CPT 99051: after-hours and weekend clinic add-on code

CPT 99051 reports that an office visit took place during evening, weekend or holiday hours the clinic had scheduled in advance. It is an add-on to a basic service, and CMS bundles it: the October 2026 fee schedule file gives it status B with no RVUs. Whether a commercial payer pays it is a contract question.

In short: 99051 marks care given in regularly scheduled evening, weekend or holiday office hours. CMS status B means payment is always bundled into the basic service, and the Q4 2026 practitioner MUE table lists 0 units for it. Premera policy says it pays 99051 only where the provider contract allows.

What does CPT 99051 report?

In plain words, 99051 is a CPT add-on for a visit delivered in the office during evening, weekend or holiday hours that the practice sets in advance. It always rides with a basic service such as an office visit and is never a service on its own.

RVU26D defines status B this way: payment for covered services is always bundled into payment for other services not specified, and if RVUs are shown they are not used for Medicare payment. Here no RVUs are shown at all.

When does 99051 apply, and when does a neighbor code fit?

Use it when the visit falls inside hours the clinic regularly schedules for evenings, weekends or holidays. The neighboring after-hours codes describe different situations.

  • 99051: regularly scheduled evening, weekend or holiday office hours. This is the usual urgent care case.
  • 99050: care at times other than regularly scheduled hours or days, when the office is normally closed.
  • 99053: services between 10:00pm and 8:00am at a 24-hour facility.
  • 99058: an emergency in the office that disrupts other scheduled services.

Premera's After Hours Services policy lists this family as 99050 through 99060. Because the codes differ only by circumstance, the visit note should record the time of day and show the hours were part of the clinic's published schedule.

Do payers pay 99051?

Medicare does not, and Premera pays only where the contract says so. The Premera policy, CP.PP.137.v2.8 approved 02/06/26, treats the whole 99050 to 99060 range as included in payment for the main service.

Other plans publish their own rules, so a payer contract or policy page is the only reason to expect payment. See Medicare for the federal side and urgent care billing for how networks decide whether to enter the charge at all.

How does 99051 sit on the CMS-1500?

99051 gets its own line in box 24D of the CMS-1500, listed after the basic service it accompanies, with 1 unit in box 24G. The place of service on both lines should match; Premera names place of service 20 for an urgent care facility.

  1. Enter the basic service first, for example 99213.
  2. Add 99051 as its own line, units 1, same date and same place of service.
  3. Keep the visit time in the note so the evening or weekend hours can be shown.
  4. If the payer's contract does not pay it, decide in advance whether the practice enters the charge or writes it off, so denials are not worked one claim at a time.

The Q4 2026 MUE table carries a value of 0 for 99051 with the rationale CMS Policy. Together with status B, it is a reason not to expect Medicare Part B to pay the line.

Which Item 24 fields carry 99051?

Chapter 26 describes Item 24D as the code and modifiers, Item 24G as the units, with 1 entered when one service is performed, and Item 24B as the setting. 99051 occupies a second line after the visit code, with matching Item 24B.

Why is 99051 denied?

Most denials are bundling denials. These are the X12 definitions.

CodeWhat X12 saysTypical reading
CARC 97The benefit is included in the payment for another service already adjudicatedThe add-on was folded into the visit payment
CARC 96Non-covered charge(s)The plan does not recognise the code for payment
CARC 204This service is not covered under the patient's current benefit planThe benefit plan excludes after-hours charges

If these denials keep coming back, our denial management team can work them. See CO-97 for the bundling code, and S9083 for the other urgent care add-on question.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01Does 99051 get paid for evening and weekend visits?

Not by Medicare. The October 2026 RVU26D file gives it status B, bundled, with no RVUs, and the Q4 2026 MUE table lists 0 units. Premera treats it as included in the main service unless the provider contract allows separate payment.

02Do payers bundle 99051 into the E/M code?

Premera's policy says yes. Premera's After Hours Services policy treats codes 99050 to 99060 as included in the payment for the main service and does not reimburse them separately unless the contract specifically allows it. Check each payer's own policy before adding the line.

03Can 99051 be billed alone?

No. It is an add-on to a basic service, such as an office visit, and has no value without one. The CMS file shows no RVUs for it, so a claim with 99051 alone has nothing to pay and nothing to attach to.

04What is the difference between 99050 and 99051?

99051 is for services in regularly scheduled evening, weekend or holiday office hours. 99050 is for care at times other than regularly scheduled hours or days, when the office is normally closed. The visit note should show which situation applied.

05What does status B mean for 99051?

CMS defines status B as a bundled code: payment for covered services is always bundled into payment for other services not specified, and any RVUs shown are not used for Medicare payment. For 99051 the RVU columns are all 0.00.
