Wellcare billing: Medicare Advantage timely filing and appeals
Wellcare's 2026 Medicare Advantage Provider Manual gives participating providers 180 calendar days to submit a clean claim, and 90 calendar days to appeal a claim or utilization review denial. A footnote to the filing sentence sets 95 days for Texas health plans.
In short: submit Wellcare Medicare Advantage claims within 180 calendar days of the date of service or discharge unless your agreement says otherwise, appeal a denial within 90 calendar days, and use Availity Essentials for claims, status and authorizations. Corrected and voided claims count against the same filing clock.

Top denials from Wellcare
| Code | Meaning | Our fix |
|---|---|---|
| CO-197 | Authorization absent, expired or exceeded | Look up the service before scheduling. On appeal, document why it was done without authorization. |
| CO-29 | Time limit for filing has expired | Send the clearinghouse acceptance report or a submission sheet with the required identifiers. |
| CO-50 | Not deemed a medical necessity by the payer | File a reconsideration within 90 calendar days with the full medical record. |
| CO-16 | Missing or invalid claim information, or a corrected claim sent without frequency code 7 or 8 and the original claim number | Resubmit with the code and REF*F8 reference, so it is not treated as a first-time claim. |
What is the Wellcare timely filing limit?
Wellcare's 2026 Medicare Advantage Provider Manual says participating providers must submit clean claims within 180 calendar days from the date of discharge for inpatient services, or from the date of service for all other services. The sentence begins "Unless otherwise stated in the Provider Agreement," so a different period in your contract controls.
The footnote on that sentence reads "95 days for Texas health plans." If you bill Wellcare in Texas, read both the footnote and your agreement before you set a work queue date.
| Claim type | Where the filing clock starts | Limit in the manual |
|---|---|---|
| Professional claim (CMS-1500 or 837P) | The "from" date on the claim | 180 calendar days; 95 days for Texas health plans per the footnote |
| Institutional claim (UB-04 or 837I) | The "through" date on the claim | 180 calendar days; 95 days for Texas health plans per the footnote |
| Inpatient services | Date of discharge | 180 calendar days |
| Corrected or voided claim | Same clock as the original | The manual says initial, corrected and voided claims all fall under the limit |
Wellcare may deny a claim that is late or not a clean claim, and the manual says a provider whose claim is denied that way must not bill or accept payment from the member for those services. See timely filing limits by payer for how other payers compare.
What counts as proof of timely filing?
Wellcare accepts a clearinghouse electronic acknowledgement showing the claim was accepted by Wellcare. It also accepts your own electronic submission sheet that carries the patient name, provider name, date of service matching the EOB or claim, prior submission bill dates and Wellcare's product name or line of business.
It does not accept a SNIP rejection letter or a copy of your billing screen as evidence of timely submission. Save the acceptance report from your clearinghouse at the time of submission, not after the denial.
How do I submit claims and check status with Wellcare?
Wellcare prefers electronic claims through EDI in ANSI ASC X12N version 5010A, sent through your existing clearinghouse or one Wellcare uses. The payer ID and clearinghouse list are in the state Quick Reference Guide at go.wellcare.com, not in the manual itself.
Availity Essentials, a multi-payer portal, is how Wellcare asks providers to handle routine work. The manual lists eligibility and benefits inquiries with ID card access, authorizations and referrals, claim and encounter submission, real-time claim responses, claim corrections and remittance advice. Wellcare's own secure provider portal at go.wellcare.com stays active.
Wellcare's remittance is called an Explanation of Payment, or EOP. Providers can view and download EOPs from PaySpan after registering at payspanhealth.com.
How do I send a corrected or voided claim to Wellcare?
Send frequency code 7 to replace a claim or 8 to void it, and put Wellcare's original claim number in the reference field. Without both, Wellcare treats the submission as a new first-time claim.
| Item | Electronic 837 | Paper |
|---|---|---|
| Frequency code | Loop 2300 CLM05-3 is 7 (replace) or 8 (void) | Institutional: third character of the type of bill, UB-04 box 4. Professional: left-justified in CMS-1500 box 22 |
| Original claim number | Loop 2300 REF01 is F8 and REF02 holds the original Wellcare claim number | Institutional: UB-04 box 64. Professional: include the original claim number with the frequency code |
| Written note | Not a substitute for the codes | Writing or stamping "corrected claim" without the code and number makes it an original claim |
The correction runs as two transactions. Wellcare reverses the original claim as "Payment lost/voided/missed," then pays the new one as "Adjusted per corrected bill" under a new claim number. The reversal can show as a negative amount on a later EOP than the one for the corrected claim, so reconcile the two remittances together.
Use these codes only on a claim that was accepted and processed. The manual says they are not for original submissions or rejected claims. Frequency codes follow the rules covered in CMS-1500 vs UB-04.
What are the Wellcare appeal levels and deadlines for a denied claim?
A participating provider has 90 calendar days from Wellcare's original utilization management review decision or claim denial to file a provider appeal, and appeals filed later are denied for untimely filing. Wellcare then has up to 60 calendar days to decide it after receiving all required documentation.
- Claim payment dispute. This covers claim denials such as untimely filing, incidental procedures, bundling, unlisted procedure codes and non-covered codes, and the manual also sends denials for no prior authorization, benefits exhausted, service exceeds authorization and authorization expired to this process. File it within the timeframe in your contract and any state-specific requirement, using the dispute form, preferably through the Wellcare Provider Portal.
- Provider reconsideration (appeal). It addresses authorization denied, medical necessity not established, denied after medical review and similar utilization decisions, such as codes DN004, DMNNE and DN019. File within 90 calendar days using the reconsideration form, preferably through the portal, or by mail or fax.
- Missing records window. If the first appeal lacked medical documentation, you have 60 calendar days from that lack-of-information denial to supply it. Records received after that are not reviewed and the appeal stays closed.
- Member reconsideration. This is the member's right, or a representative's or treating physician's with consent. It must be requested within 65 days of the Notice of Action. If Wellcare upholds the denial, it forwards the case file to the CMS independent review entity (IRE), which has 30 days for a standard pre-service appeal and 60 days for a retrospective one.
Include the date of service, place of service, denial reason code, member ID, claim number, authorization number if any, billed charges, your statement of the reason and the supporting documents. Wellcare says a dispute without the needed documentation is upheld for lack of information. Providers may not charge Wellcare or the member for copies of medical records requested for this purpose.
Wellcare also describes a reopening, which is a written request to change a binding determination for an overpayment or underpayment. Reopenings are the plan's discretion and are distinct from appeals and disputes. For the federal Medicare Advantage rules behind these steps, see Medicare Advantage billing, and Medicare appeal levels covers Original Medicare.
How do I check eligibility and submit prior authorization for Wellcare members?
Check eligibility in Availity Essentials, then look up whether the service needs authorization with the Authorization Lookup tool at go.wellcare.com. Availity Essentials is the preferred and fastest way to submit a request, and fax or phone are also offered depending on service and urgency.
Wellcare says failure to get authorization before an elective or non-emergency service is grounds for denial of a post-service authorization request or claim. Non-participating providers and facilities need authorization for all services except family-planning education and counseling, in-office family-planning visits, childhood immunization administration, and emergency transportation and services. Non-participating providers are reimbursed at 100% of the Medicare rate in effect on the date of service.
If you appeal a denial for no authorization, send documentation of why the service was rendered without it. Our prior authorization service and eligibility verification service cover the front-end checks.
What does Wellcare require from hospitals and EDs?
| Topic | What the manual says |
|---|---|
| Admission notification | Notify Wellcare by the next business day of a member's observation or inpatient admission to a hospital. Failing to may lead to denial of the inpatient authorization or the claim. This covers acute care hospitals including critical access and behavioral health hospitals, inpatient rehabilitation facilities, long-term acute care hospitals and skilled nursing facilities |
| How to notify | Fax, phone or the secure portal at go.wellcare.com. Include the member's name, date of birth and ID, the facility name and the admitting diagnosis |
| Emergency services | Covered inside or outside the network, with or without prior authorization, judged by the prudent layperson standard whatever the final diagnosis. Provider contracts may not tell providers to seek authorization before the member is stabilized |
| Unrelated care during an ED visit | Wellcare is not responsible for care of an unrelated non-emergency problem during emergency treatment |
| Advanced imaging | ER, observation and inpatient imaging do not need authorization, and Evolent authorization is not required for services in the ED, on an inpatient basis or with a surgery. Authorization or admission notification is still required through Wellcare, and a missing one can deny all or part of the claim |
| Observation notice | Contracted hospitals and critical access hospitals deliver the Medicare Outpatient Observation Notice when a member has more than 24 hours of observation, no later than 36 hours after observation starts, or at release if sooner |
How do I dispute an out-of-network underpayment from Wellcare?
Wellcare's 2026 Medicare Advantage manual does not describe open negotiation or federal IDR, so use the plan's dispute and reconsideration steps for plan-level issues and the No Surprises Act process for federal rights. Do not assume one replaces the other.
Start with open negotiation, then the IDR process when the rules allow it. Our out-of-network billing and IDR service handles both together. Keep the Wellcare 90-day appeal clock and the federal deadlines on separate calendars.
Which portal or clearinghouse do I use with Wellcare?
Use Availity Essentials for eligibility, authorizations, claims, claim correction and remittances, and use your clearinghouse for X12 5010A file exchange. The Wellcare Provider Portal at go.wellcare.com is the preferred place to file the dispute and reconsideration forms.
The manual notes that affiliated Medicare brands, including Allwell and 'Ohana Health Plan, moved to the unified Wellcare brand on Jan. 1, 2022. State Quick Reference Guides give contact details by state.

01What is the Wellcare timely filing limit for claims?
Wellcare's 2026 Medicare Advantage manual gives participating providers 180 calendar days from the date of discharge for inpatient services or the date of service for everything else, unless the provider agreement says otherwise. A footnote lists 95 days for Texas health plans. Corrected and voided claims fall under the same limit.
02What are the Wellcare appeal levels and deadlines for a denied claim?
Providers have 90 calendar days from the original decision or claim denial to appeal, and Wellcare has up to 60 calendar days to decide. Payment disputes follow your contract timeframe and state rules. Member reconsiderations must be requested within 65 days, and upheld cases go to the CMS independent review entity.
03How do I check eligibility and submit prior authorization for Wellcare members?
Use Availity Essentials to check eligibility and submit the request, after checking the Authorization Lookup tool at go.wellcare.com. Fax and phone are also available. Skipping authorization for an elective or non-emergency service is grounds for denial, and non-participating providers need authorization for nearly all services.
04What are the most common Wellcare denial reasons for hospital and ER claims?
The manual lists no prior authorization, benefits exhausted, service exceeds authorization, authorization expired, medical necessity not established, untimely filing, bundling and non-covered codes. Reconsideration codes also include denied after medical review and medical records required, so send the records with the first appeal.
05How do I dispute an out-of-network underpayment from Wellcare through open negotiation and IDR?
Wellcare's manual does not cover federal IDR. Use the plan's dispute or reconsideration process inside its timeframes, and use the No Surprises Act open negotiation and IDR steps for federal rights. Non-participating providers are paid at 100% of the Medicare rate, which is the figure to compare against.
06Which portal or clearinghouse do I use to submit claims and check status with Wellcare?
Use Availity Essentials for claims, status, corrections, authorizations and remittance advice, and send X12 5010A files through your clearinghouse. Wellcare's own Provider Portal at go.wellcare.com remains active and is the preferred route for dispute and reconsideration forms. State Quick Reference Guides list payer IDs and clearinghouses.
