Practice EHR medical billing services: how we work in your account

An outside team can bill inside Practice EHR when your administrator creates named, two-step accounts and a BAA is signed first. We work the billing screens, send claims through your clearinghouse path and report from your own data.

In short: we work in your Practice EHR account with our own named logins, using the screen names in PracticeEHR's help center and billing rules from CMS and X12. We claim no partnership with PracticeEHR.

What is Practice EHR, and who runs billing in it?

Practice EHR is a cloud EHR, practice management (PM) and revenue cycle platform from PracticeEHR. Its product menu lists Electronic Health Records, Practice Management, Revenue Cycle Management, Patient Portal, TeleVisit, AI Agents, a mobile app, a check-in kiosk and a CRM.

Its specialties page lists 25+ specialties, including Behavioral Health, Psychiatry, Chiropractic, Pain Management, Podiatry and Urgent Care. PracticeEHR's own FAQ says the product is designed for small and mid-sized practices.

Similar practice sizes: Practice Fusion, RXNT and DrChrono serve similar practice sizes. See the EHR systems hub.

Which Practice EHR billing screens does an outside biller work in?

The Practice EHR help center has a Billing section with nine walkthroughs. We use them as the map of the account.

Screen names and descriptions from helpcenter.practiceehr.com/billing.

How do we get access to a Practice EHR account?

Your practice grants access, and nothing opens until a Business Associate Agreement (BAA) is signed. PracticeEHR's features page lists Role-Based Access and Audit Logs, so your administrator can limit each of our users to billing work and see what each one did.

1
BAA first
The agreement is signed before any login exists. Our staff carry HIPAA training and NDAs.
2
Your administrator creates one named user per person
Each member of our team gets a separate account with a billing-only role. There are no shared logins.
3
Two-step login on every account
The same rule covers the clearinghouse and payer portals.
4
Scope is agreed at setup
Worklists, reports and reporting are agreed with you. See security.

How do claims leave Practice EHR and reach the payer?

Practice EHR describes an integrated clearinghouse on its claims management page: you submit claims, track status and resolve rejections without leaving the product. Its AI Claim Editor page says each claim gets a rejection risk score before a person approves it.

Professional claims travel as the X12 837 version 005010X222 and institutional claims as 005010X223, the standards named in 45 CFR 162.1102. The help center's claim editing walkthrough refers to the HCFA boxes, which is the CMS-1500 professional form layout. A facility that needs an institutional UB-04 claim should have that confirmed for its own build before we bill it.

We keep to your clearinghouse path and send claims within 24 hours of charge entry. A claim kicked back at the front end is a rejection, not a denial, so we fix the data and resend it.

How are ERA and 835 remittances posted?

PracticeEHR's claims pages say remittances are posted automatically through the clearinghouse connection. The payment screen also supports manual entry of the paid amount, the allowed amount, write-offs, secondary details and a denial. The standard for the electronic payment file is the X12 835 version 005010X221, named in 45 CFR 162.1602.

Either route can go wrong, so we compare each posted ERA to the payer's adjustment codes and your fee schedule, and tie payment batches to bank deposits.

The vendor lists integrated ERA handling for its chiropractic templates. See chiropractic billing.

What billing mistakes do we check for in Practice EHR?

Practice EHR's AI Claim Editor flags coding, modifier and payer-rule issues, and its page says your team reviews every suggestion before submission. We treat the flags as a starting list and add our own checks.

  • Modifier order and count. The CMS Claims Processing Manual, chapter 26, says the CMS-1500 can capture up to four modifiers in item 24D, so a fifth cannot ride on the form.
  • Charge capture. Visits with no charge, and encounters not billed, are matched to the schedule.
  • Demographics and plan order. Patients with more than one plan are checked so the right payer is billed first.
  • Eligibility on the date of service. PracticeEHR says it runs an auto-eligibility check 24 hours before a visit, and we re-check anything that changes after.
  • Diagnosis pointers against the procedure, and place of service on each line.

How do we track denials, appeals and out-of-network claims?

PracticeEHR describes a built-in appeal process where each appeal has an owner and a status, and a worklist sorted by status, priority, amount at stake and owner. We work denials in that list and keep our own tracking beside it, within 48 hours of the denial reaching us.

For out-of-network claims, we track the open negotiation and federal dispute dates outside the EHR. See out-of-network billing and IDR and denial management.

Which Practice EHR reports do we use for A/R and denials?

The help center's analytics walkthrough names a KPI scorecard and tabs for AR, aging, denials, follow-up and payments. We use them by purpose.

PurposeWhat we look for
Charge lagVisits with no charge, and days from visit to charge
Claim statusClaims held in the editor, rejected, or unpaid past the payer's usual window
A/R aging by payerBalances by age bucket for each payer, and which payers are stalling
Denial trendsTop denial reasons by payer and by provider
Payment varianceAllowed amounts that differ from the fee schedule or contract

What if Practice EHR is also doing our billing?

PracticeEHR also sells a Revenue Cycle Management service. Its RCM page says the EHR and practice management platform is included at no extra cost when PracticeEHR does the billing, and it lists credentialing, claims, posting and denials.

If you are comparing options, settle who owns the worklists and payer credentials first. The choice is yours. For our scope, see medical billing and revenue cycle management.

FAQ

Common questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Can an outside billing company work inside Practice EHR?

Yes, when your practice grants access. Your administrator creates a named account for each person on our team, with a billing role, after a BAA is signed. We use two-step login and no shared logins, so the audit log shows who changed what.

02What access does a billing partner need in Practice EHR?

Billing and reporting access to the screens that hold charges, claims, payments, A/R and analytics, and no clinical charting rights. PracticeEHR lists role-based access and audit logs, so your administrator can set that scope. We agree it with you at setup and use nothing beyond it.

03Which Practice EHR reports matter for denial and A/R tracking?

The AR, aging, denials, follow-up and payments tabs in the billing analytics, plus the dashboard's average days in AR. We read them by purpose: charge lag, claim status, A/R aging by payer, denial trends and payment variance. Exact names depend on your account.

04How do claims move from Practice EHR to the clearinghouse and payer?

PracticeEHR describes an integrated clearinghouse. After the provider signs the encounter, the claim is generated, scanned by the AI Claim Editor and approved by a person, then submitted. Claims ready, queued and paper sit on separate tabs, and auto submission can send them on a schedule.

05How does Practice EHR post ERA and 835 remittances?

PracticeEHR's claims pages say remittances post automatically, and its payment screen allows manual entry of paid and allowed amounts, write-offs and denials. The 835 standard is version 005010X221 under 45 CFR 162.1602. We compare postings to payer adjustment codes and deposits.

06What billing mistakes are common in Practice EHR (charge capture, modifiers, demographics)?

We check for visits with no charge, modifier count and order, wrong plan order, and diagnosis pointers that do not match the procedure. The CMS-1500 holds up to four modifiers in item 24D, so we count them on every line before submission.

07Is Practice EHR data safe with an offsite billing team under a BAA?

We sign a BAA with every client before access, our staff carry HIPAA training and NDAs, and every account uses two-step login. Backups are encrypted and automatic, and a dedicated compliance officer oversees the program. Your administrator can review our activity in the account. See security.

08Does Practice EHR handle institutional UB-04 claims or only professional claims?

Its help center claim editing walkthrough refers to the HCFA boxes, which is the CMS-1500 professional layout. Institutional claims use the 837I version 005010X223 and the UB-04. If a facility needs those, we confirm what its build sends before we bill it.

09How do we track out-of-network claims and appeals in Practice EHR?

Appeals run through the built-in appeal worklist, which shows owner, status, priority and amount at stake. Out-of-network claims also carry open negotiation and dispute deadlines, which we track beside the EHR so none is missed. See out-of-network billing and IDR.

10What does outsourcing billing cost for a Practice EHR practice or facility?

Our fee is a percentage of collections, and the figure is on the pricing page only. PracticeEHR sets its own plan prices separately and quotes them after a demo, so ask each side what is included before you compare them.

11Is there a billing company that works in Practice EHR?

Yes. Nucleus Align works inside Practice EHR with named, role-limited accounts you grant. We review charges and claim editor flags, send claims within 24 hours of charge entry, post remittances and work denials within 48 hours, under a signed BAA. Scope and reporting are agreed with you at setup.

12Can an outsourced billing team use our Practice EHR logins?

No shared logins. Your administrator creates a named user for each Nucleus Align team member with billing roles, and our staff sign in with two-step login. Every action in Practice EHR traces to a person, and you can remove access at any time.
