How does Medicare OPPS pay hospital outpatient claims?
Medicare OPPS pays a hospital outpatient claim one line at a time. Every HCPCS code carries a status indicator that says whether the line is paid on its own, packaged into another line, or paid under a different system, and each separately paid code sits in an APC with a relative weight. Multiply the weight by the calendar year conversion factor, $91.415 for CY 2026, and you have the national payment before the wage index.

In short: a $0 line on an outpatient remittance is often correct, because packaged services are paid through another line. The work is knowing which zeros are packaging and which are claim errors.
How is one outpatient claim paid, line by line?
The claim is read as a list of services, not as a visit total. The OPPS rule page defines the system, and this is the order in which a claim moves through it.
What does each status indicator do to a line?
The indicators below are the ones Chapter 4 of the Claims Processing Manual defines in its payment rules. The full list is in Addendum D1 of each year's OPPS rule, and the indicator for each code is in Addendum B.
| Indicator | What the manual says | What billing should check |
|---|---|---|
| A | Paid under a fee schedule or payment system other than OPPS | Do not expect an APC amount. Check which schedule applies. |
| N | Paid under OPPS but packaged into a separately paid service | Report it anyway. A $0 line is expected. |
| T | Paid separately, with a multiple procedure reduction when two or more are billed on the same date | Check the reduction on the second and later lines. |
| Q1 | Paid separately only if the claim has no service with indicator S, T or V; otherwise packaged | Look for an S, T or V line on the same claim. |
| Q2 | Paid separately only if the claim has no T service; otherwise packaged | Look for a T line on the same claim. |
| Q3 | Mapped to a composite APC | Expect one payment for the combined service. |
| Q4 | Laboratory code on the clinical laboratory fee schedule, packaged with J1, J2, S, T, V, Q1, Q2 or Q3 lines | Check what else is on the claim before disputing a $0 lab line. |
| J1 and J2 | Comprehensive APC primary services, and the observation comprehensive APC 8011 | Expect adjunctive services to be bundled into the primary one. |
Where a claim holds several STV-packaged codes and no S, T or V procedure, section 10.4.1 says separate payment is made for the one assigned to the highest paid APC, and the others are packaged into it.
What does an ED visit with two procedures pay?
Take a Type A emergency department visit billed under revenue code 0450 with two procedures and a supply charge. The weights are invented. The conversion factor is the CY 2026 national figure, before the wage index. The visit line is assumed to be paid separately; Chapter 4 says ED and clinic visits are paid separately in some cases and as part of a composite APC in others.
Visit weight 2.0000, procedure A weight 4.0000, procedure B weight 4.0000. Both procedures carry status indicator T. The supply line carries N.
Chapter 4, section 10.5 says the full amount is paid for the surgical procedure with the highest weight and 50 percent for any other surgical procedure performed at the same time. The wage index then moves the national figure up or down for the hospital's area.
One real anchor: for CY 2026, CMS set the weight of APC 5012 (Level 2 Examinations and Related Services, which holds G0463, the clinic visit code) at 1.00. Its national unadjusted payment is therefore the conversion factor itself, $91.415 before the wage index.
Why did an outpatient line pay $0?
Check the reason before you appeal. Some zeros are packaging, and some are claim errors that need a corrected bill.
| What you see | Likely cause | What to do |
|---|---|---|
| N line, CARC 97 (benefit included in the payment for another service) | Packaged by design | Nothing. Keep the charge on the claim. |
| Q1, Q2 or Q4 line at $0 | A qualifying S, T or V line or a T line is on the same claim | Confirm the other line is on the claim and paid. |
| Claim returned for a missing line date | FL 45 empty on a line with a HCPCS code | Add the date of service to every line and resubmit. |
| 045x or 063x line on a 12x bill, PR group code, CARC 96, RARC M28 | Revenue code not allowed on that bill type | Check the bill type. 12x is inpatient Part B and outpatient hospital claims use 13x. |
| CARC 4 | The procedure code is inconsistent with the modifier used | Fix the modifier on that line. |
Do not drop packaged lines to make the claim look cleaner. Chapter 4, section 10.4 says charges for packaged services are used for outlier payments and future rate setting, so hospitals should report all HCPCS codes and all charges, whether the service is separately paid or packaged.
How is observation paid under OPPS?
Observation is billed in hours on revenue code 0762 with HCPCS G0378, and G0378 itself is packaged. Because G0378 is packaged, it earns nothing on its own. Payment comes through the observation comprehensive APC 8011 when the claim qualifies.
- Report G0378 units equal to the hours, rounded to the nearest hour. Chapter 4 shows 3:03 p.m. to 9:45 p.m. as 7 units.
- The units must equal or exceed 8 hours for APC 8011.
- The claim needs a qualifying visit code, such as an ED visit (99281 to 99285 or G0380 to G0384), G0463 or critical care 99291.
- No procedure with indicator T and no J1 service can be on the claim.
- Only observation billed on a 13x bill type is considered.
If a period of observation crosses midnight, all the hours go on a single line dated with the day observation began. The revenue code 0450 page covers the ED lines that sit beside it.
When do OPPS rates change, and which year applies?
Rates follow the date of service. The CY 2026 final rule (90 FR 53448, effective January 1, 2026) set the conversion factor at $91.415, or $89.632 for hospitals that fail the Hospital OQR Program requirements. CMS also updates the OPPS quarterly for new pass-through drugs and devices and new HCPCS codes.
For dates of service in 2027, take the weights and conversion factor from the CY 2027 final rule and its addenda, not from the CY 2026 numbers above. The UB-04 revenue and condition codes page covers the form locators, and the facility vs professional billing guide covers what the physician bills on the CMS-1500.

01How does Medicare OPPS pay hospital outpatient claims?
OPPS pays by service. Each HCPCS code has a status indicator and, if separately paid, an APC with a relative weight. The national payment is the weight times the conversion factor, then adjusted for the hospital's wage index and any reductions. For CY 2026 the conversion factor is $91.415.
02Why does a packaged line show a $0 payment?
A status indicator N line is paid under OPPS but packaged into another separately paid service, so it carries no payment of its own. The remittance usually shows CARC 97, meaning the benefit is included in the payment for another service. Keep the line and its charge on the claim.
03How many hours of observation does APC 8011 need?
The units reported with HCPCS G0378 must equal or exceed 8 hours. The claim also needs a qualifying visit code, no T or J1 procedure, and a 13x bill type. G0378 itself is packaged, so a stay that misses a condition earns no separate observation payment.
04How are multiple procedures paid on one outpatient claim?
For multiple surgical procedures performed at the same time, Chapter 4 says Medicare pays the full amount for the procedure with the highest weight and 50 percent for each other procedure. Services with status indicator T take this reduction when two or more appear with the same date of service.
