N290 remark code: missing rendering provider identifier

N290 is a remittance advice remark code that tells you the rendering provider's primary identifier is missing, incomplete or invalid. On a Medicare CMS-1500 claim that identifier is the rendering NPI in item 24J, so the fix is to correct that field and send the claim again.

In short: N290 means the payer could not use the identifier of the provider who performed the service. Check item 24J (or the rendering provider loop on the 837P), correct the NPI or the enrolment record behind it, and resubmit as the remittance instructs.

What does the N290 remark code mean?

X12 lists N290 as: "Missing/incomplete/invalid rendering provider primary identifier." The entry shows a start date of 12/02/2004 and carries no "Alert:" label, so it is not an alert. A remittance advice remark code (RARC) adds detail to the adjustment on a remittance advice line.

The rendering provider is the clinician who performed the service, as opposed to the group or facility that bills for it. The primary identifier is normally the NPI. N290 does not say which of the three problems you have: nothing entered, a number that is incomplete, or a number the payer cannot accept. Reviewed October 25, 2026.

When does Medicare return a claim with N290?

The Medicare Claims Processing Manual, chapter 1, names N290 in two places. Section 80.3.2.1.2 tells the MAC to return a claim as unprocessable when a rendering physician, physician assistant, nurse practitioner, clinical nurse specialist, supplier or other practitioner, whether a sole practitioner or a member of a group practice, does not enter an NPI in item 24J of the CMS-1500. The remittance code set for that group of returns is group code CO with CARC 16.

The same section carves out influenza virus and pneumococcal vaccine claims submitted on roster bills, which do not require a rendering provider NPI. Section 80.3.2.1.3 adds a second case: a dialysis-patient physician paid by monthly capitation, in a professional corporation or clinic, who leaves the NPI out of item 24J.

Chapter 26 explains where the NPIs sit. Item 24J holds the rendering provider's NPI in the lower unshaded portion, and the shaded portion is not reported. The entity that bills puts its own NPI in item 33a. A group claim therefore carries two different numbers, and mixing them up is a common way to trigger this remark.

Which adjustment reason code goes with N290?

A remark code never travels alone. On the 835 it rides on a line with a CARC, and the CARC gives the reason for the adjustment while the RARC supplies the detail. X12's note on CARC 16 says at least one remark code must be provided, and that it may not be an alert, so N290 is eligible to sit beside it.

For Medicare, the CMS manual is explicit about the unprocessable returns above: group code CO, CARC 16, and the RARC shown for the item that failed. See CO-16 for the full CARC. For other payers, read the CARC on your own remit.

How do you fix N290?

1
Find the line
Match the remit line to the claim line and read what sits in item 24J, or in the rendering provider loop of the 837P.
2
Compare to the enrolment record
Check the number against the clinician's NPI and the group's enrolment. Chapter 1, section 80.3.1, defines invalid information as present but incorrect or no longer in effect, such as an expired number.
3
Fix the source
Correct the provider table in the practice management system, or finish the enrolment link, so the next claim does not repeat the error.
4
Send it the way the remit says
Section 80.3.2 lets the provider submit a corrected claim or an entirely new claim when data were not retained, as with a front-end return or a remittance-advice return. A claim returned this way has no reconsideration notice, so do not file an appeal first. Chapter 1, section 70, sets a 12-month filing limit; see timely filing limits.
Example: made-up claim, illustrative only

A radiology group adds a new reader mid-month and loads the clinician in billing before the payer shows the enrolment link.

Item 24JHolds the new reader's NPI, but the payer cannot match it to the group

How does N290 read on the remittance?

N290 sits with CARC 16 and group CO in the line-level LQ segment. The field is the rendering NPI in item 24J of the CMS-1500. Check that the NPI on the line is the rendering provider's, then send a new claim.

Where does N290 turn up?

A group claim carries a different rendering NPI on each line, so teams in radiology billing, anesthesia billing and emergency medicine billing check item 24J whenever a clinician joins or leaves. When the whole claim is unprocessable, the wording on MA130 applies. For a modifier gap on a line, see N822, and for same-session conflicts, M80. Back to the code library. FAQ

Common questions

Straight answers from the team that does the work.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01What does remark code N290 ask me to fix?

N290 asks you to correct the rendering provider's primary identifier, normally the NPI. On a CMS-1500 that is item 24J. It may be blank, incomplete, or invalid, such as expired. Compare it with the clinician's enrolment record, correct the source, and resubmit as the remittance instructs.

02How do RARCs pair with CARCs on an 835?

The CARC states why the amount was adjusted, and the RARC adds detail. X12 says CARC 16 needs at least one remark code that is not an alert. Medicare's manual pairs unprocessable returns with group code CO and CARC 16. Read both codes on each line of your own remit.

03Is N290 a denial I can appeal?

For Medicare, usually not. Chapter 1 of the Claims Processing Manual says a claim returned as unprocessable for incomplete or invalid information is not denied and carries no appeal rights. Correct item 24J and send a corrected or new claim. Appeal only if the corrected claim is later denied.

04Which claim box holds the rendering NPI?

Item 24J on the CMS-1500, in the lower unshaded portion, per chapter 26 of the Claims Processing Manual. The billing entity's NPI goes in item 33a. On an electronic 837P claim the rendering provider is reported in its own loop, so ask your clearinghouse where the value maps.

05Do vaccine roster bills need a rendering NPI?

Medicare's manual excepts influenza virus and pneumococcal vaccine claims submitted on roster bills from the rendering provider NPI requirement in item 24J. Every other service on a CMS-1500 still needs a rendering NPI in item 24J, so a missing number on an ordinary visit line will still be returned.

06Why did N290 appear when I entered an NPI?

Chapter 1 treats a number that is present but incorrect, or no longer in effect, as invalid information. The NPI may be mistyped, may not be linked to the billing group, or may belong to a clinician the payer has not enrolled. Check the registry and your enrolment record.

Rendering NPI errors repeating?

We check the rendering provider on every line before the claim leaves.
