MEDITECH billing services for community and critical access hospitals

We work inside your MEDITECH system through named, two-step accounts you control, after a signed BAA. We review charges, submit claims through your clearinghouse, post remittances and work denials and A/R, and we match our process to your build.

In short: we work in your MEDITECH environment with our own named user accounts, in the work lists you assign, and report from your own data. We never claim a vendor partnership, and every rule here comes from CMS or X12.

What is MEDITECH and who uses it?

MEDITECH is the product name of Medical Information Technology, Inc., and its current EHR platform is MEDITECH Expanse. On its own site, the vendor describes Expanse as its EHR platform and as a cloud-native, highly scalable system built for organizations of all sizes and care settings.

The vendor's care-settings menu lists acute care, ambulatory care, critical care, the emergency department, mental health, home care and hospice, post-acute, surgical services and therapies. Its product pages also name critical access hospitals among Expanse users.

MEDITECH also offers Expanse on its cloud-based MEDITECH as a Service (MaaS) platform. Hosting does not change the claim forms or payer rules your billing team works under.

Product names as listed on ehr.meditech.com. We use them only to describe what you may see on screen.

Does MEDITECH produce UB-04 and CMS-1500 claims?

MEDITECH's public pages do not publish a claim-form specification, so what your system sends depends on your hospital's build, which we confirm from a sample claim at setup. The form rules come from CMS and X12 and apply in any system.

A hospital bills facility services on the UB-04, also called Form CMS-1450, which CMS describes in the Medicare Claims Processing Manual, Chapter 25, as a uniform institutional provider bill. Electronically it travels as the X12 837I, and 45 CFR 162.1102 names ASC X12N 005010X223 as the institutional claim standard. Professional services go on the CMS-1500 or the 837P.

Critical access hospitals add a wrinkle. The same CMS chapter says a combined claim, one that holds both facility and professional components, can be required, and it gives a Critical Access Hospital claim as an example of when to report the rendering provider. Under 42 CFR 413.70(b)(3), a CAH can elect to be paid for outpatient facility services at 101 percent of reasonable costs, plus 115 percent of the fee schedule amount for professional services of practitioners who reassigned their billing rights to the CAH.

If your hospital runs that election, we confirm early which claims carry professional lines. Our critical access hospital billing rules page covers the payment side.

How do we get access to your MEDITECH system?

We work inside the accounts and permissions you control. Nothing starts before the Business Associate Agreement (BAA) is signed, and no one on our side shares a login.

1
Sign the BAA
Nucleus Align signs a Business Associate Agreement with you first. Staff are trained on HIPAA and sign NDAs before they see any record.
2
You create named accounts
Your MEDITECH administrator creates one named user account for each of our team members, with role-based access limited to billing work. There are no shared logins.
3
Two-step login
Every account uses two-step login, and we apply it on every other system we touch for you, such as the clearinghouse and payer portals.
4
Agree the work lists
You tell us which charge, claim edit, denial and follow-up lists we own, and which stay with your staff. We write that split down before go-live.
5
Start with a review
We begin by reviewing open charges and open claims, then move into daily work once your team confirms the split.

Your administrator can switch off any account at any time. Our security page lists our data protections.

What do we do inside MEDITECH for a community hospital?

We handle the billing work between a patient's charges and the final payment, using your system, your clearinghouse and your payer portals. Claims go out within 24 hours of charge entry, and denials are worked within 48 hours.

  • Charge review. We check that charges are complete and sit under the right revenue code, so the claim does not stall for a missing line.
  • Claim edits. We work the edit lists before submission and fix the cause, such as a missing authorization number, rather than only clearing the flag.
  • Submission. We submit through your clearinghouse, not ours, so you keep your existing payer enrollments and connection.
  • ERA posting. We post the ERA (X12 835) remittances and match each payment and adjustment to the claim.
  • Denial work. We work denial lists by payer and by reason, and we send appeals with the supporting record.
  • A/R follow-up. We follow up on open accounts receivable by payer and age, oldest and largest balances first.
  • Patient statements. We prepare patient statements and answer billing questions under the process you approve.

For the wider picture, see hospital and facility billing and revenue cycle management. If you work in other hospital systems too, we also describe how we work in Epic and Oracle Health.

Which reports do we pull to track denials and A/R?

We pull reports by purpose, then match each to the name your MEDITECH system uses. MEDITECH's public site does not list report names, so we match each purpose to the name your system uses at setup.

Report purposes we set up with every client. Names come from your own system.
PurposeWhat it tells youHow often
Charge lagDays between service date and charge entry, by departmentWeekly
Claim statusWhich claims are unbilled, submitted, accepted or rejectedDaily
A/R aging by payerOpen balances by payer in age bands, so old balances surfaceWeekly
Denial trendsDenials by payer, by reason code and by departmentWeekly and monthly
Payment variancePaid amount against the expected amount for each payerMonthly
Unposted paymentsRemittances not yet matched to claimsDaily

The reason codes on an 835 are CARC and RARC codes, which come from X12, not from any EHR. Grouping denials by those codes lets us compare payers the same way in any system. See denial management.

What billing checks matter most in MEDITECH?

We do not attribute a billing defect to MEDITECH. These are checks we run on any hospital system, each tied to an official rule.

General checks from CMS and X12 rules. They are not defects in any vendor's software.
CheckWhy it mattersSource
Revenue code and charge amountEach charge in FL 47 needs a revenue code in FL 42 on the same line. A charge under the wrong code can reject or price wronglyMedicare Claims Processing Manual, Chapter 25
Combined facility and professional claimsA CAH claim may need the rendering provider reported, and the CAH outpatient election changes how professional lines are paidChapter 25 and 42 CFR 413.70(b)(3)
Remittance formatHealth plans send remittances in the X12 835 standard, so posting rules should map its adjustment groups and reason codes45 CFR 162.1602
Authorization on the claimPayers may match the claim to the number in FL 63Chapter 25

A claim scrubber catches some of these before the claim leaves, and we add standard claim validation on top.

How we handle billing in MEDITECH

We work in your MEDITECH environment through named accounts you create, in the work lists you assign, and we report from your own data.

We align each EHR, clearinghouse and payer portal workflow with verified team experience, standard claim validation, reconciliation controls and performance monitoring. Our work follows the rules of the claim forms, not any one vendor's layout. See also our guide to choosing a billing partner and the EHR systems hub.

FAQ

Common questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside MEDITECH?

Yes, if your hospital gives it access. Your administrator creates a named account for each person, limited to billing roles, and signs a BAA first. We use those accounts, not shared logins, so your audit trail shows who did what.

02What access does a billing partner need in MEDITECH?

The minimum needed for billing: a named account per person with role-based access to charges, claims, remittances, denials and statements, plus the reports you choose. Each account uses two-step login, and nobody shares a password. We also set up the same named, two-step access for your clearinghouse and payer portals.

03Which MEDITECH reports matter for denial and A/R tracking?

The ones that answer a purpose: charge lag, claim status, A/R aging by payer, denial trends by reason code and payment variance. MEDITECH's public site does not list report names, so we match each purpose to the name your own system uses during setup.

04How do claims move from MEDITECH to the clearinghouse and payer?

Your system builds the claim, the clearinghouse checks and forwards it, and the payer accepts or rejects it. Facility claims travel as the X12 837I and professional claims as the 837P. We submit through your existing clearinghouse, so your payer connections stay as they are.

05How does MEDITECH post ERA and 835 remittances?

MEDITECH's public site does not publish a posting specification, so we confirm at setup how your build posts remittances. The 835 itself is an X12 standard adopted at 45 CFR 162.1602. We post each payment and adjustment to its claim, and send unmatched items to a daily review.

06What billing mistakes are common in MEDITECH, such as charge capture or demographics?

We do not attribute mistakes to the vendor. The common problems are the same in every system: late or missing charges, wrong revenue codes, missing authorization numbers and patient data that does not match the payer's file. We check each before submission and trace causes after a denial.

07Is MEDITECH data safe with an offsite billing team under a BAA?

We sign a Business Associate Agreement with every client, train all staff on HIPAA, and have staff sign NDAs. Each person uses a named account with two-step login, backups are encrypted, and a dedicated compliance officer oversees it. Details are on our security page.

08Does MEDITECH produce UB-04 institutional claims as well as CMS-1500 professional claims?

MEDITECH's public pages do not say which forms it prints, so we check a sample claim from your build at setup. Hospitals bill facility services on the UB-04, sent as the 837I, and professional services on the CMS-1500 or 837P. A critical access hospital may need a combined claim. We work with both.

09Is there a billing company that works in MEDITECH Expanse?

Yes. Nucleus Align supports community and critical access hospitals that run MEDITECH Expanse. We work inside your system with named, role-limited accounts your team grants, covering claim review, submission, remittance posting and denial work. Scope is agreed at setup, and a BAA comes first.
