MDLand medical billing and RCM support
An outside billing company can work your MDLand iClinic account on named logins your practice issues, after a BAA is signed. MDLand sells iClinic, a cloud EHR with built-in billing and practice management, in Professional and Enterprise versions.
In short: you keep MDLand and we work the billing queues you hand over, from named users with two-step login. Nucleus Align has no partnership with MDLand.
What is MDLand and who uses it?
MDLand is a digital health company, MDLand International Corp., that sells a cloud EHR called iClinic. Its site says it was founded in 1999 and serves physician practices, physician organizations (MSO, IPA, ACO and PPS), payers and government agencies.
MDLand's pages describe billing and practice management as a built-in part of iClinic, so charge entry and claim work happen in the same account.
| Name on MDLand's site | What MDLand says it is | What it means for billing |
|---|---|---|
| iClinic Professional | The iClinic version for solo, small and medium size practices | Usually one office team handling charges and claims |
| iClinic Enterprise | The version tailored to large physician groups, MSO, IPA and ACO organizations | More users, more roles and more locations to map at setup |
| Electronic Billing and Practice Management | Claim processing, payment posting, scheduling, registration, document management and financial tools | Where claims, rejections and remittances are worked |
| Care Management Suite | CCM, RPM, TCM, CoCM and PCM modules with consent, time tracking and built-in billing | Time and consent records that support care management claims |
| iGI | A procedure report writer for gastroenterology | Procedure notes that feed professional charges |
| Clinical connectivity | Interfaces to labs, imaging centers, hospitals and health information exchanges | Orders and results sit beside the charges they support |
Which of these your account uses differs by practice, so we read your setup first.
Can an outside billing company work inside MDLand?
Yes, when the practice issues the logins. MDLand's pages do not mention outside billers, so permission comes from your own administrator and contract.
Nucleus Align has no partnership with MDLand. We work the billing queues you hand over, on named logins, under a signed business associate agreement.
What access does a billing partner need in MDLand?
Each person on our team gets an individual account that the practice creates, limited to billing work, once the BAA is in place. Logins are never shared.
Data protection terms are on our security page.
How do claims move from MDLand to the clearinghouse and payer?
MDLand says the iClinic claim processing engine submits claims electronically to almost any payer and plan, returns claim status updates and posts payments electronically.
The pages do not say which clearinghouse carries the claims, so we ask your administrator which connection the account uses. Electronic professional claims use the X12 837 transaction that HIPAA names in 45 CFR 162.1102, version 005010X222.
MDLand describes tools for rejected or denied claims, with a one-click resubmit for reprocessing. We treat a rejection as a front-end fix and resend, and a denial as a payer decision that needs a different response; see rejection vs denial.
How does MDLand post ERA and 835 remittances?
MDLand says payments are posted electronically to your account. HIPAA's standard for the electronic remittance advice is the X12 835 under 45 CFR 162.1602, in version 005010X221, so the file that reaches the practice should be an 835.
MDLand's pages do not describe its posting rules, so we ask your administrator for the account's setup. On every remit we check the contractual adjustment, the patient responsibility, and the CARC and RARC codes behind any short pay, which then go to the denial queue.
What do we do inside MDLand?
- Check each charge for code, modifier, diagnosis and rendering provider
- Fix rejections fast enough that claims leave within 24 hours of charge entry
- Send claims over the clearinghouse connection your account already uses
- Match every remittance to the payment we expected and flag underpayments
- Run the denial queue, answering within 48 hours, and draft appeals when the record supports them
- Chase open balances, starting with the oldest and largest
- Issue patient statements on the schedule you choose
Which MDLand reports matter for denial and A/R tracking?
MDLand's pages mention financial tools but name no denial or A/R aging report, so we describe reports by purpose and map each to your account's names.
- Time from visit to charge entry (charge lag)
- Where each claim stands: unbilled, held, sent, rejected, paid
- Open A/R grouped by payer and age
- Repeat denial reasons by payer and provider
- Paid amounts compared with contracted or expected amounts
Your administrator points us to the matching screens or exports.
Which claim forms does MDLand produce?
MDLand's pages describe physician practices, so we expect professional claims. CMS describes the CMS-1500 as the standard claim form used by a non-institutional provider or supplier, and says the National Uniform Claim Committee (NUCC) is responsible for its design and maintenance.
If a client also bills facility claims, we confirm the claim type the account is enabled for before scope is set. Institutional work falls under hospital and facility billing.
What billing mistakes are common in MDLand?
We do not attribute defects to MDLand. These are the general checks we run on any account.
| Check | What we look at | Why it matters |
|---|---|---|
| Care management time | Minutes logged against consent date and service month | A gap in either weakens the claim |
| Patient identity | Name, birth date and subscriber ID compared with the insurance card and eligibility reply | One wrong character can bounce the claim at the front end |
| Modifiers | Every professional line against its code and the payer rule | A wrong or absent modifier cuts pay or invites a denial |
| Procedure notes | Documented procedure against the code billed, for example in iGI report writer accounts | Notes drive the charge, so a gap shows up as a denial later |
| Filing window | Date of service against the payer's timely filing limit | For Medicare, 42 CFR 424.44 sets one calendar year after the date of service |
| Payment posting | Write-offs, patient share and reason codes on each line | Wrong write-offs conceal underpayments |

Common questions
Straight answers from the team that does the work.
Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.
01Can an outside billing company work inside MDLand?
Yes, when the practice creates named logins for the billing team. MDLand's pages do not mention outside billers either way, so access is set by your administrator and your contract. We sign a BAA first and use one login per person.
02What access does a billing partner need in MDLand?
Named users with role-based access limited to billing areas, claim tools and reports, created by your administrator after the BAA is signed. Every user has two-step login and nobody shares a login. Your administrator also tells us the iClinic version, so roles map correctly.
03Which MDLand reports matter for denial and A/R tracking?
MDLand's pages do not name a specific denial or A/R aging report, so we pull by purpose: charge lag, claim status, A/R aging by payer, denial trends and payment variance. Your administrator points us to the matching screens or exports.
04How do claims move from MDLand to the clearinghouse and payer?
MDLand says the iClinic claim processing engine submits claims electronically to almost any payer and plan. It does not say which clearinghouse a practice uses, so we ask your administrator which connection the account runs on and where rejection reports arrive.
05How does MDLand post ERA and 835 remittances?
MDLand says payments are posted electronically to the account. HIPAA's remittance standard is the X12 835, version 005010X221, under 45 CFR 162.1602. The vendor's pages do not describe the posting rules, so we check the setup and verify every remit.
06What billing mistakes are common in MDLand (charge capture, modifiers, demographics)?
We do not attribute defects to MDLand. On any account we check rendering provider, service dates, modifiers, member ID and demographics before a claim leaves, then compare each remit with the expected payment. Charge capture gaps show first as charge lag.
07Does Nucleus Align have a partnership with MDLand?
No. We have no partnership, reseller status or endorsement with MDLand International Corp. We work inside systems clients already own, on logins the client creates, under a signed BAA. MDLand and iClinic are used here only as plain product names.
08Can you bill CCM and RPM from the MDLand care management modules?
Yes, if your practice uses them. MDLand says its modules hold consent forms and track care management time automatically, and we bill from those records. We check consent date, time logged and service month on each claim before it goes out, and we agree which services we bill at setup.
09How long do we have to file a claim from MDLand?
It depends on the payer. For Medicare, 42 CFR 424.44 says a claim must be filed no later than the close of the period ending 1 calendar year after the date of service, with listed exceptions. Other payers set their own limits in contracts or manuals.
10Does the MDLand claim engine replace a billing team?
No. The engine sends claims and posts payments, but someone still reviews charges, fixes rejections, works denials and follows up open A/R. We handle those queues, sending claims within 24 hours of charge entry and working denials within 48 hours.
11Medical billing service for MDLand users
Yes. Nucleus Align works inside MDLand on accounts you create, so you keep your system. We work the billing queues you hand over in MDLand, covering claim checks, remit posting, denials and A/R follow-up. Claims go out within 24 hours of charge entry and denials are worked within 48 hours.
12Can an outsourced billing team use our MDLand logins?
No. Nucleus Align uses its own named MDLand accounts, which you create with role-based access, behind two-step login and a signed BAA. No one shares a password. Each login is a named user. Your own staff keep their logins. You can remove access at any time.
