San Diego medical billing company: what is different in San Diego County
San Diego County bills differently from the rest of California in three ways: a Medi-Cal market with four county-listed plans, health systems that own their own medical groups and plan, and a large military and TRICARE presence. This page covers the county; the statewide Medi-Cal, Medicare and surprise billing rules are on the California page.
In short: san diego claims depend on which of four Medi-Cal plans, which system and which payer sits behind the patient. Scripps, Sharp, UC San Diego Health, Rady Children's, three hospital districts and two Department of Defense hospitals each bill differently. TRICARE claims have a one year filing limit.

Which San Diego health systems and hospitals matter for claims?
San Diego County has six EMSA-listed trauma centers, three public hospital districts in the CMS file, and two Department of Defense hospitals. The ownership type changes who negotiates contracts, who pays and which claim rules apply.
| Hospital (name as CMS lists it) | Ownership per CMS | Trauma status per EMSA |
|---|---|---|
| Scripps Mercy Hospital | Voluntary nonprofit, private | Adult Level I |
| Scripps Memorial Hospital La Jolla | Listed as Physician-owned | Adult Level I |
| UC San Diego Health Hillcrest Medical Center | Voluntary nonprofit, other | Adult Level I (UC San Diego Medical Center) |
| Sharp Memorial Hospital | Voluntary nonprofit, private | Adult Level II |
| Rady Children's Hospital San Diego | Voluntary nonprofit, other | Pediatric Level I |
| Palomar Health Downtown Campus and Palomar Medical Center Poway | Government, hospital district or authority | Palomar Medical Center, adult Level II |
| Tri-City Medical Center and Grossmont Hospital | Government, hospital district or authority | Not on the EMSA list |
| Kaiser Foundation Hospital, San Diego and San Marcos | Voluntary nonprofit, private | Not on the EMSA list |
| Naval Medical Center San Diego and Naval Hospital Camp Pendleton | Department of Defense | Not on the EMSA list |
The table is a sample of the CMS list for San Diego County, last modified July 22, 2026, and of the EMSA list updated October 22, 2025. CMS records Scripps Memorial La Jolla's ownership as physician, which looks like a data quirk, so confirm contract entities with the facility itself. Local EMS agencies, not EMSA, set the levels.
How are Medi-Cal managed care plans organized in San Diego County?
The County of San Diego names four Medi-Cal managed care plans with executed agreements dated September 30, 2025: Blue Shield Promise, Community Health Group, Kaiser Permanente and Molina. County staff in the Clinical and Safety Net Coordination division link those plans with Enhanced Care Management and Community Supports services.
The DHCS county table puts San Diego in the Geographic Managed Care model, one of the five Medi-Cal managed care models DHCS lists, and Los Angeles in the Two-Plan model. The practical step for a hospital is a plan-by-plan eligibility check. Our Medi-Cal and Medicaid payer guide covers fee-for-service and plan basics, and the six month fee-for-service limit sits on the California page.
There is no single San Diego filing deadline. Each plan's provider manual or contract sets its own deadline.
How do Sharp and Scripps own their medical groups and plans?
Sharp HealthCare says it has five acute-care hospitals, five specialty hospitals, three affiliated medical groups and one health plan. A Sharp patient can therefore be tied to a Sharp hospital, a Sharp-affiliated group and the Sharp plan, so confirm the affiliation on each contract.
Scripps Health describes itself as a not-for-profit integrated system with four hospitals on five campuses, Scripps Clinic and Scripps Coastal Medical Center, more than 30 outpatient centers and four emergency rooms. It states that it operates two of San Diego's three Level 1 trauma centers, out of six trauma centers in total, which matches the EMSA list.
Whether a Sharp or Scripps arrangement is capitated is set in the contract. Capitation is a fixed payment per member rather than per claim, and it appears in contracts, not in public notices. Read the contract before deciding who bills the professional claim.
What are San Diego's hospital districts, and how do they affect a claim?
CMS lists Grossmont Hospital, Palomar Health and Tri-City Medical Center as Government hospital district or authority.
For billing, the point is contractual: confirm the legal entity on the contract, the campus for each service and which entity holds the payer agreement. Palomar's CMS entries split into a Downtown Campus and a Poway medical center, so check which campus appears on a claim.
How do military and border-region patients affect payer mix?
CMS lists two Department of Defense hospitals in the county, Naval Medical Center San Diego and Naval Hospital Camp Pendleton, and a VA San Diego Healthcare System. Civilian providers who see TRICARE beneficiaries bill TRICARE under rules set by the Defense Department.
Under 32 CFR 199.7(d), TRICARE claims must be filed no later than one year after the services are provided. A claim returned for more information must be resubmitted by the later of one year after service or 90 days from the date the claim was returned. The regulation lets the OCHAMPUS director grant exceptions for retroactive eligibility, administrative error and delays by other insurance.
Payer mix for cross-border patients varies by group. If your groups see cross-border patients, record the payer class at registration instead of assuming Medi-Cal or commercial.
A TRICARE patient is treated on March 10, 2026. The claim is due by March 10, 2027. If TRICARE returns it for missing information in February 2027, the later of March 10, 2027 or 90 days from the return date applies.
How does an ER group bill when a Kaiser member uses a non-Kaiser San Diego ER?
CMS lists two Kaiser Foundation hospitals in the county, San Diego and San Marcos. A Kaiser member seen at an outside ER still produces a claim to Kaiser's southern California claims unit, and Kaiser's notice asks outside providers to file within 90 calendar days of service or discharge unless a contract or law differs.
The full claim and dispute steps are in our Kaiser Permanente billing guide. For San Diego groups, the first check is whether the emergency physician group has its own Kaiser agreement, because the agreement can change the deadline.
How do Rady Children's claims differ from adult hospital claims?
Rady Children's Hospital San Diego is part of Rady Children's Health, which joined with Children's Hospital of Orange County and describes two of the region's Level 1 pediatric trauma centers. EMSA lists Rady Children's Hospital-San Diego at pediatric Level I.
A pediatric hospital still bills under the same Medi-Cal and commercial rules as an adult hospital; the differences come from the patient's coverage. Check the DHCS California Children's Services program rules for any child enrolled in it. Check eligibility for each child, then the plan, then any special program.
Which California rules apply to out-of-network ER billing for a San Diego patient?
Emergency care is not covered by AB 72, which applies to non-emergency care by a noncontracting professional at a contracting facility. The CMS letter to California sends other items and services, including emergency care, to federal IDR. Self-funded plans are outside the state law altogether.
San Diego adds no local rule. The state rules and dispute paths are on the California page and in our state surprise billing law summary. For underpayments, see out-of-network billing and IDR.

01How do capitated contracts at Sharp and Kaiser change how a San Diego hospital gets paid?
Capitation pays a fixed amount per member, not per claim. Sharp says it has three medical groups and a health plan, and Kaiser is an integrated system. Capitation terms sit in the contract, so read it. Confirm who holds the risk before billing the professional claim.
02How does an ER group in San Diego bill when the patient has a Kaiser plan and goes to a non-Kaiser ER?
File with Kaiser's southern California claims unit within 90 calendar days of service or discharge, unless your agreement differs. Use the patient's details and medical record number. Disputes are due within 365 calendar days. See our Kaiser guide for the steps.
03How do Rady Children's claims differ from adult hospital claims in San Diego?
Rady bills under the same Medi-Cal and commercial rules as an adult hospital, and the differences come from the child's coverage. EMSA lists it at pediatric Level I. Check eligibility first, and check the DHCS California Children's Services program rules for any enrolled child.
04How do border-region and military patients in San Diego affect payer mix (TRICARE, Medi-Cal)?
TRICARE claims must be filed within one year under 32 CFR 199.7(d), and Medi-Cal fee-for-service uses the six month rule. Record each patient's payer class at registration.
05Which California rules apply to out-of-network ER billing for a San Diego patient?
AB 72 covers non-emergency care by a noncontracting professional at a contracting facility. Emergency care is not in it, so the federal No Surprises Act process applies, and self-funded plans are outside the state law. Find the plan type before choosing a path.
06How are Medi-Cal managed care plans organized in San Diego County?
The County of San Diego lists four Medi-Cal managed care plans with agreements dated September 30, 2025: Blue Shield Promise, Community Health Group, Kaiser Permanente and Molina. The DHCS county table shows San Diego in the Geographic Managed Care model. Confirm each member's plan before billing.
07Which San Diego hospitals are Level I trauma centers?
EMSA's October 2025 list shows adult Level I at Scripps Memorial Hospital La Jolla, Scripps Mercy Hospital and UC San Diego Medical Center, and pediatric Level I at Rady Children's Hospital-San Diego. Sharp Memorial and Palomar are Level II. The list can change, so check the current version.
08What is the TRICARE filing deadline for San Diego claims?
Under 32 CFR 199.7(d), TRICARE claims must be filed no later than one year after the services are provided. A claim returned for more information is due by the later of one year after service or 90 days from its return. Exceptions exist for retroactive eligibility and administrative error.
