San Antonio Medical Billing: TRICARE, BAMC and Civilian Payers
San Antonio billing differs from the rest of Texas in three ways. A military medical center takes civilian trauma patients, the county hospital district owns its own health plan, and TRICARE claims follow a one-year filing rule that Texas Medicaid and commercial plans do not share. This page covers those local facts. Texas-wide rules stay on the Texas page, and other states are listed on our states hub.
In short: San Antonio claims split into four tracks: TRICARE through a regional contractor with a one-year filing limit, Texas Medicaid and the University Health plan Community First, Medicare through Novitas, and commercial plans. Brooke Army Medical Center and University Hospital both run Level I trauma programs, so trauma claims depend on which door the patient came through.

Major Payers
Which Hospitals and Payers Shape Billing in San Antonio?
Seven kinds of owner sit in the San Antonio hospital list, and each one points to a different billing conversation. CMS Care Compare, in the Hospital General Information file last modified July 22, 2026, lists San Antonio hospitals as hospital district, state, church-affiliated nonprofit, proprietary, physician-owned and Veterans Health Administration. Brooke Army Medical Center runs on the military side and does not appear in that file.
The table below uses the ownership label CMS prints for each hospital. It tells you who owns the facility, not how that facility contracts with any payer, so treat the last column as a question to ask, not a finding.
| Hospital or system | CMS ownership label | What to ask first |
|---|---|---|
| University Hospital (listed as University Health System) | Government, hospital district or authority | Which claims bill under the district, and which under the UT Health San Antonio physician group? |
| UT Health San Antonio Multispecialty and Research Hospital | Government, state | Does the state-owned status change which payer contracts and enrollments apply? |
| Methodist Hospital and Methodist Hospital Stone Oak | Proprietary | Which commercial contracts cover each campus, and is each on its own CMS certification number? |
| Baptist Medical Center, Westover Hills Baptist Hospital, Baptist Neighborhood Hospital Thousand Oaks | Proprietary | Baptist Health System says its system has seven full-service hospitals and a children's hospital, so which entity is the billing provider on the claim? |
| CHRISTUS Santa Rosa Medical Center | Voluntary nonprofit, private | Is the claim going out under the right campus and NPI for the department that treated the patient? |
| Children's Hospital of San Antonio | Voluntary nonprofit, church | Pediatric Medicaid and CHIP claims go to the child's managed care plan, so check the plan before the filing clock. |
| Brooke Army Medical Center | Military, not in Care Compare | Is the patient a TRICARE beneficiary, a civilian trauma patient, or both across different encounters? |
These hospital names and labels come from the CMS file and each system's own site, not from our own contracts. For the payer side, start with the TRICARE billing guide, the Medicare guide and the Blue Cross Blue Shield of Texas guide, then compare each hospital's contracts against what the table asks.
How Does the Bexar County Hospital District Change the Picture?
University Health is the Bexar County Hospital District, and it owns a health plan. The system says it is officially the Bexar County Hospital District, a separate governmental entity and political subdivision of the State of Texas, governed by a seven-member Board of Managers appointed by the Bexar County Commissioners Court.
Two features matter on a claim. First, University Health states that the physicians of UT Health San Antonio serve as the medical staff for University Hospital and the Women's and Children's Hospital. A patient stay can therefore produce a facility claim and one or more physician claims from a different group, and a reconciliation that matches only one side will look short.
Second, University Health states that Community First Health Plans is the only local nonprofit health plan serving Bexar County and surrounding communities. Community First's own site says University Health established it in 1995 to cover Bexar County and seven surrounding counties. Its product list includes Medicaid STAR, STAR Kids and STAR+PLUS, CHIP and CHIP Perinate, Medicare Advantage, a Marketplace plan and a commercial HMO.
For a hospital that bills the district's plan, a patient can look like a county patient and a Medicaid managed care member at the same time. Run eligibility verification against the plan on the card, and keep the 95-day Texas Medicaid managed care limit on the Texas page in view.
What Does Brooke Army Medical Center's Trauma Program Mean for Claims?
Brooke Army Medical Center (BAMC) treats military and civilian trauma patients, and that mix is the billing issue. On its own news page dated December 17, 2025, BAMC says its Level I Trauma Center is the only Level I Trauma Center in the Department of War and that it cares for more than 6,000 military and civilian trauma patients each year, including 750 burn patients, from 22 counties in Southwest Texas.
University Health also describes University Hospital as the region's only Level I trauma center for both adults and children. We quote both claims as each organization words them. A San Antonio trauma patient may therefore be admitted to a military facility or to the county hospital, and the payer that owes the bill differs: the Defense Health Agency system for a TRICARE beneficiary, or an insurer, Medicaid or the district for a civilian.
BAMC does not publish how it bills civilian trauma. If you are a civilian hospital that transfers a patient to BAMC or receives one back, ask the receiving facility's patient business office which entity bills the encounter and for which dates of service before you code your own part of the stay.
How Does TRICARE Filing Work for a San Antonio Facility?
A San Antonio facility files TRICARE claims with the regional contractor within one year of service. The TRICARE claims page says that in the U.S. and U.S. territories claims must be filed within one year of service, and that in other overseas areas the limit is three years. The Texas Medicaid limit is 95 days, so a mixed payer mix needs two clocks.
TRICARE has an East and a West region, and the contractors differ. The TRICARE claims page lists Humana Military for the East Region and TriWest Healthcare Alliance for the West Region, with a ZIP code lookup to tell you which region a patient is in. Check the beneficiary's ZIP code on that lookup before choosing a claims address.
Retirees add a third payer. TRICARE For Life works alongside Medicare: Medicare pays first and sends the claim to the TRICARE For Life claims processor, and TRICARE then pays the provider directly for TRICARE-covered services. In Texas the Medicare side goes to Novitas, as the Texas page explains. The full TRICARE deadline detail, appeals and denial handling are in the TRICARE billing guide.
Which Filing Clocks Apply to a San Antonio Payer Mix?
Four clocks apply. The table lists each one with where it comes from. Commercial plans are left to the provider contract, so those need a contract check rather than a rule lookup.
| Payer | Filing limit | Where it comes from |
|---|---|---|
| TRICARE (regional contractor) | One year from service in the U.S. | TRICARE claims page |
| Medicare, Novitas Jurisdiction H | One calendar year after the date of service | 42 CFR 424.44 |
| Texas Medicaid and Community First STAR plans | 95 days from the date of service | Texas Medicaid Provider Procedures Manual |
| Commercial plans | Per the provider contract | Contract |
The Texas page has the Medicaid and Medicare detail and the prompt pay rules, so they are not repeated here. Texas Medicaid itself is covered in the Medicaid guide. For other metros, see Houston, Dallas and Austin.
Which Dispute Pathway Applies to Out-of-Network Claims in San Antonio?
The pathway depends on the plan type, not the city. Texas mediation and arbitration cover fully insured plans; federal IDR covers plans the Texas law does not reach, including self-funded employer plans. The Texas page lays out the Chapter 1467 mediation and arbitration rules, and the state surprise billing laws page shows how the state and federal systems fit together.
TRICARE is a different case. TRICARE publishes no No Surprises Act dispute route for its claims. Treat a TRICARE underpayment as a TRICARE appeal first.
How We Handle Billing for San Antonio Groups and Facilities
We serve San Antonio facilities and groups remotely. Our only US office is the registered office in Richmond, KY; our team works from Vancouver, BC, Dubai, UAE and Islamabad, PAK, and we have no office or staff in San Antonio.
For hospitals we prepare institutional UB-04 claims with checks on bill type, revenue codes, patient status, condition and occurrence codes, authorization requirements and payer-specific rules, then submit, reconcile payments and manage denials for inpatient, outpatient and emergency care. Claims go out within 24 hours of charge entry and denials are worked within 48 hours. See hospital and facility billing.

Common Questions
Straight answers from the team that does the work.
Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.
01How does a civilian hospital in San Antonio bill TRICARE for care of an active duty family member?
Verify the TRICARE plan and region from the beneficiary's ZIP code, then send the claim to the regional contractor, Humana Military for East or TriWest for West. The TRICARE claims page sets a one-year filing limit in the U.S. The payer guide covers network status and denials.
02What is the TRICARE claims filing deadline for a San Antonio facility?
One year from the date of service for care in the U.S., per the TRICARE claims page. That is longer than the 95 days Texas Medicaid allows, so keep payer-specific clocks in your work queue. Some exceptions exist for inpatient stays and other insurance, which the TRICARE payer guide covers.
03How do civilian trauma transfers from Bexar County to BAMC affect billing?
BAMC says it treats more than 6,000 military and civilian trauma patients each year. BAMC does not publish how it bills civilian trauma, so confirm with BAMC's patient business office which entity bills each date of service. Do not assume the receiving facility's claim covers your transport or stabilizing care.
04Who is the TRICARE contractor for Texas claims and where do providers submit?
TRICARE lists Humana Military for the East Region and TriWest Healthcare Alliance for the West Region. Use the ZIP code lookup on the TRICARE claims page for each patient and send claims to the claims address listed for that region.
05How does Medicare MAC JH (Novitas) interact with TRICARE For Life claims for San Antonio retirees?
Medicare pays first. TRICARE says Medicare sends the claim to the TRICARE For Life claims processor, and TRICARE then pays the provider directly for TRICARE-covered services. In Texas the Medicare claim goes to Novitas Solutions in Jurisdiction H, so a clean Medicare claim is the first step.
06Which commercial payers lead in San Antonio besides TRICARE?
No official market share ranking exists for San Antonio, so payers are not ranked. The payers in play include the local plan Community First Health Plans, Medicare through Novitas, Texas Medicaid, and commercial carriers that vary by contract. Check each hospital's contract list.
07What is Community First Health Plans, and why does it matter for San Antonio billing?
Community First Health Plans is the nonprofit plan University Health established in 1995 for Bexar County and seven surrounding counties. Its site lists Medicaid STAR, STAR Kids, STAR+PLUS, CHIP, Medicare Advantage, Marketplace and a commercial HMO. Check which product a patient holds before choosing the filing rule.
08Who runs University Hospital, and does that change how it bills?
University Health is officially the Bexar County Hospital District, a governmental entity governed by a Board of Managers that Bexar County Commissioners Court appoints. Physicians of UT Health San Antonio serve as the medical staff, so expect separate facility and physician claims for one admission.
09Does the Texas Medicaid 95-day limit apply to Community First claims?
Yes for STAR and the other Medicaid managed care products. The Texas Medicaid Managed Care Handbook says claims must go to the appropriate entity, TMHP, the MCO or the DMO, within 95 days of the date of service. A claim sent to the wrong plan has its own refiling rule on the Texas page.
10Does the Texas out-of-network dispute process change in San Antonio?
No. The pathway follows the plan type, the service and the billing entity, not the city. Texas mediation or arbitration covers fully insured plans, and federal IDR covers plans outside the Texas law. The Texas page and the state surprise billing laws page explain how to choose.
