CPT 97597: Selective Wound Debridement, First 20 Sq Cm
97597 reports selective debridement of an open wound: devitalized tissue is removed without anesthesia, and the code covers the first 20 square centimeters of wound surface or less. Area alone picks the code, with no depth step, and CMS lists it as a service that can be billed as a medical service or under a therapy plan of care.
In short: 97597 is a 000-global, area-only debridement code with an MUE of 1, and 97598 adds each further 20 sq cm. It is on the sometimes-therapy list, so a therapy modifier belongs on it only under a therapy plan of care. It is not reported with surgical debridement 11042 to 11047 for the same wound.
What Does CPT 97597 Report?
97597 is selective debridement of an open wound. The clinician removes devitalized tissue without anesthesia, by sharp instruments or high-pressure waterjet, for example, and the code covers a total wound surface of 20 sq cm or less. The CMS fee schedule file labels it "Dbrdmt opn wnd 1st 20 cm/<", with status A and a 000 global period.
Count the surface of every wound treated in the session toward the 20 sq cm. 97598 is the ZZZ add-on code for each further 20 sq cm, and it cannot be billed without 97597 on the same claim.
| Code | CMS label | Work RVU | Global | MUE (Q4 2026) |
|---|---|---|---|---|
| 97597 | Dbrdmt opn wnd 1st 20 cm/< | 0.75 | 000 | 1 |
| 97598 | Dbrdmt opn wnd addl 20cm/< | 0.49 | ZZZ | 8 |
What Is the Difference Between 97597 and 11042?
97597 is chosen by wound area only. 11042 is surgical debridement, chosen by the deepest tissue removed (subcutaneous tissue for 11042, with 11043 and 11044 for muscle or fascia and bone) and then by area. Both start a 000-day global and both carry status A.
| Point | 97597 | 11042 |
|---|---|---|
| What sets the code | Total wound area | Deepest tissue removed, then area |
| Work RVU (RVU26D) | 0.75 | 0.98 |
| Multiple procedure indicator | 0, no multiple-procedure reduction | 2, standard ranking and reduction |
| MUE adjudication indicator | 3, a clinical edit | 2, a policy edit |
| Therapy designation | Sometimes therapy | Not on the sometimes-therapy table |
The two are never reported for the same wound. NCCI Chapter XI says debridement codes 97597 to 97602 are not reported with surgical debridement 11042 to 11047 for the same wound. The wound-by-wound choice is worked through on wound care billing.
Is 97597 a Therapy Code, and Does It Take a GP Modifier?
97597 is on CMS's sometimes-therapy list, so a physician or other practitioner can bill it as a medical service and a therapist can bill it under a plan of care. Claims Processing Manual chapter 4, section 200.9, lists 97597 and 97598 among the codes that may be paid as non-therapy services for hospital outpatients.
For that payment, the hospital should not append GP, GO or GN, and should not report revenue code 042x, 043x or 044x, with the code. When a qualified therapist performs it under a certified therapy plan of care, the manual says to append the therapy modifier and bill the charges under the matching therapy revenue code.
A physician or non-physician practitioner billing it as a medical service also leaves the therapy modifier off. Chapter 5 adds that the units billed must not exceed the units in its allowed-units chart.
Which Services Bundle with 97597 on the Same Wound?
- Surgical debridement 11042 to 11047, and 97602 for the same wound (NCCI Chapter XI).
- Ultrasound wound therapy 97610, which is not separately reportable for the same wound with 97597 to 97606 or with debridement codes (NCCI Chapter XI).
- A skin graft or skin substitute (15050 to 15278): debridement before it is included in the graft procedure (NCCI Chapter III).
- Adjacent tissue transfer for the same lesion or injury, which includes debridement of the site (NCCI Chapter III).
A separate visit on the same day is reported with modifier 25 only when it is significant and separately identifiable. NCCI Chapter I treats a 000-global procedure as minor and does not require a different diagnosis for the visit.
How Does 97597 Appear on the Claim?
On the CMS-1500, Item 24D carries 97597 and Item 24G carries 1 unit. An added area goes on its own 97598 line with the added units in Item 24G, and Item 24E points each line to the wound diagnosis. Item 24B is 11 for an office and 22 or 19 for a hospital outpatient department.
A hospital reports the same HCPCS code on its outpatient claim, where Claims Processing Manual chapter 4 requires a HCPCS code on all outpatient hospital services unless the manual excepts them. Wound centers split facility and professional lines under hospital and facility billing.
What Fields Hold Wound Area for 97597?
Open wound debridement is one line, 97597 in Item 24D, with the units your payer expects for the first 20 square centimeters in Item 24G and a pointer in Item 24E to the wound diagnosis. Additional area uses the add-on code on its own line. The wound size and depth belong in the note.
On a hospital outpatient bill, the wound care revenue code goes in FL 42, 97597 in FL 44, the date in FL 45 and units in FL 46. RVU26D gives 97597 status A and a 000 global, and the MUE table allows 1 per date with indicator 3.
Example: A diabetic wound on the foot is cleaned of devitalized tissue on 10/15 over 15 sq cm. The line is 97597, pointer A (E11.621), 1 unit, $145.00. Billing 97597 and 11042 for the same wound that day draws a bundling denial under CO-97.
Why Is 97597 Denied?
| CARC | What X12 says | What usually causes it |
|---|---|---|
| 97 | The benefit is included in the payment for another service already adjudicated | 11042 to 11047 or a graft billed for the same wound |
| 236 | The procedure combination is not compatible with another on the same day under NCCI | 97602 or 97610 reported with it for one wound |
| 151 | The information submitted does not support this many or this frequency of services | More than 1 unit of 97597, or 97598 above 8 units |
| 4 | The procedure code is inconsistent with the modifier used | A therapy modifier on a claim billed as a medical service |
| 50 | Non-covered because the payer does not deem it medically necessary | Notes that show no devitalized tissue or no measured wound |

01What does CPT 97597 mean?
It reports selective debridement of an open wound, removing devitalized tissue without anesthesia, for a total wound surface of 20 sq cm or less. CMS shows status A and a 000 global. Each further 20 sq cm is reported with the add-on 97598.
02What is the difference between 97597 and 11042?
97597 is picked by wound area alone. 11042 is surgical debridement picked by the deepest tissue removed and then area. They are never billed for the same wound, per NCCI Chapter XI. 97597 has a multiple procedure indicator of 0 and 11042 has 2.
03How many units of 97597 can I bill?
One per date of service. The NCCI MUE for 97597 effective October 1, 2026 is 1 with indicator 3, in both the practitioner and outpatient hospital tables. Area beyond the first 20 sq cm goes on 97598, whose MUE is 8.
04Does 97597 need a therapy modifier?
Not as a medical service. CMS lists 97597 as a sometimes-therapy code. A hospital seeking OPPS payment as a non-therapy service leaves off GP, GO and GN and the 042x to 044x revenue codes. The modifier applies when a therapist bills it under a plan of care.
05Is debridement billed separately before a skin substitute?
No. NCCI Chapter III says debridement of a skin wound, including 97597 and 97598, before a graft or skin substitute is included in the graft procedure and is not reported separately. The same holds for debridement of a tissue transfer site.
