CPT 29580: Unna Boot Application and When It Is Bundled
CPT 29580 is the code for applying an Unna boot. It is a separately payable minor procedure with a 000-day global, but NCCI bundles it when the wrap is only the dressing after another procedure.
In short: 29580 has a 000-day global, includes its supplies and carries an MUE of 1 per date of service. A same-day visit needs modifier 25, and a wrap that only dresses a debridement is not reported separately under NCCI Chapter IV.
What Does CPT 29580 Report?
CPT 29580 reports applying an Unna boot, a compression wrap on the leg. The CMS fee schedule file labels it "Strapping unna boot". The multilayer compression system for the lower leg is a different code, 29581.
| Item | Value |
|---|---|
| Payment status | A (active code) |
| Work RVU | 0.54 |
| Practice expense RVU, non-facility and facility | 1.28 and 0.09 |
| Malpractice RVU | 0.07 |
| Total RVU, non-facility and facility | 1.89 and 0.70 |
| Global period | 000 |
| Multiple procedure and bilateral indicators | 2 and 1 |
| NCCI MUE, practitioner table, October 1, 2026 | 1 (indicator 2, policy edit; rationale: CMS policy) |
The gap between 1.89 and 0.70 sits in the practice expense column: 1.28 in the non-facility column and 0.09 in the facility column. RVUs are relative values, not dollars.
Can an Unna Boot Be Billed Separately?
Yes. 29580 is an active, separately payable code, but the supplies are not. Noridian's wound care article A53296 says all supply items related to the Unna boot are included in the payment for 29580, so the bandage and wrap do not get their own lines.
The wrap also cannot be billed when it is only the dressing. Chapter IV of the NCCI Policy Manual says casting, splinting and strapping codes shall not be reported for application of a dressing after a therapeutic procedure. It adds that payment for a surgical procedure includes dressings, supplies and local anesthesia.
When Is an Unna Boot Bundled into a Debridement?
When the wrap closes a debridement, it is the dressing, and the debridement payment includes it. A wound visit that debrides with CPT 11042 or 97597 and then wraps the leg has one service to report, not two.
NCCI does not list wounds that qualify. Its test is whether the wrap is a dressing after another procedure. Where the compression is the planned treatment and nothing else was performed, there is no procedure for the wrap to dress. Document which one it was.
Two related rules from the same manual. A fracture or dislocation code includes casts, splints and strapping, and strapping codes are not reported with them. Removal of a strapping the same practice applied is included, not billed again.
Can an E/M Visit Be Billed with 29580?
Only with modifier 25, and only for work beyond the decision to wrap. A 000-day global makes 29580 a minor surgical procedure under NCCI Chapter I, and the decision to perform a minor procedure is included in its payment. The manual says the visit and the procedure do not need different diagnoses.
Chapter IV says the same for strapping specifically: an E/M may be reported with a casting, splinting or strapping code only if it is significant and separately identifiable. The modifier 25 page covers the documentation.
A patient with a venous leg ulcer returns for a wrap change. The practitioner also reviews new medications, orders labs and refers for a vascular study.
How Does 29580 Sit on the Claim?
- CMS-1500 box 24D: 29580, one unit, with modifier 25 on the visit line when both are billed.
- Two legs: RVU26D bilateral indicator 1 applies a 150 percent adjustment when the code is reported with modifier 50, with RT and LT or with two units. The MUE is 1 per date of service, so confirm with your MAC which format it expects for two legs. See LT and RT.
- Multiple procedures: indicator 2 means the standard multiple procedure reductions apply when another procedure is reported the same day.
- Hospital outpatient clinics: Noridian's article says hospitals may only bill G0463 for a clinic visit on dates of service from 2014.
How Is 29580 Entered with a Dressing Change?
An Unna boot is one line, 29580 in Item 24D with 1 unit in Item 24G and a pointer in Item 24E to the leg condition, such as venous ulcer. Noridian's article A53296, cited above, includes the supplies in the 29580 payment, so no supply line is added.
RVU26D lists 29580 as status A with a 000 global and a MUE of 1 with indicator 2, a CMS policy edit. A hospital outpatient bill uses the wound care revenue code in FL 42, the code in FL 44 and 1 unit in FL 46.
Example: A patient with a venous leg ulcer is seen on 10/12 and a boot is applied. The line is 29580, pointer A (I87.2), 1 unit, $74.00. If the same visit includes debridement of the ulcer, the boot may be bundled, as the page explains.
Why Do 29580 Claims Deny?
| Code | What X12 says | What usually causes it on 29580 |
|---|---|---|
| CO-97 | The benefit is included in the payment for another service already adjudicated | The wrap went out with a debridement or procedure it dressed, or the visit has no modifier 25. |
| CO-50 | Non-covered because the payer does not deem it medically necessary | The payer's policy does not cover the condition treated. Noridian, for one, keeps lymphedema treatment with high compression bandage systems non-covered. |
| CO-16 | Claim lacks information or has a billing error | A required claim field is missing. X12 requires a remark code with this CARC. |
| CO-18 | Exact duplicate claim or service | The same wrap sent twice for one date of service. X12 limits it to group code OA. |

Common Questions
Straight answers from the team that does the work.
Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.
01Can an Unna boot be billed separately?
Yes, when the wrap is the service. 29580 is an active code with a 000-day global, and supplies are included in it. It is not separately reportable when the wrap is only the dressing after a debridement or another procedure.
02Does 29580 have a global period?
Yes, 000 days in the October 2026 CMS file. That means the related work on the day of the procedure is in the payment and an E/M the same day is generally not payable unless it is significant, separate and carries modifier 25.
03Can I bill the Unna boot supplies?
No. Noridian's article A53296 says all supply items related to the Unna boot are inclusive in the reimbursement for 29580. Do not add a supply line or a separate dressing code for the wrap or the bandage. The supply cost is part of the 29580 payment.
04Is there a limit on 29580 per day?
The CMS MUE for 29580 is 1 unit per date of service, with a policy-based date of service edit and the rationale "CMS Policy". RVU26D has a bilateral indicator of 1, so ask your MAC how to report two legs.
05Can I bill 29580 with a debridement the same day?
Only when the wrap is more than the dressing. NCCI Chapter IV bars strapping codes for a dressing after a therapeutic procedure. Document what the treatment was. For debridement pairs, NCCI Chapter XI also bars 97597 to 97602 with 11042 to 11047 on the same wound.
