HCPCS G2211: Office Visit Complexity Add-On Code
G2211 is a Medicare add-on code reported with an office, outpatient or home visit when the practitioner is the continuing focal point for a patient's care or is part of ongoing care for a serious or complex condition. The complexity it pays for sits in the relationship, not in the day's diagnosis. It cannot be billed alone.
In short: G2211 is a Medicare-specific add-on to office, outpatient and, since January 1, 2026, home or residence visits. It has no minimum level, no required diagnosis and no extra documentation rule, but it is not paid when the base visit carries modifier 25, except with a wellness visit, vaccine administration or Part B preventive service.
What Does HCPCS G2211 Report?
G2211 is a HCPCS Level II add-on code. Its official descriptor begins "Visit complexity inherent to evaluation and management" and names care that is the continuing focal point for all needed services or part of ongoing care for a serious or complex condition. It is an add-on code: it is listed beside a base visit and never stands alone.
| Item | Value in the file |
|---|---|
| Short label CMS uses | Complex e/m visit add on |
| Payment status | A (active code) |
| Work RVU | 0.33 |
| Non-facility total RVU | 0.52 |
| Facility total RVU | 0.43 |
| Global period | ZZZ (add-on, always with another service) |
| NCCI MUE | Not listed in the October 1, 2026 practitioner table |
When Can G2211 Be Added to an Office Visit?
The relationship between the practitioner and the patient decides, not the problem treated. Chapter 12 of the Claims Processing Manual gives two tests: the practitioner is the continuing focal point for all needed services, as a primary care practitioner is, or is part of ongoing care for a single, serious or complex condition, such as sickle cell disease or HIV.
- Fits: a primary care physician sees a regular patient for sinus congestion. Chapter 12 says the complexity lies in the continued responsibility, not in the sinus problem.
- Fits: an infectious disease physician in ongoing care of a patient with HIV.
- Does not fit: discrete, routine or time-limited care. CMS names a mole removal, a simple virus, seasonal allergy counseling, initial onset reflux and a fracture.
The CMS FAQ adds that G2211 is not appropriate when the practitioner has not taken, or does not plan to take, responsibility for ongoing care over time. It defines no minimum visit interval.
Which Visits Can Carry G2211?
Chapter 12 section 30.6.19 allows G2211 with office and outpatient codes 99202 to 99205 and 99211 to 99215 at any level, and from January 1, 2026 with home or residence codes 99341, 99342, 99344, 99345 and 99347 to 99350. Any specialty may report it, and 99213 and 99214 qualify as much as a high-level visit.
The CMS FAQ says G2211 cannot be billed with hospital inpatient, emergency department or nursing facility visits.
Can G2211 Be Billed When the Visit Has Modifier 25?
Sometimes, since 2025. The CY 2024 final rule made G2211 not payable with a modifier 25 visit, and clarified that this applies when the same practitioner bills the visit and the other service that day.
| Date | Rule for a base visit with modifier 25 | Source |
|---|---|---|
| Before January 1, 2025 | G2211 not paid on the same date as an office visit with modifier 25 by the same practitioner | Chapter 12 section 30.6.19 |
| January 1, 2025 | May be paid when an annual wellness visit, vaccine administration or any Medicare Part B preventive service is also done that day | CY 2025 final rule, 89 FR 97858; MLN006764 |
| January 1, 2026 | The same exception applies to the home or residence visit codes | CMS G2211 FAQ, question 6 |
The CY 2025 rule confirms the initial preventive physical exam counts as a Part B preventive service. It kept the bar for visits billed with modifier 25 beside minor procedures, so see modifier 25 before you add the code.
Who Gets Paid, and What Does the Patient Owe?
G2211 is separately payable in both facility and non-facility settings, and the usual Part B coinsurance and deductible apply. CMS does not treat it as duplicative of care management in the same period.
Federally Qualified Health Centers and Rural Health Clinics are different. CMS says G2211 is bundled into the encounter-based rate, with no separate payment. If the patient sees a colleague in your group, CMS says a team-based practice may still be the focal point, so the add-on may fit.
What Must Be Documented, and How Does It Appear on the Claim?
CMS has set no extra documentation rule. Reviewers may use the record to confirm necessity and the care relationship, and the FAQ names diagnoses, the assessment and plan, and other codes billed as support. The base visit needs its own MDM or time.
On the CMS-1500, G2211 goes on its own line in Item 24D beside the base visit. The NCCI practitioner MUE table effective October 1, 2026 has no entry for G2211.
Where Does G2211 Go on the CMS-1500?
G2211 takes its own line in Item 24D, with 1 in Item 24G, below the base visit. Item 24E points to a diagnosis in Item 21 for each line, one reference per line (Chapter 26).
Why Is G2211 Denied?
| Code | What X12 says | What usually causes it |
|---|---|---|
| CO-97 | The benefit is included in the payment for another service already adjudicated | G2211 was sent with an E/M that carries modifier 25 and no preventive service that day |
| B15 (CARC) | This service requires that a qualifying service be received and covered | The base visit was denied or not on the claim |
| CO-4 | The procedure code is inconsistent with the modifier used | An ED, inpatient or nursing facility base code, or a modifier mismatch on the base visit |
| CO-50 | Non-covered because the payer does not deem it medically necessary | The note shows a one-off problem with no ongoing relationship |

Straight answers from the team that does the work.
Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.
01When can G2211 be added to an office visit?
When the practitioner is the continuing focal point for all of the patient's needed services, or is part of ongoing care for a single, serious or complex condition, and the base visit is medically necessary. Level and specialty do not matter, and no specific diagnosis is required. One-off, routine or time-limited problems do not qualify.
02Can G2211 be billed with modifier 25?
Only in the exception cases. Medicare does not pay G2211 when the base visit carries modifier 25, except on days with an annual wellness visit, vaccine administration or Part B preventive service. That exception began January 1, 2025 for office visits and January 1, 2026 for home visits.
03Can G2211 be billed in an FQHC or RHC?
CMS does not pay it separately. It is bundled, with the base visit, into the all-inclusive rate for a Rural Health Clinic or the prospective payment rate for a Federally Qualified Health Center, so there is no extra payment for the add-on.
04Does the patient pay coinsurance on G2211?
Yes. CMS states that the usual Part B coinsurance and deductible apply when G2211 is billed, so the patient's share applies to the add-on line as well as to the base visit. Patients should hear about it before the visit.
G2211 Denied on Visits That Qualify?
We check the base visit, the modifier and the relationship before the add-on is sent.
