HCPCS S9088: services provided in an urgent care center
S9088 is a HCPCS Level II code for services provided in an urgent care center, and its descriptor says to list it in addition to the code for the service. It cannot stand alone. Medicare does not pay it, and the two payer policies cited here treat it as information about the place of service.
In short: S9088 is a companion line that says a service happened in an urgent care center. CMS marks it status I and not payable by Medicare. UnitedHealthcare calls it informational only and Premera says it indicates where the service was rendered, so it adds no payment where those policies apply.
What does HCPCS S9088 report?
S9088 flags that the service on the claim was provided in an urgent care center. The official descriptor reads "Services provided in an urgent care center (list in addition to code for service)", so the code is built to ride beside another code, never to replace one.
| Item | Value in the CMS file |
|---|---|
| Short label CMS uses | Services provided in urgent |
| Payment status (RVU26D, October 2026) | I, with 0.00 in every RVU column |
| Medicare coverage code (HCPCS October 2026 file) | I, "Not payable by Medicare" |
| Added date in the October 2026 file | January 1, 2001 (S9083 shows January 1, 2002) |
| Termination date | None listed |
| NCCI MUE (Q4 2026 practitioner table) | No row for this code |
Can S9088 be billed alone?
No. The descriptor itself says to list the code in addition to the code for the service, and a line with no service behind it has nothing for the add-on to describe.
It is a different tool from its sibling. S9083 is meant to stand for the whole visit as one fee, while S9088 sits beside the service code. A claim that carries both without a contract asking for it is sending two urgent care labels for one encounter.
How do payers treat S9088?
The published policies call it informational, which is why it earns no payment.
| Payer policy | What it says about S9088 |
|---|---|
| UnitedHealthcare Commercial and Individual Exchange, 2026R0108A | Considered informational only, as it pertains to the place of service and not the components of the service; CMS lists it with status I; UnitedHealthcare will not reimburse it |
| Premera Blue Cross, CP.PP.369.v2.1 | Informational in nature, used to indicate the place where the services are rendered; not reimbursed alone or with other services; contract exceptions allowed |
Both policies tell the biller to describe the service with a CPT or HCPCS code and to report place of service 20, urgent care facility. That makes the place of service field, not S9088, the carrier of the same information. Medicare is covered on the Medicare payer page.
Does S9088 change what the visit pays?
Not under any policy cited here. Medicare shows 0.00 in every RVU column for the code, UnitedHealthcare does not reimburse it, and Premera pays nothing for it alone or with other services. The payment for the visit rests on the service code beside it.
The one route to payment is a written exception. Premera's policy says exceptions may be made where a provider or employer group contract dictates otherwise. Treat a contract that names S9088 as the only evidence, and file the page that names it so a later denial can be answered against the wording.
How does S9088 sit on the CMS-1500?
S9088 occupies its own service line in box 24D of the CMS-1500, after the line for the service it describes. Place of service 20 goes in box 24B on each line, and units are 1.
- Put the CPT or HCPCS code for the service first, for example an office visit such as 99213.
- Add S9088 only when a contract or manual calls for it. Where none does, leave it off and rely on place of service 20.
- Do not attach a payer's global fee logic to this code. A case rate belongs to S9083 contracts, not to a place-of-service flag.
Which Item 24 fields carry the S9088 line?
S9088 takes its own Item 24 line. Item 24D holds the code, Item 24B repeats the place of service of the service line beside it, and Item 24G holds the unit, as Chapter 26 describes. Because the place of service already identifies the urgent care setting, the form carries the same information twice.
Why is S9088 denied?
S9088 denials usually arrive as a non-covered or bundled line. These are the X12 definitions.
| Code | What X12 says | What to check |
|---|---|---|
| CARC 96 | Non-covered charge(s) | Policy wording: informational only, no payment |
| CARC 97 | The benefit is included in the payment for another service already adjudicated | Whether the service line beside it was paid |
| CARC 204 | This service is not covered under the patient's current benefit plan | The member's plan and product line |
If these denials keep coming back, our denial management team can work them. For the specialty view, see urgent care billing and CO-97.

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.
01Can S9088 be billed alone?
No. The official descriptor says to list it in addition to the code for the service. A line with no service code beside it has nothing for S9088 to describe, and the payer policies cited here do not reimburse it even when it is billed with other services.
02Do I bill S9088 with an E/M code?
Only if the payer's contract or manual asks for it. Where it does, S9088 follows the E/M line, with place of service 20 on both. UnitedHealthcare and Premera do not reimburse S9088, so for them the E/M line carries the payment.
03Does Medicare pay S9088?
No. The October 2026 RVU26D file lists S9088 with status I, not valid for Medicare purposes, and the October 2026 HCPCS file gives it coverage code I, not payable by Medicare. Medicare uses other codes to report and pay urgent care services.
04What is the difference between S9088 and S9083?
S9083 is a global fee code meant to represent a whole urgent care visit, while S9088 is listed in addition to another service code. UnitedHealthcare calls S9083 a global code without encounter level specificity and S9088 informational only, and it reimburses neither one.
05Is place of service 20 enough without S9088?
Where a payer's policy says S9088 only indicates the place of service, place of service 20 does the same job. Premera tells billers to describe the service with a CPT or HCPCS code plus place of service 20, urgent care facility, and not to rely on the add-on.
