Revenue Code 0762: Observation Hours

Revenue code 0762 is the UB-04 line for observation. Medicare tells hospitals to report observation under 0760 or 0762 and to count the time as units of HCPCS G0378, one unit per hour.

In short: 0762 reports observation room charges on an outpatient claim, with G0378 and one unit per hour rounded to the nearest hour. Time runs from the physician's order to the end of observation care. The line is not allowed on 12x bills, and comprehensive APC 8011 needs at least 8 hours.

How Are Observation Hours Reported Under Revenue Code 0762?

The NCHS codebook labels 0762 as treatment or observation room, observation room, effective September 1993. Its neighbours are 0760 general classification, 0761 treatment room and 0769 other. Chapter 4 section 290.2.1 says hospitals are required to report observation charges under 0760 or 0762, and that other ancillary services during the stay use their own revenue and HCPCS codes.

Medicare defines observation as a well-defined set of clinically appropriate services, including short-term treatment, assessment and reassessment, furnished while deciding whether a patient needs inpatient admission or can go home. It is ordered by a physician or another person authorized to admit or order outpatient services. For the line between observation and an inpatient stay, see the two-midnight rule.

How Do I Count the Units?

Units on the observation HCPCS code are hours, rounded to the nearest hour. Chapter 4 section 290.2.2 gives the example of a patient who starts observation at 3:03 p.m. by the nurses' notes and leaves at 9:45 p.m.: the line shows 7 units. The HCPCS code is G0378, hospital observation service, per hour.

  • Start: the clock time in the medical record that matches the physician's order for observation.
  • End: when all medically necessary services related to observation are done. It may include follow-up care after the discharge order, but not time spent waiting for a ride home.
  • Interruptions: observation is not billed at the same time as procedures with active monitoring, such as colonoscopy or chemotherapy. The hospital may add the separate periods together or deduct the average length of the procedure.
  • Spanning midnight: all hours go on one line, dated the day observation began.

Recovery after surgery is not observation. Chapter 4 says standing orders for observation after all outpatient surgery are not recognized, and routine postoperative monitoring of 4 to 6 hours is billed as recovery room services.

How Does 0762 Sit on the Claim?

Only observation billed on a 13x bill type can be considered for payment under the comprehensive APC. Chapter 25 requires a reason for visit on bill types 013x and 085x when 0762 is reported with an FL 14 priority of 1, 2 or 5, the same trigger as the 045x family covered on 0450.

The related visit codes matter. Section 290.5.3 says G0378 has status indicator N, which means its payment is always packaged, and comprehensive APC 8011 can pay the whole encounter when conditions are met. A hospital reports G0379, direct referral for observation, only when a patient is sent straight to observation by a community physician with no ER, clinic or critical care visit that day.

Section 290.5.3 sets the APC 8011 test: observation time documented, at least 8 hours of G0378, a Type A or B emergency department visit (99281 to 99285 or G0380 to G0384), clinic visit G0463, critical care 99291 or G0379 with the observation, no T or J1 status procedure on the claim, and a physician who assessed the patient's risk. Short stays that miss the test are packaged.

Where Does Medicare Block 0762?

Section 240.1 of Chapter 4 lists 0762 in the revenue codes not allowed on type of bill 12x, and section 240 explains that observation services, clinic visits and emergency visits require an outpatient status. The remittance carries group code PR, CARC 96 and RARC M28. Chapter 4 also says that in only rare and exceptional cases does reasonable and necessary observation run past 48 hours, and usually the decision is made in under 24.

What Does a 0762 Line Look Like on the Form and the Remittance?

The observation line has 0762 in FL 42, G0378 in FL 44 and the hours as units in FL 46, rounded to the nearest hour. Observation goes on bill type 13x.

A simplified facility claim with form locator 42, the revenue code, and form locator 44, the HCPCS code and modifiers, highlighted
Simplified, not the official form. FL 42 holds 0762; FL 44 holds the HCPCS code or rate.

0762 is not allowed on a 12x bill, and the remittance shows PR-96 with M28 for it. A CO-16 with M51 points to a missing or invalid code, and M53 to missing or invalid days or units. Since G0378 is packaged, a zero payment on the G0378 line can be normal and is read with the visit line.

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01How are observation hours reported under revenue code 0762?

Under 0762 with HCPCS G0378 on the same line, and one unit for each hour of observation, rounded to the nearest hour. The clock starts at the documented time of the physician's order. A stay of 3:03 p.m. to 9:45 p.m. is 7 units, per Chapter 4 section 290.2.2.

02Do I use 0760 or 0762?

Chapter 4 allows either: it says hospitals are required to report observation charges under 0760, general classification, or 0762, observation room. The manual does not choose between them. Follow your chargemaster and be consistent, because the payer reads the HCPCS code and units for payment.

03What if observation runs past midnight?

Put all the hours on one line. Chapter 4 section 290.2.2 says the date of service for that line is the date observation care begins. Do not split the stay into one line per calendar day, or the hour count and the line dates will disagree.

04How many hours do I need for APC 8011?

At least 8 hours of G0378 under section 290.5.3, along with one of the qualifying visits, no T or J1 status procedure, and a physician evaluation documented in the record. Claims that fall short are packaged into other payments, not paid separately, so count the hours before submitting.

05Can 0762 be billed on an inpatient claim?

Not on a 12x Part B inpatient claim. Section 240.1 of Chapter 4 lists 0762 among the revenue codes not allowed on type of bill 12x. Observation is an outpatient status, so it belongs on a 13x claim, never on the inpatient Part B bill.
