Miami hospital billing: Medicare Advantage and payers
Miami hospital billing runs through a county-run system, Jackson Health System, and private systems such as Baptist Health South Florida that publish plan participation by hospital. The plan lists change by date, one Medicare Advantage plan has a pending termination, and Miami-Dade sits in Medicaid Region I.
In short: miami claims depend on which hospital entity is on the claim. Jackson screens unfunded patients by county residency and income, Baptist lists its accepted plans per hospital as of October 1, 2026, and five Miami-Dade hospitals hold state trauma designations. Whether Florida law or federal IDR decides an ER payment depends on the plan type.
Major payers
Which Miami hospital systems shape how claims get paid?
Miami-Dade County has one public system and several private ones, and each bills differently. Jackson Health System is run for the county, Baptist Health South Florida publishes plan participation by hospital, and HCA Florida Kendall and HCA Florida Aventura sit inside a national for-profit chain. Check which CMS certification number (CCN) your claim is filed under before you read any network list.
CMS Care Compare's hospital general information file (modified July 22, 2026) lists 20 hospitals in Miami-Dade County. It classes Jackson Health System as Government, Hospital District or Authority, Baptist Hospital of Miami, South Miami Hospital and West Kendall Baptist Hospital as voluntary non-profit, and HCA Florida Kendall Hospital as proprietary. The same file lists Mount Sinai Medical Center of Florida in Miami Beach as voluntary non-profit.
| System or hospital | What the source says | What to expect on the claim |
|---|---|---|
| Jackson Health System (Jackson Memorial, Jackson South, Jackson North, Jackson West) | Governed by the Public Health Trust on behalf of the Miami-Dade Board of County Commissioners (Jackson's About Us page) | County residency drives charity and unfunded accounts; specialist fees bill separately |
| Baptist Health South Florida (Baptist Hospital, South Miami, West Kendall, Homestead, Doctors, Mariners, Fishermen's) | Publishes accepted plans per hospital, dated October 1, 2026 | Pull the list for the entity on the claim; Doral Hospital is covered by the South Miami Hospital list |
| HCA Florida Kendall Hospital | Level I trauma center on the Florida Department of Health list updated July 23, 2026 | Trauma activation claims; proprietary ownership per CMS |
| HCA Florida Aventura Hospital | Level II trauma center on the same list | Level II designation; CMS lists it as voluntary non-profit |
| Nicklaus Children's Hospital | Pediatric trauma center on the same list | Pediatric designation; confirm the payer on each claim |
| Mount Sinai Medical Center of Florida | Listed by CMS in Miami Beach as voluntary non-profit | Read its current plan list directly |
Check Mount Sinai's current plan list on its own site before quoting a network.
Which Miami-Dade hospitals are trauma centers?
The Florida Department of Health list, updated July 23, 2026, names five Miami-Dade trauma designations: Jackson Memorial Hospital and HCA Florida Kendall Hospital at Level I, Jackson South Medical Center and HCA Florida Aventura Hospital at Level II, and Nicklaus Children's Hospital as a pediatric trauma center. A trauma claim from any of the five carries a designation the payer can check against that list.
Jackson's own history page says Ryder Trauma Center opened on August 3, 1992, that Jackson South opened a provisional Level II trauma center, and that Jackson Memorial was designated Level I when an eight-hospital trauma network formed in the county. It also says voters approved an extra half-penny sales tax to support Jackson Memorial, with trauma care named as one of three primary needs. That county funding is why Jackson accounts are screened for Miami-Dade residency, covered next.
Keep trauma activation documentation in the chart before billing. Payers ask for proof the center was designated on the date of service, and the state list is dated, so print the version that matches the date.
How do Jackson Health System claims differ for uninsured and Medicaid patients?
Jackson sorts unfunded patients by Miami-Dade residency and income before it bills anyone. Its financial assistance policy says patients who are potentially eligible for charity care and reside in the county must complete an interview and sign the Jackson Charity Care and Grant Programs application. It requires proof of county residency and proof of family gross income.
The fee scale follows the Federal Poverty Guidelines. Jackson states that charity plan code classification and patient fee responsibility come from a sliding fee scale based on income and current guidelines up to 300 percent. A patient who no longer lives in the county does not qualify for Jackson Prime Card benefits.
Jackson's billing page also tells patients that fees from their personal physician and specialists do not appear on the hospital bill. Anesthesiologists, surgeons, pathologists and radiologists send separate bills, and the page tells patients to file claims with their insurer for those. If you bill professional services for a group that covers a Jackson site, expect the patient and the payer to see two claims for one stay.
Jackson says its patient access staff can help with Medicaid applications, and it uses outside agencies for collection of patient balances. A Medicaid denial on a Jackson account is therefore often an eligibility timing problem, not a coding one. Our Medicaid payer guide covers the general appeal path.
Which Medicare Advantage plans are in network at Baptist Health South Florida?
Baptist Hospital of Miami and South Miami Hospital list Aetna, CarePlus, Devoted Health, Doctors Healthcare Plans, Florida Blue, HealthSpring, Humana, Leon Health and Preferred Care Network (UnitedHealthcare's United MedicareMax) as accepted Medicare Advantage plans, as of October 1, 2026. Traditional Medicare is accepted too. The Leon Health line carries a note that a contract termination is pending and the date is being finalized.
Read that note as a billing trigger. A termination date that is not yet set means claims for dates of service near the cutoff may be treated differently, so ask the plan for the effective date in writing and hold a flag on Leon accounts until you have it.
Baptist's list is a one-system fact. Pull the current lists for Jackson, HCA Florida Kendall and Mount Sinai before quoting their Medicare Advantage networks. Our Medicare Advantage guide and the Humana guide cover appeal steps that apply across hospitals.
A caution on sources: an older Baptist Health Medicare Advantage chart at baptisthealth.com belongs to a different Baptist Health, whose chart lists Passport and Louisville and Hardin locations, not the Miami system. The Miami system's list lives at baptisthealth.net. Check the domain before you quote a network status.
What do the Baptist lists say about commercial, exchange and Medicaid plans?
Baptist Hospital of Miami lists these commercial plans: Aetna (excluding Aetna First Choice Advantage HMO), AvMed, Florida Blue, Cigna, Great West (a Cigna company), Neighborhood Health Plan and UnitedHealthcare. It also lists the Multiplan family of networks, Dimension Health and First Health. Exchange plans listed are AvMed Entrust, Florida Blue and Oscar, plus Cigna Connect, which the list says is no longer offered as of 01/01/2027.
The Aetna exclusion matters at the front desk. An Aetna card with the First Choice Advantage HMO product is not covered by the Aetna line, so eligibility has to read the product, not the carrier name. Our Oscar guide and Aetna guide cover those payers' own rules.
Medicaid HMO and PSN plans listed are Aetna Better Health (Medicaid and HealthyKids), Florida Community Care, Humana Medicaid, Molina Healthcare of Florida, Sunshine Health (including Child Welfare) and UnitedHealthcare of Florida Medicaid. The list also names TRICARE Prime and TRICARE Select, and workers' compensation networks. Baptist's list says it is not a promise of coverage or payment from the insurer, so a listed plan can still deny on benefits.
How does Miami's international patient volume change the payer list?
Baptist's accepted-insurance list carries two groups most US hospitals do not: international plans and international PPO networks. Named international plans include Pan American Life Insurance Group, Seguros Sura and Generali Global Health Services, and networks include Aetna International, Cigna International and UnitedHealthcare International. Baptist also runs an International Patients page in its patient resources.
These claims do not follow a domestic payer's electronic path. Confirm the payer's claim address and format before charge entry, and set a separate follow-up queue, because a US clearinghouse rejection will not tell you the plan sits abroad.
Which Medicaid managed care region covers Miami-Dade?
Miami-Dade County is in Statewide Medicaid Managed Care Region I, together with Monroe County. Florida Statutes section 409.966 lists nine regions, A through I, and Region I consists of Miami-Dade and Monroe Counties. AHCA's page on the new regions says the program moved from eleven numbered regions to nine lettered regions in February 2025.
That change is a billing trap for old paperwork. A plan-by-region chart dated before February 2025 uses the eleven-region numbering, so Miami-Dade will not appear as Region I on it. Use the Baptist plan list above to see which Medicaid plans one major system accepts, and the plan's own provider manual for filing rules. The statewide rules are on the Florida state page.
Does Florida law or federal IDR apply to a Miami ER claim?
It depends on the plan, not the hospital. Baptist's patient notice cites Florida Statutes 641.3154 and 627.64194 and the federal No Surprises Act, which matches the split we describe on the Florida state page and in our state surprise billing laws guide. Fully insured plans follow the state route; self-funded employer plans follow federal IDR.
The Baptist notice names the services where an out-of-network provider at an in-network hospital cannot balance bill: emergency medicine, anesthesia, pathology, radiology, laboratory, neonatology, assistant surgeon, hospitalist and intensivist. For Medicare Advantage, federal rule 42 CFR 422.113(b)(2) makes the plan financially responsible for emergency services whether obtained inside or outside its network and whether or not prior authorization existed. Our out-of-network and IDR service handles the dispute side.
How we handle billing for Miami groups and facilities
We bill Miami hospitals and emergency groups remotely. Our only US office is the registered office in Richmond, KY, and our team works from Vancouver, BC, Dubai, UAE and Islamabad, PAK. We do not have staff in Miami.
For hospital claims we prepare institutional UB-04 claims, validate revenue codes, and check bill type, patient status and payer-specific rules before submission, then reconcile payments and work denials within 48 hours. Claims go out within 24 hours of charge entry. For out-of-network disputes we run eligibility assessment, open negotiation and federal IDR submissions with deadline tracking. 01Which Medicare Advantage plans are in network at the large Miami hospital systems?
Baptist Hospital of Miami lists Aetna, CarePlus, Devoted Health, Doctors Healthcare Plans, Florida Blue, HealthSpring, Humana, Leon Health and Preferred Care Network as of October 1, 2026, with Leon's termination pending. Pull each other hospital's current list before you quote a network.
02How does a Miami hospital handle an out-of-network Medicare Advantage ER claim?
Under 42 CFR 422.113(b)(2) the plan is financially responsible for emergency services inside or outside its network, with or without prior authorization. Bill the claim with the emergency indicators intact, keep the triage record, and appeal a denial citing the prudent layperson standard in the same section.
03Does Florida's surprise billing law or federal IDR apply to a Miami ER claim?
It turns on the plan. Fully insured plans fall under Florida's emergency payment statutes and the AHCA dispute program. Self-funded employer plans follow federal open negotiation and IDR. Check the plan type on the card or through eligibility, then use the Florida state page for the statute detail.
04How do Jackson Health System claims for uninsured and Medicaid patients differ from private hospital claims?
Jackson screens unfunded patients for Miami-Dade residency and income, using a sliding fee scale up to 300 percent of the Federal Poverty Guidelines, and helps with Medicaid applications. Specialist fees bill separately from the hospital bill, so one stay can produce two claims.
05How do Spanish-language Medicare Advantage plans like CarePlus affect denial rates?
The useful number is your own denial rate by plan. Baptist Health's plan list shows that CarePlus is listed as accepted at Baptist Hospital of Miami. Track denials by plan in your own remittance data and compare.
06What are the Florida Blue timely filing and appeal steps for a Miami facility?
Florida Blue's provider manual, part 2, states a 180-day timely filing limit for claim submissions. Part 3 lists Timely Filing Appeal as an administrative appeal type on its reconsideration form. Follow the steps in our Florida Blue payer guide and keep the clearinghouse acceptance report as proof.
07Which Medicaid region is Miami-Dade in?
Region I, together with Monroe County, as listed in Florida Statutes section 409.966. The program moved from eleven numbered regions to nine lettered regions in February 2025, so older charts show a different region number for Miami-Dade. Use the lettered regions for any new plan lookup.
08How many Miami-Dade trauma centers does Florida list?
The Florida Department of Health list updated July 23, 2026 names five: Jackson Memorial and HCA Florida Kendall at Level I, Jackson South and HCA Florida Aventura at Level II, and Nicklaus Children's as a pediatric trauma center. A trauma claim should match one of those five names and the stated level.
09How should we bill international patients treated at a Miami hospital?
Baptist lists international plans and international PPO networks beside domestic ones. Confirm the payer's claim address and format before charge entry, keep these accounts in a separate follow-up queue, and do not assume a US clearinghouse will route the claim.
