Buffalo Medical Billing for Western New York Hospitals and ER Groups
Buffalo billing runs on a small set of regional payers. Six Medicaid managed care plans are listed for Erie County, and Catholic Health's contracts with Highmark BCBSWNY and Independent Health were last announced in December 2022.
In short: buffalo claims depend on which regional contract applied on the date of service. Catholic Health announced a long-term Highmark BCBSWNY agreement on December 28, 2022 and a two-year Independent Health agreement through 2024, so confirm what is in force now. State IDR fees are due up front and the decision clock is 45 business days from complete information.

Which Buffalo Health Systems Have Announced Payer Contracts, and What Do the Notices Say?
Two Catholic Health contracts are documented in the system's own press releases, both from December 2022. Neither release gives rates, and one has a stated end date that has passed. Everything else about who is in network today has to come from the payer portal and your executed contract.
| Organization | Payer | What the release says | What it leaves out |
|---|---|---|---|
| Catholic Health and Catholic Medical Partners | Highmark BCBSWNY | Dated December 28, 2022. A new long-term contract agreement keeps Highmark BCBSWNY members in network at Catholic Health hospitals. Details to be finalized in the coming weeks. | Rates, term length, product scope |
| Catholic Health and Catholic Medical Partners | Independent Health | Dated December 16, 2022. A two-year agreement keeps Independent Health members in network, including the 900 physicians affiliated with Catholic Medical Partners, through 2024. | Whether it was renewed after 2024 |
The December 2022 releases name Kenmore Mercy Hospital, Mercy Hospital of Buffalo, Mount St. Mary's Hospital, and Sisters of Charity Hospital with its St. Joseph Campus. Catholic Health's own site now says it has six hospitals in its network, so a Catholic Health claim may carry a facility that the 2022 notices never named.
No later official notice for either Catholic Health contract is cited here. Ask the plan for the current agreement in writing, and compare it with the fee schedule loaded in your practice system.
What Does a Long-Term Agreement Announcement Mean for Claims Already in Process?
A press release is not a fee schedule. Catholic Health's Highmark release said the details would be finalized in the coming weeks, so the terms in force for dates of service right after the announcement are worth checking against the signed contract.
A billing office can test that. Pull the paid claims from the first months after a release, compare allowed amounts with the contract exhibit, and file an adjustment or underpayment appeal on any gap that the contract supports. The payer contracting work starts there, and the Highmark guide covers that plan's own appeal steps.
The same check applies to Independent Health. The two-year term in its release ended with 2024. Treat any 2025 or 2026 Independent Health claim at Catholic Health as needing confirmation before you write off a balance.
Which Medicaid Managed Care Plans Cover Erie County?
Six Medicaid managed care plans are listed for Erie County in the Department of Health directory revised September 2026. Each keeps its own provider manual and appeal windows. Erie County claims for these members go to the plan, not to eMedNY fee-for-service.
| Plan as listed | Programs listed for Erie County |
|---|---|
| Upstate HMO and Univera Healthcare HMO (Excellus Health Plan) | Medicaid, Child Health Plus, HARP, commercial |
| Highmark Blue Cross Blue Shield of Western New York | Medicaid, Child Health Plus, HARP, commercial |
| Independent Health Association | Medicaid, Child Health Plus, HARP |
| Molina Healthcare or Affinity by Molina Healthcare | Medicaid, Child Health Plus, HARP |
| Fidelis Care | Medicaid, Child Health Plus, HARP |
| UnitedHealthcare Community Plan | Medicaid, Child Health Plus, HARP |
State IDR no longer takes Medicaid managed care disputes, per DFS Insurance Circular Letter No. 5 (2026), so a denial from any of these goes through the plan's appeal. The cutoff date and the New York City plan list are on the New York City page. Our Medicaid billing guide covers the national rules.
How Do New York's IDR Fees, Timing and Refunds Work for a Buffalo ER Bill?
For disputes submitted on or after August 26, 2026, both the out-of-network provider and the health plan must pay the IDR entity before it reviews the dispute. The circular letter says payment is due on submission, or, for the responding party, when it answers the IDR entity's request for eligibility information and supporting documents.
DFS Insurance Circular Letter No. 5 (2026) lengthened the decision period. IDR entities used to have 30 business days from receipt of the application. They now have 45 business days from receipt of all the information the entity determines it needs, so the clock starts after your file is complete.
| Situation | What DFS says happens to the fee |
|---|---|
| Prevailing party after a decision | Full fee refunded within 30 days of the determination |
| Dispute rejected as ineligible | Refund to the applicant within 30 days, minus an application processing fee under 23 NYCRR 400.10 |
| Settlement or split decision | Pro-rated refund under 23 NYCRR 400.10 |
| Handling of the money | The IDR entity must hold payments in a separate account, not commingled with other funds |
Two eligibility traps matter to a Buffalo hospital. First, the IDR entity may not award any fee if the plan shows it had a contract in effect on the date of service with the provider, or an entity the provider owns or operates, for the same service at the same location. Second, no fee follows if the plan issued a prior authorization notice that named the service as out of network.
The plan has to provide that documentation at the start of the dispute and cannot raise it after the decision. For that reason, pull your own contract history and any prior authorization notices before you file, especially for a system with several facilities.
For Empire Plan and Student Employee Health Plan disputes, the emergency regulation effective August 26, 2026 limits the "same geographical area" for the benchmark to the geozip where the provider rendered the services, unless a hospital-employed physician under Financial Services Law 604(b)(5) is involved. An IDR decision is binding and carries no appeal to DFS.
How Does State IDR Apply to a Buffalo ER Bill for a Fully Insured Highmark Member?
A fully insured Highmark BCBSWNY plan is coverage subject to New York law, so an out-of-network emergency bill from a Buffalo hospital goes through the DFS IDR process, not federal IDR. DFS says the ID card is the test: fully insured means New York law, and self-funded or silent means the federal No Surprises Act.
The plan type, not the carrier name, decides the pathway. Check the card, or the eligibility response, before you decide where the dispute goes. Our state surprise billing laws guide explains how the two systems divide the work.
Catholic Health's Highmark release also notes that Highmark BCBSWNY members reach care nationally through the Blue Cross Blue Shield BlueCard program. Out-of-area Blue accounts are covered in our Blue Cross Blue Shield guide.
Which Buffalo Hospitals Hold State Trauma Designations?
The Department of Health trauma list shows Erie County Medical Center as a Level I adult trauma center and Golisano Children's Hospital on Ellicott Street as a Level I pediatric trauma center, both in Buffalo. No other Erie County hospital appears with a trauma level on that page.
A trauma designation shows where serious injury cases are treated. It does not tell you the payer. The no-fault billing rules for motor vehicle injuries are on the New York City page with the DFS source.
What Is the Highmark Timely Filing Rule for New York Claims?
Under Insurance Law 3224-a(g), providers have 120 days to submit claims under a health insurance policy or contract, according to DFS Insurance Circular Letter No. 2 (2024). A Highmark contract can set its own limit, so confirm the plan's current filing limit in its provider manual and your contract.
Use the Highmark billing guide for the manual's limits and corrected claim windows, and the New York state page for Medicare, Medicaid and prompt pay. For Buffalo Medicare claims, the contractor is the same National Government Services Jurisdiction K listed there.

01Which Western New York payers contract with Kaleida and Catholic Health today?
Catholic Health's own releases announced a long-term Highmark BCBSWNY agreement on December 28, 2022 and a two-year Independent Health agreement through 2024. Check network status for the date of service on the payer portal.
02How does a payer-hospital contract dispute affect claims during a cooling-off period?
Claims follow the contract in force on the date of service. If a notice of termination or extension arrives, save it with the account and record the network status for each date of service.
03How does New York's state IDR apply to a Buffalo ER bill for a fully insured Highmark member?
It applies through the DFS process. A fully insured plan is subject to New York law, so the hospital files in the DFS portal rather than federal IDR. Self-funded Highmark plans follow the federal No Surprises Act. DFS says to check the ID card for the words fully insured.
04What is the Highmark timely filing rule for New York claims?
Insurance Law 3224-a(g) gives providers 120 days to submit claims under a policy or contract, per DFS Circular Letter No. 2 (2024). Highmark's own manual may set a different number. Check the provider manual and your contract, and see our Highmark guide for corrected claim windows.
05Which Medicaid managed care plans operate in Erie County?
The Department of Health directory, revised September 2026, lists Univera Healthcare (Excellus), Highmark BCBSWNY, Independent Health, Molina or Affinity, Fidelis Care and UnitedHealthcare Community Plan with Medicaid in Erie County. Each has its own appeal windows in its provider manual, and disputes with them no longer go to state IDR.
06Is Erie County Medical Center a Level I trauma center?
Yes for adults. The Department of Health trauma list shows Erie County Medical Center, 462 Grider Street, as a Level I adult trauma center, and Golisano Children's Hospital as a Level I pediatric trauma center. Confirm with the hospital before citing the designation.
07How long does a New York IDR decision take after the August 2026 changes?
IDR entities have 45 business days from receipt of all the information they determine they need, up from 30 business days from receipt of the application, per DFS Circular Letter No. 5 (2026). The clock starts only after the file is complete, so missing documents delay the decision.
08When is the IDR fee due, and when is it refunded?
For disputes submitted on or after August 26, 2026, each side pays the IDR entity before review: on submission for the applicant, or when answering the entity's request for eligibility information for the responding party. The prevailing party gets a full refund within 30 days of the determination.
09What does the Medicare contractor change for a Buffalo hospital?
Medicare Part A and B claims from Buffalo go to National Government Services in Jurisdiction K, the same contractor as the rest of New York, according to the CMS list of MACs by state. Our New York state page covers the Medicare filing limit and links to the Medicare guide for appeals.
