codes and authorization numbers against payer rules.
  • Claims out. We send claims within 24 hours of charge entry, through your clearinghouse, so your payer enrollments stay as they are.
  • Remittance posting. We post the ERA to the claim, match adjustments to their reason codes and flag short payments.
  • Denials. We work denials within 48 hours, by payer and reason, and file appeals with the record attached.
  • Old balances. We follow up on aged A/R by payer and patient statements for what remains.

More on the wider process: revenue cycle management.

Which reports matter for denial and A/R tracking?

The reports that matter measure lag, status, age and variance. We describe them by purpose, because report names differ between builds and we do not assume yours.

Purposes we set up with a client. The exact report names come from your system.
PurposeWhat it showsTypical rhythm
Charge lagDays from date of service to charge entry, by departmentWeekly
Claim statusClaims waiting, sent, accepted or rejectedDaily
A/R aging by payerOpen balances in age bands for each payerWeekly
Denial trendsDenials grouped by payer, reason code and departmentMonthly
Payment variancePaid amount against the amount the contract predictsMonthly
Unmatched remittancesPayments received but not yet applied to a claimDaily

Denial reports carry more value when they group by CARC and RARC codes, since those codes come from the payer's remittance and say why a line was adjusted.

What billing checks matter most for rural hospital claims?

We do not attribute defects to TruBridge. The table lists general checks that come from CMS and X12, which we run on any claim a hospital system produces.

General checks, each tied to the source named.

We trace each rejection to its cause in the build, then fix the cause as well as the claim.

How we handle billing in TruBridge

We work in the accounts you grant, under a signed BAA, and report from your own data. We start with a review of open charges and denials, agree the split of work, then move into daily claims, posting and follow-up.

We align each EHR, clearinghouse and payer portal workflow with verified team experience, claim validation and reconciliation controls. Compare the pages for MEDITECH, Altera Digital Health and Epic, or browse every EHR system we describe.

If your hospital runs TruBridge and wants a second pair of eyes on denials or aged balances, book a call.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside TruBridge?

Yes, if your hospital grants access to its system. Your administrator creates a named account for each person, limited to billing roles, after a signed BAA. We use those accounts and no shared logins, so your audit trail shows who did what.

02What access does a billing partner need in TruBridge?

Named accounts with role-based access to the billing screens and work lists you assign, plus two-step login on each. In practice that means charges, claims, remittances and denial queues, and access to your clearinghouse. You grant it, you can see it in your audit trail, and you can remove it at any time.

03Which TruBridge reports matter for denial and A/R tracking?

Charge lag, claim status, A/R aging by payer, denial trends and payment variance. We describe them by purpose because report names differ by build. Group denial reports by CARC and RARC codes from the remittance, since they say why a line was adjusted.

04How do claims move from TruBridge to the clearinghouse and payer?

TruBridge does not publish how it routes claims, so we confirm the route at setup. In general, the hospital system creates an X12 837 institutional file, the clearinghouse checks and forwards it to the payer, and the payer returns acknowledgements and later an 835 remittance. We confirm your actual path during setup.

05How does TruBridge post ERA and 835 remittances?

We cannot confirm TruBridge's posting method from public documents. The standard is fixed: 45 CFR 162.1602 names the X12 835, version 005010X221, for remittance advice. We check that each payment and adjustment lands on the right claim, and we flag any remittance that stays unmatched.

06What billing mistakes are common in TruBridge (charge capture, modifiers, demographics)?

We do not attribute errors to TruBridge. In any hospital system the common causes are charges missing a revenue code, a wrong type of bill frequency digit, absent authorization numbers, and demographic mismatches with the payer. We trace each rejection to its cause in your build and fix the cause.

07Is TruBridge data safe with an offsite billing team under a BAA?

We sign a BAA before access. Our staff have HIPAA training and NDAs, every system we use has two-step login, backups are encrypted and automatic, and a compliance officer oversees it. You control the accounts and can disable them at any time.

08Does TruBridge produce UB-04 institutional claims as well as CMS-1500 professional claims?

TruBridge's public pages do not list the forms it produces, so we ask your administrator for a sample claim at setup. Hospitals bill facility services on the UB-04, sent as the 837I, and professional services on the CMS-1500, sent as the 837P. We check whichever form your build creates.

09Can a billing vendor connect TruBridge to our existing clearinghouse and payer portals?

We submit through your existing clearinghouse, so your payer enrollments stay in place. For payer portals, you add our named users with two-step login. We do not claim any integration with TruBridge itself. We work through the connections your hospital already has.

10What does outsourcing billing cost for a TruBridge facility?

Our pricing page is the only place we publish fees, and the model does not depend on which EHR you use. We confirm the exact terms for your hospital in writing before any work starts, so you see the scope and the fee together.

11Is there a billing company that works in TruBridge?

Yes. Nucleus Align works inside TruBridge for community and rural hospitals, using named, role-limited accounts you grant. We handle claim review, submission, remittance posting and denial work under a signed BAA, with scope and reporting agreed at setup. Scope and reporting are agreed with you at setup.
