What does CPT 66984 report?

66984 reports removal of a cataract by the extracapsular route, with an intraocular lens (IOL) placed in the same operation, in a routine case. This page paraphrases the AMA CPT descriptor. CMS labels the code "Xcapsl ctrc rmvl w/o ecp", and the more difficult case is 66982, labeled "Xcapsl ctrc rmvl cplx wo ecp".

How does the 90-day global period work for cataract surgery?

A 090 code is a major surgery with a 90-day global period, and chapter 12 of the Medicare Claims Processing Manual counts 1 day before the surgery, the day of surgery and the 90 days after it. Related follow-up visits in that window are inside the surgeon's payment.

  • Included: preoperative visits after the decision to operate, related postoperative visits, and complications that do not need a return to the operating room.
  • Not included: the visit that first decided on surgery, unrelated visits, diagnostic tests, and treatment of a complication that needs a return trip to the operating room.

The visit that decides on cataract surgery on the day before or day of surgery is billed as an E/M with modifier 57, not 25 (chapter 12, section 40.2). Premium lenses and injections are on ophthalmology billing.

How do modifiers 54 and 55 split the payment?

When two physicians agree to transfer postoperative care, the surgeon bills 66984-54 for surgical care only and the other physician bills 66984-55 for postoperative management only. Both claims carry the same date of service and the same code, and the date care changed hands goes in item 19 of the CMS-1500 or the matching 837P free-text segment.

The file's shares make the arithmetic plain. With 10 percent pre, 70 percent intra and 20 percent post, a surgeon billing -54 is paid the combined 80 percent. Section 40.4 divides the postoperative 20 percent by the days each doctor was responsible. A written transfer agreement must be kept in the chart.

The comanaging doctor uses modifier 24 for any unrelated visit.

How do I bill the second eye inside the first eye's global period?

Add modifier 79 and the eye modifier to the second surgery. Chapter 12, section 40.2 defines 79 as an unrelated procedure by the same physician during the postoperative period, and says a new postoperative period begins when it is billed. WPS billing article A59805 adds that a procedure can be unrelated because it is performed in the other eye.

The same article says cataract claims must show laterality with LT or RT. So a left eye done 6 weeks after a right eye is 66984-79-LT. See modifier 79, RT and LT.

Chapter VIII of the NCCI Medicare Policy Manual allows only one cataract code in the 66830 to 66991 range per eye. Modifier 79 does not permit a second extraction on the same eye. A related return to the operating room is modifier 78.

How does 66984 sit on the claim, and what bundles into it?

A surgeon bills 66984 on the CMS-1500 with the eye modifier and the matching diagnosis. An ASC bills its facility payment through the ASC X12 837 professional format or, rarely, the CMS-1500, according to chapter 14 of the Claims Processing Manual. A hospital outpatient department bills on the UB-04.

  • NCCI chapter VIII: an iridectomy done to complete the extraction and the minimal vitreous loss of a routine extraction are not reported separately. A distinct trabeculectomy for glaucoma can carry a PTP-associated modifier.
  • NCCI chapter VIII: antibiotic or steroid injection during cataract surgery is not reported separately, including 67028.
  • Both eyes the same day: indicator 1 means 150 percent of one fee, and chapter VIII asks for modifier 50 with one unit on a single line.

Which fields and modifiers belong on a cataract claim?

Cataract surgery with lens insertion is one line per eye, 66984 in Item 24D with RT or LT, 1 unit in Item 24G and a pointer in Item 24E to the cataract diagnosis. RVU26D shows a 090 global, so a second eye inside the first eye's period needs the modifiers the page describes. Item 24A holds the surgery date.

On a hospital outpatient bill, the revenue code is in FL 42, 66984 in FL 44 with the side modifier, the date in FL 45 and units in FL 46. The MUE table allows 1 per date with indicator 2.

Example: The right eye is operated on 10/08 and the left on 10/22. Claim 1 is 66984 RT, pointer A (H25.11), 1 unit, $2,100.00. Claim 2 is 66984 LT with the modifiers for a second procedure inside the global of the first. A line with no side modifier cannot be matched to an eye.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01How do I bill cataract surgery with lens insertion?

Bill 66984 for a routine case or 66982 for a complex one, once per eye, with RT or LT. Related visits in the 90-day global period are included. Use 54 and 55 only when care is formally transferred, and 79 for the other eye.

02What is the global period for 66984?

090. The October 2026 fee schedule file gives 66984 a 90-day global period, counted as the day before surgery, the day of surgery and 90 days after, per chapter 12 of the Medicare Claims Processing Manual. Related visits are not paid separately.

03Do I need modifier 79 for the second eye?

Yes, when the second eye is operated inside the first eye's global period. Chapter 12 defines 79 as an unrelated procedure in the postoperative period, and WPS article A59805 says it can be unrelated because it is the other eye. Add RT or LT to both claims.

04What do modifiers 54 and 55 do on cataract surgery?

54 reports surgical care only and 55 reports postoperative management only. Both claims carry the same date and code, and the transfer date goes in item 19. For 66984 the file's shares make -54 worth 80 percent, pre plus intra operative.

05Can an assistant surgeon be paid for 66984?

Not on Medicare. RVU26D gives 66984 an assistant at surgery indicator of 1, which it defines as a statutory payment restriction, and a co-surgeon indicator of 0, meaning co-surgeons are not permitted. Other payers can follow different rules, so check the contract before billing an assistant.

06Is 66984 limited to one unit?

Yes. The October 2026 MUE table sets 1 unit per date of service with indicator 2, a policy edit. NCCI chapter VIII also allows only one code from 66830 to 66991 per eye, so a repeat on the same eye is not payable.

Second-eye cataract claims denied?

We check 79, RT and LT and the global dates before the claim goes out.
