CPT 20610: Large Joint Injection or Aspiration Without Ultrasound

20610 reports a needle placed in a larger joint or its bursa to draw fluid out or put medicine in, with no ultrasound guidance. Its pair, 20611, is the same service with ultrasound guidance, and Medicare counts one unit per joint rather than one per needle stick.

In short: 20610 is billed once per joint, bursae included, and carries a 000-day global period. That global is why a visit on the same day needs modifier 25, and why 20610 and 20611 are never both reported for one joint.

What Does CPT 20610 Report?

20610 is the procedure code for arthrocentesis of a larger joint or its bursa, meaning a needle is placed to remove fluid, inject a drug or both. The 2026 NCCI Policy Manual, Chapter IV, describes 20600 to 20611 as one family that differs by joint size and by whether ultrasound guidance is used, and its own example for 20610 is a shoulder.

The CMS file labels the pair "Drain/inj joint/bursa w/o us" (20610) and "w/us" (20611).

The rest of the family, 20600 to 20606, covers other joint sizes.

Should You Report 20610 or 20611?

Choose by what happened at the bedside: 20611 when ultrasound guided the needle and the note says so, 20610 when it did not.

Do not report both for one joint. NCCI Chapter IV says a provider shall not report more than one unit of service for arthrocentesis of any one joint, whether or not its surrounding bursae are also aspirated or injected.

Example (from NCCI)

A shoulder and two bursae of that shoulder injected without ultrasound are 1 unit of 20610, because the unit of service is the joint together with its bursae.

How Many Units of 20610 Can You Bill, and How Are Both Sides Reported?

The unit is the joint with its bursae, so two different large joints on one date, such as a knee and a shoulder, are two units. The practitioner MUE table effective October 1, 2026 lists 20610 at 2.

The same joint on both sides follows the bilateral rule. Chapter 12, section 40.7 B, says to report a procedure not described as bilateral with modifier 50 on a single line, and NCCI Chapter IV adds that one unit is used. An ambulatory surgery center instead reports two lines of one unit each with modifier LT or RT.

Can You Bill a Joint Injection with an E/M Visit?

Yes, but only when the visit is significant and separately identifiable from the injection. 20610 has a 000 global, so NCCI Chapter I treats it as a minor procedure: the decision to inject is part of the injection and cannot be billed as a visit.

A separate visit goes on the claim with modifier 25. NCCI Chapter I says the visit and the minor procedure do not need different diagnoses, but the work must differ from the usual pre-injection check. Because the global is 000, Chapter 12, section 40.1 C, adds that visits after the day of the procedure are not included in the injection payment.

Example (Made Up)

A patient returns for a scheduled steroid injection and the note records only the usual history, consent and the injection. Bill 20610 alone. If the physician also works up new chest tightness, that evaluation carries modifier 25.

What If the Joint Is Also Operated On?

Chapter IV, item 17, says arthrocentesis (20600 to 20611) shall not be reported with an open or arthroscopic procedure on the same joint. If the injection is in one joint and the open or arthroscopic procedure is on a different joint, the injection may be reported separately.

How Does 20610 Appear on the Claim?

On the CMS-1500, Item 24D carries 20610 (modifier 25 sits on the visit line, modifier 50 on the injection line for both sides), Item 24G carries the units, and Item 24B carries the place of service: 11 for the office, 22 or 19 for a hospital outpatient department, 24 for an ambulatory surgical center.

The injected drug is reported with its own HCPCS code. Chapter 12, section 20.4.4, says drugs are not supplies and may be paid incidental to physicians' services, and the drug waste modifiers JW and JZ have their own pages.

Which Fields and Modifiers Belong on a 20610 Line?

A joint injection with no imaging takes 20610 in Item 24D with RT or LT, 1 unit per joint in Item 24G and a pointer in Item 24E to the joint diagnosis. A second joint is a second line or a second unit as the payer instructs, and the chart must name each joint. A drug supply charge goes on its own line.

RVU26D gives 20610 status A, a 000 global and a bilateral indicator of 1. The MUE table allows 2 per date with indicator 3. A visit billed beside it takes 25 only when it is separate.

Example: On 10/18 the clinician injects the right knee and the left shoulder. Line 1 is 20610 RT, pointer A (M17.11), 1 unit, $110.00. Line 2 is 20610 LT, pointer B (M25.512), 1 unit, $110.00. The note names each joint and the drug lot.

Ask a billing lead directly. A 20-minute call, Monday to Friday, 9am to 5pm Eastern.

01What does CPT 20610 mean?

It reports arthrocentesis of a larger joint or bursa without ultrasound guidance: a needle removes fluid, delivers a drug or does both. The NCCI manual uses the shoulder as its example. Ultrasound-guided work on the same size of joint is reported with 20611 instead.

02Can I bill a joint injection with an E/M visit?

Yes, if the visit is significant and separately identifiable, with modifier 25 on the visit line. NCCI says the decision to perform a minor procedure is included in it, and the visit does not need a different diagnosis. Routine pre-injection assessment alone does not qualify.

03How many units of 20610 can be billed?

One unit per joint, including its surrounding bursae. NCCI says injecting a shoulder and two of its bursae is still 1 unit. The practitioner MUE for 20610 is 2 as of October 1, 2026, which fits two different large joints on one date.

04What is the difference between 20610 and 20611?

Guidance. 20610 is performed without ultrasound and 20611 is performed with ultrasound guidance. The October 2026 CMS file shows work RVUs of 0.77 and 1.07. Report one code of the pair per joint and do not add both for the same joint.

05Can I bill 20610 during an arthroscopy?

Not for the same joint. NCCI Chapter IV says arthrocentesis shall not be reported with an open or arthroscopic procedure on the same joint. If the injection is in a different joint from the surgery, it may be reported separately.
