Practice Fusion Medical Billing and RCM Support

An outside billing team can work inside Practice Fusion through named user accounts you create under a signed BAA. Because the vendor also sells its own billing service, the first step is to settle who owns each claim today.

In short: practice Fusion is a cloud EHR for solo and small independent practices, and its vendor offers a billing service of its own. We work in it from named, two-step accounts under a BAA, and we agree with you at setup who owns which queue.

Already on this system? We work in it. No EHR, or tired of paying for one? Our billing clients get EHR or PM software free.

Who Owns Practice Fusion, and Which Practices Is It Built For?

Practice Fusion is a cloud-based electronic health record (EHR) that its own site describes as built for solo providers and small independent clinics. The site also states that Practice Fusion is a Veradigm network solution, so Veradigm is the parent company.

The vendor says the EHR needs no dedicated IT team and opens on any device. It lists e-prescribing, lab and imaging ordering, and a free patient portal called FollowMyHealth.

That focus matters for billing. See all EHR systems we work in.

Does Practice Fusion Already Include Billing Services?

Yes, Practice Fusion sells a billing service of its own next to the EHR. Its billing page lists payer enrollment, claims scrubbing, claim submission, denial management, payment posting, insurance follow-up, refund and credit balance monitoring, and financial reporting, with an integrated clearinghouse.

So the first question for any practice on this system is who owns the claims today. Three setups are common, and each changes what we touch:

SetupWho works the claimsWhat we would do
Your staff bill from the EHRYour own teamTake over charge review, submission, posting and denials in named accounts you create
You use the vendor billing serviceThe vendor's teamNothing changes until you decide to end that service and agree the handover date with the vendor
Mixed: vendor submits, you follow upSplitAgree in writing who owns each queue, so a denial is never worked twice or not at all

Two teams must never work the same claim at once. Our medical billing services page describes the full scope.

How Do Claims Leave Practice Fusion and Come Back as Payments?

Under the federal HIPAA transaction rules, a professional claim goes out as an X12 837P, and the payer answers with an X12 835 remittance advice. 45 CFR 162.1102 names the 005010X222 version for professional claims, and 45 CFR 162.1602 names 005010X221 for the 835.

Practice Fusion's billing page mentions an integrated clearinghouse. Practice Fusion's public pages do not say which clearinghouses a self-billing practice can use, so we confirm that with you at setup.

An ERA is the electronic file that explains each payment and adjustment. We ask whether your clearinghouse delivers the 835 file and how it reaches the system, because that decides if payments post line by line or as a lump.

A clearinghouse also returns rejections before the payer ever sees a claim. A rejection is a data fix, while a denial is a payer decision, so we sort the two into different queues (rejection vs denial).

What Access Does a Billing Partner Need in Practice Fusion?

A billing partner needs named user accounts that you create, limited to billing roles, after a BAA is signed. We do not use shared logins.

1
Sign the BAA First
The Business Associate Agreement is in place before anyone on our team sees patient data.
2
You Create the Accounts
Your account owner adds one named user for each person on our team, with only the permissions the billing work needs. Practice Fusion's own pricing page mentions a set number of signing staff licenses, so check how many seats your plan includes before inviting people.
3
Two-Step Login
Every account uses two-step login. Nobody borrows another person's credentials.
4
Clearinghouse and Payer Portals
You add us to the clearinghouse and the payer portals under the same named-user rule, and you can disable any account at any time.

Our security page covers HIPAA training, NDAs, encrypted backups and our compliance officer.

What Do We Do Inside a Practice Fusion Account?

We review charges, code, edit and submit claims, post payments, and work denials and open balances. Scope is agreed with you at setup, and so is how we report to you.

  • Charge review against the visit note, so a missing diagnosis, provider or place of service is caught before submission.
  • Claim edits and submission through your clearinghouse, with claims sent within 24 hours of charge entry.
  • Posting of ERA 835 files and manual EOBs, including adjustments and patient responsibility.
  • Denial work queues, with denials worked within 48 hours.
  • Insurance A/R follow-up by payer and age, then A/R recovery on older balances.
  • Patient statements for what insurance leaves behind.

Where a denial shows a pattern, we trace it back to the charge step that caused it.

Which Practice Fusion Reports Help with Denial and A/R Tracking?

Practice Fusion says its reporting comes in three types: quality, clinical and practice management. The practice management type is the one a billing review draws on. Practice Fusion's public pages do not list report names, so we describe them by purpose.

What we need to seeWhy
Charge lag, from visit date to charge enteredLate charges delay every payment behind them
Claim status by payerShows what is unsent, rejected, pending or paid
A/R aging by payerShows which payer is holding money past 60 and 90 days
Denial trends by reason and by providerPoints to the step that needs fixing
Payment against the expected amountFlags underpayments to appeal

If a report is missing from your plan, we build the same view from clearinghouse and 835 data. See days in A/R.

What Billing Mistakes Are Common on Small-Practice Systems Like This One?

We attribute no defect to Practice Fusion; we trace a problem to the setting or payer rule behind it. These are checks we run on any ambulatory system, and each one has an official basis.

CheckWhy it matters
Modifiers on the chargeThe CMS-1500 can carry up to four modifiers per line in item 24D, according to the CMS Claims Processing Manual, Chapter 26. We confirm the right modifier survives from the note to the claim.
Service locationItem 32 of the CMS-1500 holds the facility name and address when care is not given at the home or the physician's office, per the same chapter.
Claim form typePractice Fusion's pages describe physician billing. If you also bill facility fees, ask whether the system produces a UB-04, because its public pages do not say; we confirm it at setup.
Demographics and eligibilityA wrong ID or date of birth sends the claim back. We verify coverage before the visit, with automated eligibility checks.
Payer enrollmentEnrollment with each payer is a separate task from billing, and the vendor lists it among its services.

How We Handle Billing in Practice Fusion

We start by finding out who owns each claim today, then take the queues you give us from named accounts under a signed BAA. Claims run through your clearinghouse.

Our own reporting covers charge lag, claim status, denial trends and A/R by payer, agreed at setup. If you are weighing the vendor billing service, our guide to choosing a billing partner lists the questions to ask.

Neighbouring systems have their own pages: DrChrono, Tebra and eClinicalWorks. To talk through your setup, book a call.

FAQ

Common Questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside Practice Fusion?

Yes, if you give it access. Your account owner creates a named user for each person, limited to billing roles, after a BAA is signed. We use those accounts and never a shared login. You keep control and can disable any account whenever you choose.

02What access does a billing partner need in Practice Fusion?

Named user accounts with billing roles, the reports you choose to share, and portal access to your clearinghouse and payers. Each account uses two-step login. Check how many staff seats your plan includes, since the vendor's pricing page mentions a set number of signing licenses.

03Which Practice Fusion reports matter for denial and A/R tracking?

The vendor lists quality, clinical and practice management reporting. For billing we want charge lag, claim status, A/R aging by payer, denial trends and payment against expected amounts. Report names depend on your plan, so we confirm what yours offers at setup.

04How do claims move from Practice Fusion to the clearinghouse and payer?

Professional claims travel as X12 837P files, and payers answer with the 835. The vendor's billing page mentions an integrated clearinghouse. Whether a practice billing on its own can use another one is something we confirm with you before setup, not something we assume.

05How does Practice Fusion post ERA and 835 remittances?

Its public pages do not describe posting steps, so we confirm at setup how your account posts. The standard 835 under 45 CFR 162.1602 carries payments and adjustments, so we ask how your clearinghouse delivers the file and then check each posted payment against the payer's remittance.

06What billing mistakes are common in Practice Fusion (charge capture, modifiers, demographics)?

We attribute no defects to the vendor. We check that modifiers carry onto the CMS-1500, that the service location is right, that demographics match the payer file, and that the provider is enrolled. Those checks apply to any ambulatory system.

07Is Practice Fusion data safe with an offsite billing team under a BAA?

We sign a BAA before access, use named accounts with two-step login, train all staff on HIPAA, and hold NDAs. Backups are encrypted and a compliance officer oversees the work. You can disable any account yourself at any time, and you keep full control of who has access.

08Does Practice Fusion handle institutional UB-04 claims or only professional claims?

The vendor's pages describe physician billing for independent practices. Its pages do not say it produces UB-04 claims, so we confirm at setup what your build sends. If you bill facility fees, ask the vendor in writing, and we will build our claim checks around the written answer you receive.

09How do we track out-of-network claims and appeals in Practice Fusion?

We keep a separate queue for out-of-network claims, with the denial reason, the appeal deadline from the payer's letter and the date each step was sent. Cases that qualify for the federal dispute process run in a separate workflow of their own. Out-of-network cases that qualify for the federal process run in a separate workflow.

10What does outsourcing billing cost for a Practice Fusion practice?

Cost depends on collections and scope, not on which EHR you use. Our pricing page states the rate. If you also pay the vendor for its billing service, compare both fee models side by side, line by line, before you move anything.

11Is there a billing company that works with Practice Fusion?

Yes. Nucleus Align works inside Practice Fusion with access you grant. We first confirm who owns claims in your setup, then handle charge review, claim submission, remittance posting and denial work as agreed. A BAA is signed before access. Scope and reporting are agreed with you at setup.

12Can an outsourced billing team use our Practice Fusion logins?

No shared logins. Your Practice Fusion administrator creates a named user for each Nucleus Align team member with the roles you choose. Our staff use two-step login under HIPAA training and NDAs, and you can change or end that access whenever you want.
