CPT 59510: cesarean delivery, global package

59510 reports a whole pregnancy episode that ends in a cesarean: routine antepartum care, the cesarean delivery and postpartum care by the same physician or group. It follows the maternity global period MMM, and the NCCI manual applies global surgery rules to it, which is where cesarean claims differ from 59400.

In short: 59510 is the global package for a cesarean delivery with antepartum and postpartum care, billed once. Because a cesarean resembles surgery, closure of the incision, control of bleeding and complications that need no return to the operating room are part of it. A return to the operating room is reportable with modifier 78.

What does CPT 59510 report?

59510 reports total obstetric care when the delivery is a cesarean: antepartum care, the surgical delivery and postpartum care, billed as one package. NCCI Policy Manual Chapter VII, section G.1, names it with 59400 as a total obstetrical package, and section G.2 lists it among the delivery codes that include fetal monitoring, episiotomy and delivery of the placenta.

How does the cesarean package differ from the vaginal one?

The package structure is the same as 59400, but the work value is higher and the surgical rules apply more directly. In the fee schedule file 59510 carries a work RVU of 41.05 against 37.00 for 59400, and a total of 74.05 against 66.30.

59610Vaginal after a previous cesarean38.7169.78
59618Attempted vaginal birth after a previous cesarean (cesarean performed)41.5774.86

When a trial of labor after a previous cesarean ends in a cesarean, the package is 59618, not 59510. NCCI Chapter VII describes 59618 as routine obstetric care with cesarean delivery after a previous cesarean.

Which surgical rules apply to a cesarean package?

Section G.4 says maternity procedures with an MMM period are subject to the global surgery and anesthesia rules, and the same codes bundled into surgery with a 000, 010 or 090-day global are bundled into them. Chapter VII, section I, then adds the points that matter for a cesarean.

  • Wound repair codes 12001 to 13153 are not reported to close the surgical incision for an MMM code.
  • Postoperative control of bleeding that does not need a return to the operating room is included, and control of bleeding that does need one is reportable with modifier 78.
  • Bladder catheter insertion 51701 to 51703 is not reported with some MMM procedures.

Chapter 12, section 40.1, lists complications that do not require additional trips to the operating room as part of a global surgical package. The same chapter says critical care 99291 and 99292 can be payable separately in some situations.

For a teaching hospital, section 100.1.6 requires the teaching physician to be present for the delivery. Our OB/GYN billing page covers the rest of the pregnancy claim.

How does 59510 sit on the claim?

Report one unit on one CMS-1500 line, with item 24B set to 21 when the patient is a registered inpatient, as chapter 26 describes. The anesthesia provider bills separately: 01961 is the cesarean-only anesthesia code, and the file gives anesthesia codes status J, meaning no RVUs. See anesthesia billing.

The assistant-at-surgery indicator is 1, so an assistant at the cesarean is not paid under the statutory restriction. The delivery-only code 59514 differs: its file row shows an assistant indicator of 2, which means an assistant may be paid, and a co-surgeon indicator of 1.

For a cesarean after a failed trial of labor, use 59618 in place of 59510, and check that the note supports the attempted vaginal birth.

What fields carry a cesarean package?

A cesarean package is one line, 59510 in Item 24D with 1 unit in Item 24G and the delivery date in Item 24A. Item 24E points to the delivery diagnosis. RVU26D gives it a global indicator of MMM, the maternity value. An assistant surgeon bills on a separate claim, and RVU26D shows an assistant indicator of 1, meaning an assistant may not be paid.

Simplified CMS-1500. The cesarean package line points to the delivery diagnosis in Item 21.

On an inpatient facility claim, Chapter 25 places the principal procedure and date in FL 74. The MUE table allows 1 per date with indicator 2.

Example: A patient with a low transverse scar from a prior cesarean has a planned cesarean on 10/14. The line is 59510, pointer A (O34.211), 1 unit, $4,300.00. A claim for a vaginal delivery code beside it would be a code mismatch and a payer may deny it under CO-97.

Why does a 59510 claim deny?

CARCWhat X12 saysWhat usually causes it
97 with RARC M144Benefit included in the allowance for another service already adjudicated; M144: pre-/post-operative care payment is included in the allowance for the surgery/procedureVisits during the postpartum period billed on top of the package
97 with RARC N19Benefit included in another service; N19: procedure code incidental to primary procedureIncision closure or catheter insertion billed as a separate procedure
18Exact duplicate claim/service (use only with group code OA except where state workers' compensation regulations require CO)The package billed by two groups, or billed again after a split claim
50These are non-covered services because this is not deemed a 'medical necessity' by the payerA record that does not state the indication for the cesarean

If these denials keep coming back, our denial management team can work them.

Still have a question?

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01What does the cesarean global package 59510 include?

Antepartum care, the cesarean delivery and postpartum care. NCCI Chapter VII lists fetal monitoring, episiotomy and delivery of the placenta as included, and the surgical rules bundle incision closure, bleeding control and complications that need no return to the operating room. Ultrasound and unrelated visits stay outside.

02Can I bill wound repair codes with a cesarean?

No. NCCI Chapter VII, section I, says wound repair codes 12001 to 13153 shall not be reported to describe closure of surgical incisions for procedures with global indicators of 000, 010, 090 or MMM. Closure is part of the delivery.

03What if the patient returns to the operating room?

NCCI Chapter VII says control of bleeding that requires a return to the operating room in the postoperative period is separately reportable with modifier 78. Complications that do not require a return are included in the package under Claims Processing Manual chapter 12, section 40.1.

04How is a cesarean after a failed trial of labor billed?

Use the code for attempted vaginal birth after a previous cesarean, 59618, not 59510. The October 2026 file gives it a work RVU of 41.57 against 41.05 for 59510. Both are MMM codes with status A, and both have an MUE of 1.

05Can an assistant surgeon bill with 59510?

Not for Medicare. The assistant-at-surgery indicator for 59510 is 1, a statutory restriction, so an assistant may not be paid. The delivery-only code 59514 carries indicator 2, which means an assistant may be paid. Commercial payers set their own rules.
